Effects of shenmai injection on blood SOD activity and MDA level in senile patients with coronary heart disease.

In order to approach the effects of Shenmai Injection on SOD activity and MDA level in the senile patients with coronary heart disease, 48 cases, who had stenosis of over 70% in more than one branches of the coronary arteries, were assigned randomly into a treatment group (given Shenmai Injection plus the routine treatment) and a control group (given the routine treatment only). The superoxide dismutase (SOD) activity and malondialdehyde (MDA) level were determined before treatment and at the end of a 3-week treatment. The results showed that in the treatment group, the SOD activity was significantly increased (P<0.05) and the MDA level markedly decreased (P<0.01) in the treatment group. It can be concluded that Shenmai Injection may enhancethe antioxidant ability of the senile patients with coronary heart disease.

[Use of direct portal vein puncture portography with modified TIPS treatment in patients with portal hypertension and gastro-esophageal varices bleeding].

OBJECTIVE: To explore a better procedure for transjugular intrahepatic portosystemic shunt (TIPS) in order to improve its safety and to extend its indications. METHODS: To puncture the right portal branch under sonographic guidance in 20 patients with portal hypertension and gastro-esophageal bleeding. The Teflon sheath with gold marker was put into the portal vein; anterior and lateral portography was made, portal pressure was measured and the gastric coronal vein was embolized. The gold marker was put into the portal vein puncture site and the Rups-100 was guided under the gold marker during the TIPS puncture procedure. Anterior and lateral portography was again made to make sure the puncture site was 2 cm away from the portal vein bifurcation. In some cases a 10F sheath was used to suck the thrombosis in the portal vein, and a balloon was used to dilate the parenchyma channel and then a stent was released smoothly. RESULTS: 20 reformed TIPS were successfully performed on all patients and their gastric-esophageal bleedings were controlled immediately. 37 punctures were made in 20 of those cases; the average puncture per patient was 1.85+/-0.67, lower than that of the traditional method. The pressure of the portal vein declined from (30.5+/-1.1) mmHg to (16.9+/-0.9) mmHg, P < 0.05, showing that the difference of portal vein pressure before and after the reformed TIPS was significant. 25 stents were placed, and no complications occurred during the procedure in any of the cases. CONCLUSION: Direct portal vein puncture portography and gold marker guided TIPS procedure is feasible and safe; the indications of TIPS could be further extended.

Safe time to warm ischemia and posttransplant survival of liver graft from non-heart-beating donors.

AIM: To explore the dynamical changes of histology, histochemistry, energy metabolism, liver microcirculation, liver function and posttransplant survival of liver graft in rats under different warm ischemia times (WIT) and predict the maximum limitation of liver graft to warm ischemia. METHODS: According to WIT, the rats were randomized into 7 groups, with WIT of 0, 10, 15, 20, 30, 45, 60 min, respectively. The recovery changes of above-mentioned indices were observed or measured after liver transplantation. The graft survival and postoperative complications in each subgroup were analyzed. RESULTS: Liver graft injury was reversible and gradually resumed normal structure and function after reperfusion when WIT was less than 30 min. In terms of graft survival, there was no significant difference between subgroups within 30 min WIT. When WIT was prolonged to 45 min, the recipients' long-term survival was severely insulted, and both function and histological structure of liver graft developed irreversible damage when WIT was prolonged to 60 min. CONCLUSION: The present study indicates that rat liver graft can be safely subjected to warm ischemia within 30 min. The levels of ATP, energy charge, activities of glycogen, enzyme-histochemistry of liver graft and its recovery potency after reperfusion may serve as the important criteria to evaluate the quality of liver graft.

[Animal model and modified technique of orthotopic liver transplantation in rats].

OBJECTIVE: To modify orthotopic liver transplantation method with two-cuff technique. METHODS: On the basis of cuff technique, the donor liver was perfused through the abdominal aorta with 20 ml cold perfusate. The anastomosis of the suprahepatic vena cave(SVC) was sutured end-to-end with 8/0 nylon line, and the continuity of infrahepatic vena cave(IVC) and portal vein(PV) were established by means of cuff method respectively. The bile duct anastomosis was performed by internal stent. RESULTS: Orthotopic liver transplantations were performed in 360 rats. The time for donor operation and liver preparation was (31.2 +/- 5.0) min and (12.0 +/- 3.0) min, and the recipient operative and anhepatic time was (45.0 +/- 5.5) min and (20.0 +/- 2.5) min, respectively. The two-day survival rate was 91.4%. In non-intervention group, one-week survival rate was 86.5%. CONCLUSION: The modified model is easily available and highly reproducible which provides a practical and stable experimental model for the study of liver transplantation.

3,4-oxo-isopropylidene-shikimic acid inhibits adhesion of polymorphonuclear leukocyte to TNF-alpha-induced endothelial cells in vitro.

AIM: To examine the effect of 3,4-oxo-isopropylidene-shikimic acid (ISA) on human polymorphonuclear leukocyte (PMN) adhesion to human umbilical vein endothelial cells (HUVEC) and explore its mechanism. METHODS: Adhesion of PMN to HUVEC was measured by rose bengal staining assay. Cell-ELISA and RT-PCR methods were used to examine the expression of adhesion molecules ICAM-1. Cell viability was detected with MTT assay. RESULTS: ISA (1-100 micromol/L) effectively reduced PMN adhesion to TNF-alpha-induced HUVEC with the inhibitory rate from 17.2 % to 53.5 %, and exerted no effect on PMN adhesion to normal HUVEC. Adhesion molecule ICAM-1 surface protein and mRNA expression induced by TNF-alpha (400 kU/L) were significantly inhibited by ISA. In addition, the cell viability of HUVEC was unchanged 48 h after treatment with ISA. CONCLUSION: ISA inhibited TNF-alpha-stimulated PMN-HUVEC adhesion and expression of ICAM-1.

Dynamic microcirculatory changes in liver graft from non-heart-beating donor with warm ischemia injury in rat.

BACKGROUND: Since the 1990s, liver grafts from non-heart-beating donor (NHBD) have become an alternative because of the deficiency of grafts from heart-beating-donors (HBDs). Warm ischemia injury, however, directly influences the grafts' activity and functional recovery after operation. We investigated the microcirculatory change of liver graft at different warm ischemia time (WIT) in rats and determined the maximum limitation of liver graft to warm ischemia. METHODS: According to WIT, 120 rats were divided randomly into 5 groups of 0, 15, 30, 45, 60 minutes respectively. The microcirculatory changes of their liver grafts were measured including serum level of hyaluronic acid (HA) and ultrastructural changes. After orthotopic liver transplantation (OLT), the recovery of microcirculation of the liver grafts after 24 hours, 48 hours and 3 days was observed. RESULTS: Microcirculatory changes and function of the liver grafts became normal after reperfusion when the WIT was less than 30 minutes. In the 45-minute WI group, part of blood sinusoids was full of cytoplasmic blebs stemming from the microvilli of hepatocytes and hemocytes. The serum level of HA in each group after 45 minutes of WI recovered after reperfusion. CONCLUSIONS: The microcirculatory change of rat liver graft is reversible when the WIT is less than 30 minutes: rat liver graft could be safely subject to warm ischemia within 30 minutes. The maximal 45 minutes of WI can be tolerated by the microcirculatory function of liver graft. After 60 minutes of WI, irreversible disturbance of microcirculation may appear.

Beta2-adrenergic receptor gene polymorphism in Chinese Northern asthmatics.

OBJECTIVE: To investigate whether the polymorphisms of beta2-adrenergic receptor (beta2-AR) at position 16, 27, 164 are associated with asthma in Northern Chinese subjects. METHODS: Genomic DNA was collected from unrelated Northern Chinese population of Han ethnicity, including 125 unrelated asthmatic individuals and 96 healthy controls. Beta2-AR genes at position 16, 27, 164 were amplified by using restriction fragment length polymorphism (RFLP) and allelic specific polymerase chain reaction methods. All asthmatics had their serum IgE (total and specific) antibody or skin-prick test measured, bronchial reactivity to methacholine (Mch) and bronchial reversibility by beta2-agonist evaluated. RESULTS: (1) The frequency of Gly 16 homozygous was significantly higher in the asthmatic group than that in healthy controls (22.4% vs. 8.3%, P < 0.05), OR was 2.9 with 95% CI 1.26-6.78. The proportion of Gly 16 allele was also higher in asthmatics than that in control (0.46 vs. 0.36, P < 0.05); Gly16 homozygous was not independently associated with asthma pathogenesis (P = 0.21, OR 0.42 with 95% CI 0.11-1.61). (2) Of 51 night attack patients, 18 carrying Gly16 homozygosity, if compared with 10 of 74 nonnocturnal asthmatics carrying this genotype, there was significant difference between these two groups (35.3% vs. 13.5%, P < 0.01). (3) The average dose of PD20-Mch was significantly lower in patients carrying Gln 27 homozygous than those carrying homozygous Glu 27 and Gln/Glu 27 heterozygous (0.2 +/- 0.3, 1.6 +/- 0.8, and 2.1 +/- 3.0 micromol/L, P < 0.05). CONCLUSION: Beta2AR gene polymorphisms might confer susceptibility to asthma in Chinese Northern patients. Beta2-AR gene, coordinated with other candidate loci, plays a role in the development of asthma.

[Prevalence of respiratory and atopic disorders in Chinese school children].

OBJECTIVE: To compare the prevalence of respiratory and atopic disorders, and to assess the role of atopy in the development of asthma in Chinese school children from Beijing, Guangzhou, and Hong Kong. METHODS: Community-based random samples of school children aged 9 approximately 11 years from three Chinese cities (Beijing, Guangzhou and Hong Kong) were recruited for study using the International Study of Asthma and Allergies in Childhood (ISAAC) Phase II protocol. Subjects were studied by parental questionnaires (n = 10 902), skin-prick tests and skin examination (n = 3 479). RESULTS: The prevalence rates of current wheeze, speech limiting wheeze, rhinoconjunctivitis and flexural dermatitis were significantly higher in Hong Kong than in Beijing and Guangzhou. The atopy rate was also higher in Hong Kong (41.2%) than in Beijing (23.9%) and Guangzhou (30.8%). Atopy was strongly correlated with current wheeze (OR = 7.74; 95% CI = 5.70 approximately 10.51). Subgroup analyses of children from Hong Kong revealed that children born in mainland China who had subsequently migrated to Hong Kong had a significantly lower rate of allergic symptoms and atopy than those born in Hong Kong. CONCLUSION: The prevalence of asthma, allergic diseases and atopy was highest in school children from Hong Kong. Atopic sensitizations is an important factor associated with asthma in Chinese children.

Association between HLA-DQA1, -DQB1 gene polymorphisms and susceptibility to asthma in northern Chinese subjects.

OBJECTIVE: Human leukocyte antigen (HLA) class II genes, especially HLA-DQ genes, which are highly polymorphic, have been thought to be candidate loci for the etiology of asthma, and shown to be involved in antigenic presentation. This study was conducted to investigate whether susceptibility or resistance to asthma is associated with HLA-DQA1 and DQB1 genes polymorphism. METHODS: Venous blood samples were collected from northern Chinese population with Han ethnic. (1) One hundred and twenty-five unrelated asthmatic individuals and 52 subjects from 12 asthmatic pedigrees. (2) Ninety-six healthy controls without asthma and atopy with the same ethnic. Genomic DNA was extracted using standard phenol-chloroform method. The second exon of HLA-DQA1 and DQB1 genes were amplified by sequence-specific primer-polymerase chain reaction (SSP-PCR) method. All asthmatics had their serum IgE (total and specific) antibody or skin-prick test measured, bronchial reactivity to methacholine (Mch) and bronchial reversibility by beta(2)-agonist evaluated. RESULTS: HLA-DQA1 * 0104 allele and HLA-DQB1 * 0201 allele were significantly higher in asthmatics than those in healthy controls (0.408 vs 0.177, P < 0.01; 0.568 vs 0.198, P < 0.01). Odds ratios (ORs) were 3.203 (95% CI 1.699 - 6.037), 5.328 (95% CI 2.883 - 9.849) respectively. Conversely, HLA-DQA1 * 0301 allele and HLA-DQB1 * 0301 were significantly decreased in asthmatics compared to healthy controls (0.296 vs 0.50, P < 0.01; 0.4 vs 0.563, P < 0.05); Logistic regression analysis showed that HLA-DQA1 * 0104 allele was associated independently with asthma etiology, OR [represented by Exp(B)] was 5.0942 with 95% CI 2.2520 - 21.1813; Spearman's analysis showed that HLA-DQA1 * 0104 allele and HLA-DQB1 * 0201 allele were positively associated with atopy, the correlation coefficient were 0.183 and 0.289 respectively, P < 0.01. By contrast, HLA-DQA1 * 0301 allele was negatively related to atopy, the correlation coefficient was -0.168, P < 0.05; linkage analysis did not support the view that HLA-DQA1/DQB1 genes were linked to asthma with LOD value being 0.72. CONCLUSIONS: HLA-DQA1 * 0104 allele and HLA-DQB1 * 0201 allele were implicated in susceptibility to asthma and atopy, HLA-DQA1 * 0301 allele and HLA-DQB1 * 0301 might be protective factor against asthma. Asthma and atopy are multifactorial disorders, HLA-DQA1 and DQB1 genes are involved in the regulation of immune specific response to common allergen.

[Application of St George's respiratory questionnaire in evaluating the life quality of Chinese patients with chronic obstructive pulmonary disease].

OBJECTIVE: To study the value of St George's respiratory questionnaire (SGRQ) in evaluating the life quality of Chinese patients with COPD. METHODS: The SGRQ score and spirometry were collected from 30 patients with COPD. The validity was documented by performing correlation analysis, and Pearson's correlation coefficients were calculated. Nineteen patients were reassessed after three months' treatment with inhaled glucocorticoids. Paired t test was performed to evaluate the sensitivity of SGRQ to changes of disease severity. RESULTS: The correlation coefficients between the total score and the symptoms section score, the activity section score, and the impacts section score of SGRQ were 0.61, 0.79 and 0.97, respectively. The symptoms section score, the activity section score, the impacts section score, and the total score were significantly correlated with FEV(1)% pred, and with similar coefficients (-0.4 - -0.5, P < 0.05). The result of paired t test showed that the changes of SGRQ between the two assessments were significant (P < 0.001) and consistent with changes of clinical symptoms and spirometry. SGRQ was more sensitive than spirometry in evaluating the changes of disease severity. CONCLUSION: SGRQ was a valid, sensitive and feasible method in measuring the quality of life in Chinese patients with COPD. Large-scale application in China needs to be carried out.

Influence of warm ischemia injury on hepatic functional status and survival of liver graft in rats.

OBJECTIVE: To investigate the changing patterns of functional and histological status, observe the posttransplantation survival of liver graft under different warm ischemia time (WIT) in rats, and determine the maximum limitation of liver graft to warm ischemia. METHODS: According to WIT, the rats were randomized into 7 groups, with WIT of 0, 10, 15, 20, 30, 45, 60 minutes respectively. Serum concentrations of alanine aminotransferase, aspartate aminotransferase, and lactate dehydrogenase were measured at 1, 2, 3 and 5 days after orthotopic liver transplantation respectively. Liver graft specimens were observed histopathologically at the same interval. The rats' survival in each subgroup was observed. RESULTS: In terms of graft survival, there was no significant difference between subgroups within 30-minute WIT. In the group with 30-minute WIT, the recipient rats' survival rate was 83.3% (10/12) at one week, 58.3% (7/12) at one month, and 50.0% (6/12) at 3 months. In the group with 45-minute WIT, the recipient rats' survival rate was 66.7% (8/12) at one week, 33.3% (4/12) at one month, and 8.3% (1/12) at 3 months, whereas only 8.3% (1/12) of the rats had one-week survival in the group with 60-minute WIT. CONCLUSIONS: These results indicate that rat liver graft could be safely subject to warm ischemia within 30 minutes. When WIT is prolonged to 45 minutes, the recipients long-term survival is severely insulted, and both function and histological structure of liver graft may develop irreversible damage when WIT is prolonged to 60 minutes.

[Role of single-breath carbon monoxide-diffusing capacity in monitoring the bleomycin-induced lung toxicity in human].

OBJECTIVE: To investigate if the carbon monoxide-diffusing capacity (D(LCO)) could be the early indictor monitoring the bleomycin-induced lung toxicity (BILT) with retrospection of the influence of bleomycin cumulative dose on the pulmonary functions. METHODS: During June 1985 to October 2000, 42 patients with malignant tumor of ovarian germ cells received chemotherapy containing bleomycin in the department of Obstetrics and Gynecology of Peking Union Medical College Hospital. Twenty three patients (54.76%) among them had >or= 2 courses of chemotherapy with bleomycin and performed >or= 2 measurements of lung function. These 23 patients were included in the study. The forced expiratory volume in one second (FEV(1)), forced vital capacity (FVC), total lung capacity (TLC) and D(LCO) were measured and recorded as % of predicted. The D(LCO) was corrected by hemoglobin concentration [D(LCO) corrected = D(LCO) measured x (9.38 + Hb) divided by (1.76 x Hb)]. All the data were divided into 4 groups according to the cumulative dose of bleomycin (pre-treatment group, < 100 mg group, 101 - 200 mg group and > 201 mg group). The relationship between D(LCO) and bleomycin cumulative dose was examined by linear regression analysis and t-test. RESULTS: The values of FEV(1%)predicted in the 4 groups were 99.83 +/- 16.41 (14), 101.43 +/- 12.32 (42), 99.41 +/- 10.22 (23), and 90.96 +/- 13.63 (12), respectively. The FVC%predicted were 97.74 +/- 18.23 (14), 101.11 +/- 13.95 (42), 96.49 +/- 12.04 (23) and 89.63 +/- 18.20 (12), respectively. The TLC%predicted were 101.22 +/- 10.68 (13), 106.14 +/- 12.16 (40), 102.13 +/- 11.33 (23) and 95.05 +/- 14.06 (11), respectively. There were no statistical significant differences in the parameters among the 4 groups. The D(LCO%)predicted of the 4 groups were 93.27 +/- 12.75 (14), 94.51 +/- 12.50 (40), 80.93 +/- 10.05 (24) and 70.99 +/- 11.69 (15), respectively, and the D(LCO) of the last group (> 201 mg group) was significantly decreased as compared to that of the other groups (P < 0.05). A bleomycin dose-related fall in D(LCO) was observed, y = 100.59 - 0.11x, r = -0.649, P < 0.001. CONCLUSION: The D(LCO) might be the most sensitive indicator of subclinical BILT. However, the cumulative dose of bleomycin did not show significant influence on the FEV(1), FVC and TLC.

[Thoracic high resolution CT findings of 100 SARS patients in convalescent period].

OBJECTIVE: To study thoracic high resolution CT findings of severe acute respiratory syndrome (SARS) patients in convalescent period and the relationship between the features with laboratory and pulmonary function results. METHODS: All the 100 SARS patients who had been discharged from hospital for about 2 months underwent thoracic high resolution CT examinations. Among them, 65 also had laboratory results and 91 underwent examinations of pulmonary function. RESULTS: Forty-nine SARS patients (49.0%, 49/100) in convalescent period still had abnormal findings in CT examination. And ground-glass opacification (95.9%, 47/49) and reticular opacification (59.2%, 29/49) were common. There was no significant difference in the level of lymphocytes and CD4 cells among groups divided according to severity of CT findings (P > 0.01). Diffusing capacity for carbon monoxide (DLco%) of level 2 and 3 were statistically lower than that of level 0 (P < 0.01). CONCLUSIONS: The features of CT findings of SARS patients in convalescent period are not the same as those of patients in period of apparent manifestation. The high resolution CT can reflect pulmonary diffusing function to some degree.

[Assessment of health-related quality of life in cured SARS patients].

OBJECTIVE: To assess the quality of life in cured patients with severe acute respiratory syndrome (SARS). METHODS: One hundred and nineteen SARS outpatients, including 64 men and 55 women, with mean age (34.1 +/- 11.4) years and average days of discharge from hospital (28.0 +/- 12.8) days, were assessed by the St George's respiratory questionnaire (SGRQ), and 72 patients with spirometry test. RESULTS: The patients had a moderately degree physiological impairment and increased SGRQ score. Scores of all four part of SGRQ correlated significantly with diffuse capacity of the lung for carbon monoxide/pre (DLco%). The correlation coefficients between the activation, impaction, total score, and diffuse capacity of the lung for carbon monoxide/alveolar ventilation/pre (DLco/Va%) were resembled (r = 0.47-0.54, P < 0.01). There was a significant relationship between the score of SGRQ and patients age, and gender. The time leaving hospital only had a significant relationship with the symptom score of SGRQ. CONCLUSIONS: SGRQ is a sensitive tool for assessing quality of life in cured SARS patients. The cured SARS patients' quality of life decrease moderately.

[Assessment of pulmonary function in SARS patients during the convalescent period].

OBJECTIVE: To investigate the pulmonary function in severe acute respiratory syndrome (SARS) patients during the convalescent period. METHODS: Followup 89 outpatients of SARS. The follow-up study included interview, physical examination, and pulmonary function test. RESULTS: The interval between hospital discharge and functional assessment was 1.75 +/- 0.53 months (0.5-3.4 months). Mild to moderate abnormalities in pulmonary function were found in 48 patients (53.9%). Diffusion capacity for carbon monoxide (DLco) was impaired in 38 patients (42.7%); in 7 patients (7.9%), lung function was restrictive defect combined DLco impairment; Other patterns of impairment were revealed in 3 patient. Dyspnea during acute phase and CT during the convalescent period were found to have significant influences on DLco and total lung capacity (TLC). CONCLUSIONS: Diffusing capacity impairment as well as restrictive defect persist in convalescence SARS.

[Protective effect of 3,4-oxo-isopropylidene-shikimic acid on vascular endothelial cell injured by hydrogen peroxide].

AIM: To study the effect of 3,4-oxo-isopropylidene-shikimic acid (ISA) on H2O2 (200 mol.L-1, 4 h) injured human umbilical vein endothelial cells (HUVEC). METHODS: Morphological change was observed under microscop. Cell viability was assessed by MTT assay. The release of intracellular lactate dehydrogenase (LDH) and NO was assessed by colorimetry. Radioimmunoassay was used to assess 6-keto-prostaglandin F1 alpha (6-keto-PGF1 alpha). RESULTS: Pretreatment with ISA for 6 h alleviated the morphological damage of H2O2 induced HUVECs. At the concentration of 1-100 mumol.L-1, ISA prevented the inhibitory effect on cell viability induced by H2O2 in dose-dependent manner, but increased the ratio of cell viability from 60.4% to 84.3%. ISA reduced LDH release and increased the level of NO and 6-keto-PGF1 alpha in H2O2 induced HUVECs. CONCLUSION: ISA exerted protective effect on H2O2 injured HUVEC.
