[Study on the cause of tuberculosis recurrence by Mycobacterium tuberculosis genotyping method].

OBJECTIVE: To study the contribution of exogenous reinfection to tuberculosis recurrence. METHODS: Mycobacterium tuberculosis isolates from patients who had experienced 2 successive tuberculosis episodes during the years from January 1999 to March 2004 were genotyped by mycobacterial interspersed repetitive units (MIRU) method. The cause of tuberculosis recurrence was determined by comparing the MIRU pattern of Mycobacterium tuberculosis isolates responsible for different tuberculosis episodes from patients. RESULTS: Of 37 qualified patients with recurrent tuberculosis, the isolates from 25 patients in their two tuberculosis episodes showed different MIRU patterns, indicating that 68% recurrent patients were due to exogenous reinfection. The exogenous reinfection rate decreased with the age from 3/3 (under 30 years) to 73% (11/15, 30 to 60 years) and 58% (11/19, over 60 years). The frequency of exogenous reinfection increased with the tuberculosis recurrent interval. Within 6 months, the exogenous reinfection accounted for 58% (7/12) of all the recurrence, while for the time more than one year, the percentage increased to 79% (11/14). CONCLUSIONS: Exogenous reinfection was a major cause of tuberculosis recurrence in Shanghai. MIRU genotyping method is useful to study the cause of tuberculosis recurrence.

[Modified abdominal radical trachelectomy for treatment of cervical carcinomas].

OBJECTIVE: To investigate the possibility and effect of modified abdominal radical trachelectomy in treating women with early cervical cancers who desired to preserve fertility. METHODS: From October 2002 through September 2004, we performed abdominal radical trachelectomy with a modified Smith's style on 13 patients which were biopsy-proven early stage cervical carcinomas and had a desire to preserve fertility. Pelvic lymphadenectomies were performed in all patients. Radical trachelectomy was performed immediately if lymph nodes were negative. RESULTS: Thirteen women underwent this procedure. The average age was 29.4 years (range 21 - 35 years); 9 were null gravid and 4 were multipara. Mean operative time was 158 minutes (135 - 185 minutes), with a mean blood loss of 120 ml (100 - 180 ml), and an average hospital stay of 9.7 days. There was no intra- or post-operative complication. With an average follow-up of 21.3 months, there have been no recurrences. Five out of 13 patients have tried to get pregnant after operation, 3 patients conceived pregnancy, 1 patient was premature labor at 34 weeks, and the other 2 patients are pregnant at the moment. CONCLUSION: The modified abdominal radical trachelectomy which has less injury and fewer side-effects is safe and effective in treating patients with early cervical carcinoma, and can also give the patients chances to preserve their fertility in the future.

[Evaluation of the mycobacterial interspersed repetitive units typing as a practical approach in molecular epidemiology of Mycobacterium tuberculosis].

OBJECTIVE: To introduce a new genotyping method, mycobacterial interspersed repetitive units (MIRU) typing, for Mycobacterium tuberculosis and to evaluate its feasibility. METHODS: Mycobacterium tuberculosis strains stored at Shanghai Municipal Center for Disease Control and Prevention during 2000 to 2002, were randomly selected by simple digital table and genotyped by MIRU and Spoligotyping. RESULTS: By Spoligotyping method, 91 strains were typed to 20 genotypes, of which 89% (81/91) strains belonged to Beijing genotype, while by MIRU method, these strains were divided into 46 genotypes. The MIRU typing showed high discriminatory power, especially for the Beijing genotype strains. The 81 Beijing genotype strains could be subdivided into 39 different MIRU genotypes. In this sample collection, 12 MIRU loci showed different discriminative according to their allelic diversity. Locus 26 showed highly discriminative, while locus 16, 31, and 40 showed moderately discriminative. CONCLUSIONS: MIRU genotyping is a simple and fast method. Its numerical result facilitates the comparison among strains of Mycobacterium tuberculosis from different labs.

[Transplantation of mouse bone marrow mesenchymal stem cells into the xenogeneic testis].

OBJECTIVE: To study transplantation of mouse bone marrow mesenchymal stem cells (MSCs) into the xenogeneic testis. METHODS: (1) The tibias and femurs were dissected from 5-6-week-old mice. The marrow in the tibias and femurs was flushed out with medium. MSCs were isolated, cultured and purified in vitro by Percoll density gradient centrifugation combined with adherent method. (2) MSCs of the third generation were adopted and marked with Hoechest33342 for observation, and then made into cell-suspending fluid. (3) The marked MSCs were transplanted into the testis of the xenogeneic mouse by testis net injection. The biopsies of the testis tissues were carried out at different time and made into frost slices at three sites for observation. RESULTS: (1) A lot of purified MSCs were obtained at the third generation. (2) The nucleoli of the marked MSCs showed light-yellow under the fluoroscope. (3) Xenogeneic transplantation of mouse bone marrow MSCs by testis net injection was successful, without immunoreaction. On the 1 st day after transplantation, MSCs only concentratively distributed in the medial slices, the nucleoli being light-yellow; On the 1 st and 3 rd day, MSCs dispersively distributed in the medial slices; On the 6th, 9th and 12th day, MSCs presented in all the slices of the three sites, some ranging tubally; On the 15th and 18th day, the fluorescence of MSCs weakened; On the 21 st day, the fluorescence of MSCs disappeared. CONCLUSION: Transplantation of mouse bone marrow MSCs into the xenogeneic testis by testis net injection is effective and feasible, without immunoreaction. MSCs can survive after transplantation.

[Study on cytokeratin and CA125 for diagnosing lymph node micrometastases in endometrial cancer].

OBJECTIVE: To study the roles of cytokeratin (CK) and CA(125) in diagnosing lymph node micrometastasis in endometrial cancer. METHODS: The expressions of CK and CA(125) in 50 primary tumors and 298 lymph nodes from 50 patients with endometrial cancers were analysed by immunohistochemical methods. RESULTS: (1) The positive expression rates of CK and CA(125) in primary tumors of cases with endometrial cancer were respectively 100%, and 78%. (2) The expressions of CK and CA(125) in metastatic lymph nodes were both strong with 100% positive rate. In lymph nodes without metastasis, the expressions of CK and CA(125) were both weak, with positive rates of 15% and 12% respectively. (3) Among patients with stage I, II diseases, tumor recurrence rate was significantly higher in patients with positive CK or CA(125) expression in lymph nodes than in patients without CK or CA(125) expression (P < 0.05). (4) Multiple regression analysis revealed that in stages I, II endometrial cancer, CK expression in lymph nodes, and CA(125) expression in lymph nodes together with depth of myometrial invasion were relevant factors with tumor recurrence. CONCLUSIONS: CK expression in lymph nodes without metastasis can predict lymph node micrometastasis and is an independent risk factor for recurrence of disease in stages I, II endometrial cancers. CA(125) expression in lymph nodes without metastasis can also suggest lymph node micrometastasis, but it is not an independent predictive factor for tumor recurrence in stages I, II endometrial cancers.

Enhancement of immune responses to hepatitis B vaccine in immunodepressed mice by CpG oligodeoxynucleotide.

AIM: To investigate enhancement of immune response to hepatitis B vaccine in immunosuppressed mice by CpG oligodeoxynucleotide (ODN). METHODS: Immunodepressed C57BL/6 mice caused by cytoxan were given an injection of either hepatitis B vaccine alone or hepatitis B vaccine and CpG ODN into the left tibialis anterior muscle, then the mice were given another shot after 2 weeks using the same formulation. Blood was collected at 5 weeks after immunization and anti-hepatitis B surface antigen IgG and IL-12 levels were measured by ELISA. Spleens of immunized mice were observed under microscope. RESULTS: There was a two-fold increase in anti-hepatitis B surface antigen IgG level when hepatitis B vaccine was mixed with CpG ODN to immunize immunodepressed mice. There was a significant increase in IL-12 level when hepatitis B vaccine was mixed with CpG ODN. Under optical microscope, there were fewer lymphocytes in immunodepressed mice than in normal control group. The proliferation level of spleen lymphocytes in CpG ODN group was elevated compared with that of normal control group and nuclei of lymphocytes became larger. CONCLUSION: CpG ODN can enhance the immune response to hepatitis B vaccine in immunodepressed mice.
