[Clinical study of immediate implant].

OBJECTIVE: To evaluate the clinical result of immediate implant. METHODS: 32 cases underwent immediate implant surgery for 59 implants simultaneously when the teeth or roots extracted. 55 implants were placed with bone grafts or bone substitutes. The second stage operation were performed in the sixth month and ceramic crown were fixed in the seventh to eighth month postoperatively, the follow up were mean 39 months. RESULTS: One implant was lost. Bone resorption of alveolar crest was observed in 4 implants. Mean resorption was 2 mm. CONCLUSIONS: The result of immediate implant is predictable, the advantages are avoiding the alveolar bone resorption after teeth extraction, implant could be placed in an ideal position; less operation trauma, good esthetics results.

[A comparative study of masticatory efficiency and mandibular movement pattern among patients with CD, COD and IOD].

OBJECTIVE: To compare the masticatory efficiency and the character of mandibular movement trace of the patients with complete denture (CD), complete overdenture (COD) and implant-supported overdenture (IOD). METHODS: A total of 42 subjects were rehabilitated with CD, COD and IOD respectively and wore the dentures at least half a year. Each study group was composed of 14 subjects. The masticatory function of patients were investigated by means of masticatory efficiency and mandibular movement trace. The characters of mandibular movement trace were evaluated from four aspects: the regulation of chewing cycle, centralization of end traces, the type of the contact slides and the type of the trace on the frontal plane. RESULTS: The average chewing efficiency of almond and jujube in the IOD patients was higher than that in the CD group (P < 0.01), and the chewing efficiency of almond in COD group was higher than that in the CD group (P < 0.01). In the IOD group, there were more patients with regular chewing cycle (P < 0.05) and centralized end traces (P < 0.01) in mandibular movement than those in the CD group and COD group. CONCLUSIONS: IOD and COD benefit the fully or partially edentulous patients with a relatively good masticatory function. The patients with IOD have more regular mandibular movement than those with CD.

[Clinical study of pure titanium membranes in oral implant surgery].

OBJECTIVE: To evaluate predictability and treatment outcome of the pure titanium membranes in bone defects around endosseous implants. METHODS: 81 patients and 92 dental implants were included in this study. The defects around implants were filled with bone grafts and Bio-Oss, and then covered by titanium membranes. 6 - 10 months later, all of the membranes were removed. RESULTS: 5 patients showed complications in soft tissue healing with exposure of the membranes. Bone defects around 89 implants were successfully covered with new regenerated bone. CONCLUSIONS: Pure titanium membranes show excellent biocompatibility, exhibit good mechanical properties, avoid grafts displacement, ingrowth of connective tissue and contour collapse.

[Application of distraction osteogenesis in severe maxillary hypoplasia secondary to cleft palate].

OBJECTIVE: To evaluate the effect of distraction osteogenesis in correction of severe undeveloped maxilla secondary to cleft palate and to evaluate the selection of distraction modality. METHODS: Distraction osteogenesis was performed upon 8 patients, 6 males and 2 females, aged 11 to 25 years, one with incomplete cleft palate, 4 with unilateral complete left palate, and 3 with bilateral complete cleft palate, all accompanied with severe maxillary hypoplasia. The 7 adult patients were treated by modified high-stepped Le Fort I osteotomy, the other patient, a child, was treated by Le Fort I osteotomy followed by rigid external distraction. X-ray films of skull in lateral, frontal, or panoramic positions, and X-ray film of temporomandibular joint in Schiller's position were taken pre- and postoperatively. In model surgery the accurate distraction distance was measured and the direction of distraction designed. Orthodontic therapy was started immediately after the distraction was completed and the distractor was removed after a period of 4 months' consolidation. RESULTS: The designed distraction was achieved in a11 8 patients. Except in one case with a bone defect 1 cm x 0.5 cm at the osteotomy line in the lateral wall of the right maxillary sinus, dense new bone was formed in the area of distraction in the other cases. The postoperative occlusal relationship was good and stable. No infection and other complication was found. During an average of 20 months' follow-up, the maxilla and occlusion were stable, and no relapse was found. The distance of forward distraction of the maxilla reached an average of l2 mm (5 approximately 15 mm). The SNA angle increased from 71 degree on average preoperatively to an average of 79 degree postoperatively. Both the facial appearance and occlusal relationship returned to normal in a11 the patients. CONCLUSION: Without need of bone grafting and with a stable effect and little influence on platatopharyngeal competence, distraction osteogenesis is an effective method of correcting maxillary hypoplasia secondary to cleft palate and is worthy of spreading.

The masticatory efficiency of mandibular implant-supported overdentures as compared with tooth-supported overdentures and complete dentures.

This study assesses the comparative masticatory efficiency (CME) of mandibular implant-supported overdentures (ISOs) to tooth-supported overdentures (TSOs) and complete dentures (CDs). Three groups of patients in equal numbers were evaluated by assessing chewing efficiency, and the results were correlated with tracings of mandibular excursion (ME) and electromyographic (EMG) computerized analysis. CME was measured by utilizing standardized portions of 2 food staples with fixed masticatory sequencing. In a following session, utilizing the same fixed masticatory sequence, tracings of ME and EMG of the masseter and temporal muscles were recorded and analyzed. The results revealed that the ISO provided the greatest degree of efficiency, followed by the TSO and then the CD group. CME is more correlated to ME and less correlated to EMG.

Clinical study of alveolar vertical distraction osteogenesis for implant.

OBJECTIVE: To evaluate the clinical result of alveolar vertical distraction osteogenesis for implant. METHODS: 19 cases with severe vertical alveolar defects (more than 10 mm defect) underwent vertical distraction procedure before implant placement. 15 cases were male and 4 cases were female. The alveolar bone defects were caused by tumor resection in 10 cases, by trauma in 8 cases, and due to congenital defect in 1 case. The vertical defects in all cases were over 10 mm. X-ray examination were taken prior to operation and in 1 week, 2 week, 4 week, the end of active distraction and before removing distractor. Implants were placed in 17 cases after removing distractors. The prosthesis were finished 6 month later. RESULTS: 11 cases out of 19 had implant prosthesis. 6 cases had implant in the jaw but waiting for the prosthesis. Altogether 65 dental implants were placed, 2 cases did not receive further implant treatment because of cost problem. The study showed that new bone formed after distraction in all 19 cases. Average gained height of new bone was 13 mm. COMPLICATIONS: Unexpected mandible fracture in 2 cases, infection in 1 case. CONCLUSIONS: Alveolar vertical distraction is a good alterative for severe alveolar defects. The distractor should be further developed and modified.

Effects of non-surgical treatment modalities on peri-implantitis.

OBJECTIVE: To evaluate the effects of local-delivery of 25% metronidazol gel and mechanical cleaning using ultrasonic carbon fiber tip on dental implants with peri-implantitis. METHODS: 27 implants with peri-implantitis were randomly assigned to receiving either 25% metronidazol gel treatment or carbon fiber tip ultrasonic scaling. All parameters including plaque index (PLI), probing depth (PD) of pocket, sulcular bleeding index (SBI), and BANA enzyme analysis were measured at baseline, 1, 2, 6 and 12 weeks after treatment. RESULTS: Statistically significant decrease (P < 0.05) in SBI, BANA test and PLI occurred in both treatment groups at all time intervals compared to baseline. PD had a decreasing tendency in both groups, but only metronidazole group reached statistically significant level (P < 0.05) at 2 and 6 week intervals compared to baseline. None of the treatment modalities produced any side effects on the implant and peri-implant tissues. CONCLUSIONS: Both 25% metronidazol gel and mechanical cleaning using ultrasonic carbon fiber tip can be safely and effectively used in the treatment of peri-implant diseases.

[Development of internal distractors for jaws: experimental and clinical studies].

OBJECTIVE: To introduce a series of internal distractors developed by the authors and results of experimental study and clinical trials. METHODS: Based on the demand of clinical use and experimental studies, 7 kinds and 17 types of internal distractors were developed. Required biomechanics were evaluated and animal studies were undertaken in 16 Rhesus monkeys and 12 goats with 40 distractors. Clinically, 68 Patients with 7 kinds of severe dento-gratho-facial deformities were treated with 82 distractors and the histocompatibility, stability, influence on new bone formation were evaluated. RESULTS: The 7 kinds of distractors were State-approved products made of Titanium alloy and permitted for use in human. The pull and twist force tests demonstrated that the loading capability of them were larger than 7.8 kgf and 5.0 kgf respectively. The domestic made Grade II screw was used to transmit the moving, it was nimble, without dead point, and with a forward-backward space of < 0.1 mm. The horizontal space in between the two fixing parts was < 0.1 mm. The parallel degree of the two direction-guide rods was within 0.05 mm. Animal and clinical studies demonstrated that there was no rejection response of the tissues. There were fractures of the direction-guide rods in two early cases, the design was improved then. Other results both experimentally and clinically were satisfactory with good osteogenesis. CONCLUSIONS: This series of domestic made internal distractors can be used satisfactorily in distraction osteogenesis for various kinds of jaw deformities. The successful production of these distractors will provide a good foundation to popularize the internal distraction osteogenesis of the jaws.

Mandibular functional reconstruction using internal distraction osteogenesis.

OBJECTIVE: To study the potential and advantages of internal distraction osteogenesis in mandibular functional reconstruction. METHODS: Three types of mandibular distractors were used in eight patients with various mandibular defects due to tumor or cyst resection. The average age of the patients was 31.5 years old (ages ranged from 8 to 54 years). For two patients with ramus defects, specially designed distractors were used to restore the normal ramus height. In two other patients, specially designed trifocal distractors were used. In three patients, vertical distractors were used. RESULTS: All the mandibles were successfully reconstructed in accordance with the direction and distance designed before operation except in one patient. In that patient the distal 23 mm defect failed to be corrected because of loosening of transport block fixation screws. CONCLUSIONS: Mandibular defects can be successfully corrected using internal distraction osteogenesis. Performing mandibular reconstruction using distraction osteogenesis is best done at the time of tumor or cyst resection.

[A clinical and histological study of Bio-Oss in combination with guided bone regeneration membrane technique].

OBJECTIVE: To evaluate the reliability of Bio-Oss particles as bone grafts in combination with membrane-technique. METHODS: 6 small biopsy specimens were retrieved without causing iatrogenic damage in 6 patients (implant sites). The Donath's hard tissue section and micro grinding technique was adopted, and Leval Laczko staining method was used. RESULTS: The histological results showed Bio-Oss particles were easily distinguished from the newly formed bone, but new bone was formed among different particles of Bio-Oss, no connective tissue was found. Intimate contact between newly formed bone and particles of the graft was present. CONCLUSIONS: Bio-Oss appears to be highly biocompatible and osteoconductive. It can be used success fully as a bone substitute in combination with guided bone regeneration membrane technique.
