[Influence of clinical and pathomorphological parameters on prognosis in colon carcinoma and rectal carcinoma].

OBJECTIVE: To investigate the effects of clinical and pathomorphological parameters on the prognosis of colon carcinoma and rectal carcinoma. METHODS: Univariate and multivariate COX proportional hazard models were used to study the effects of the clinical and pathomorphological factors on the prognosis in 101 cases of colon carcinoma, 219 of rectal carcinoma and 137 of rectal carcinoma under curative resections. RESULT: By using univariate analysis, we identified that lymph node metastasis and distant metastasis were the common prognostic factors for both colon carcinoma and rectal carcinoma. Smoking, deep infiltration, chemotherapy and serum albumin concentration were the uncertain prognostic factors for colon carcinoma. Signet-ring cell carcinoma, larger tumor size (>6 cm), deep infiltration, lack of radical surgery, and advanced TNM stage were the exclusive adverse prognostic factors for rectal carcinoma. Further studies showed that the adverse prognostic factors for the rectal carcinoma under curative resection included deep infiltration, lymph node metastasis, vessel invasion, less of peritumoral lymphocyte infiltration, lack of Crohn's like reactivity, high level of tumor budding, advanced TNM stage and positive urine glucose. By using multivariate analysis based on a COX proportional hazard model, it was identified that smoking, lymph node metastasis and serum albumin concentration were independent prognostic factors for colon carcinoma; advanced TNM stage, distant metastasis and palliative surgery for rectal carcinoma; and vessel invasion, lymph node metastasis and urine glucose for rectal carcinoma under curative resections. CONCLUSION: The various clinical and pathomorphological parameters show different prognostic value for colon carcinoma, rectal carcinoma and rectal carcinoma under curative resections.

[Outcome of in vitro fertilization-embryo transfer in treatment of polycystic ovarian syndrome].

OBJECTIVE: To evaluate the outcome of in vitro fertilization embryo transfer (IVF-ET) in treatment of polycystic ovarian syndrome (PCOS) with infertility. METHODS: A retrospective analysis was performed from 52 patients with PCOS (PCOS group) and 408 cases with tubal infertility (control group). Both groups underwent IVF-ET treatment from 2001 to 2004. The duration of stimulation, amps of gammaFSH, the level of serum E2, P on the day of HCG injection, the number of oocytes retrieved, the rates of fertilization, cleavage, implantation and pregnancy, the incidence of ovarian hyperstimulation syndrome (OHSS) and cancelled rate of ET were compared between the two groups. RESULT: The duration of stimulation and amps of gammaFSH were not significantly different between the two groups. The concentration of serum E2, P on the day of HCG injection, the numbers of oocytes retrieved and cleavaged embryos were significantly higher in PCOS group (P <0.01, <0.05). Fertilization rate was significantly lower in PCOS group (P <0.01). The implantation, pregnancy and miscarriage rates per ET were not statistically significant. The OHSS rates and cancelled rates of ET were higher in PCOS group (P <0.01). CONCLUSION: Women with PCOS have a lower fertilization rate compared with those with tubal-factor fertility during IVF-ET. However, more oocytes are recovered and the preimplanted embryo has a normal chance of implantation leading to similar pregnancy rates. The OHSS rates and cancelled rates of ET are higher in PCOS because of a greater number of oocytes developed and a higher level of E2.

[Effect of salpingectomy on ovarian function].

OBJECTIVE: To investigate the effects of salpingectomy on ovarian function. METHODS: Fifty-four women with previous unilateral salpingectomy and 64 with bilateral salpingectomy (study groups) and 59 infertile patients with obstruction of fallopian tube (control group) received in vitro fertilization-embryo transfer (IVF-ET) treatment. The baseline levels of serum FSH, LH, E2, PRL, ovarian response to superovulation and outcome of IVF-ET among the three groups were compared. RESULT: There were no significant differences in the baseline endocrine levels among three groups. There were more ampules and days of gonadotropin (Gn) administration, fewer oocytes retrieved in bilateral salpingectomy group than in control group (P <0.05). The duration of Gn administration of unilateral salpingectomy group was significantly longer than that of control group (P <0.05). The number of oocytes retrieved and the total ampules of Gn administration in unilateral salpingectomy group were not different with those of bilateral salpingectomy group and control group (P >0.05). Among the patients with the history of unilateral salpingectomy, significantly fewer oocytes were retrieved from the ovary on the operated side (P <0.05). There were no significant difference in the fertilization rates, cleavage rates, good-embryo rate, implantation rate, clinical pregnancy rates and good-embryo number among the three groups. CONCLUSION: Salpingectomy has no obvious effect on the outcome of IVF-ET and baseline endocrine within a period of time, but it reduces the ovarian response to superovulation on ipsilateral ovary in IVF-ET.

[Report of the first human case of H5N1 avian influenza pneumonia in Hunan, China].

OBJECTIVE: To summarize and analyze the clinical characteristics and diagnostic and therapeutic measures for the first human case of H5N1 avian influenza pneumonia in mainland of China. METHODS: The clinical data of the first case of H5N1 avian influenza virus infection in China were analyzed and summarized. RESULTS: The case is a 9-year old boy, who developed acute symptoms of a light common respiratory infection, including fever and dry cough without obvious catarrh. On the 7th day after onset, his temperature reached 40 degrees C, tachypnea occurred, distinct rales could be heard and large areas of consolidation were seen in the lungs on chest X-ray. The patient's peripheral blood leukocyte count was 2.81 x 10(9)/L and neutrophils dominated. After comprehensive therapeutic approaches, including antiviral therapy (amantadine) and use of low-dosage glucocorticoid, the patient's temperature returned to normal on the 3rd hospitalization day, chest X-ray showed absorbed inflammatory change on the 5th day after admission, and leukocyte count became normal on the 6th day. No complication occurred during the whole course. The case was diagnosed by the 4 fold raised antibody to the H5N1 influenza virus in recovery stage serum because the H5N1 nucleic acid test in early stage was negative. The case was cured and discharged after 3 weeks comprehensive treatment. CONCLUSIONS: It is very important for clinicians to pay enough attention to epidemiological history, especially history of exposure to avian influenza virus contaminated material, which will be very helpful for early detection, early diagnosis of the disease, and also very important for effective treatment and better prognosis.

[Study on independent factors on the prognosis of colorectal carcinoma: TNM stage, tumor budding, perineural invasion, peritumoral-lymphocytic infiltration and urine glucose].

OBJECTIVE: To study the influence of clinical and pathological-morphological parameters on the prognosis of colorectal carcinoma. METHODS: Univariate and multivariate Cox proportional hazard model were used to study the influence of clinical and pathological-morphological factors on the prognosis in 226 colorectal carcinoma cases. RESULTS: Using univariate analysis, data showed that the factors significantly related to disease prognosis would include: the depth of direct spread, vessel invasion, perineural invasion, tumor budding, peritumoral-lymphocytic infiltration, Crohn-like reaction, number of positive lymph nodes, distant metastasis, TNM stage and urine glucose. Multivariate Cox proportional hazard model showed that six factors were identified to be associated with higher relative-risk (RR), including: older age, advanced TNM stage, more severe budding, perineural invasion, less peritumoral-lymphocytic infiltration and urine glucose. CONCLUSION: Age, TNM stage, tumor budding, perineural invasive, peritumoral-lymphocytic infiltration and urine glucose were independent predictors to the prognosis of colorectal carcinoma.

[Structural features of GR6 gene and its expression in colorectal neoplasm].

OBJECTIVE: To determine the characteristics of GR6 gene and putative GR6 protein, and to evaluate the expression of GR6 gene in colorectal cancer and normal mucosa. METHODS: Bioinformatic software and databases were applied to analyze the characteristics of GR6 gene and putative GR6 protein. Semi-quantitative RT-PCR was used to detect the expression of GR6 gene in colorectal carcinoma, adenoma and normal mucosa. RESULT: GR6 gene, encoding putative GR6 protein, consisted of 3 exons and contained 4 CpG islands by sequence analysis. It was predicted that putative GR6 protein included one protein kinase C phosphorylation site, one casein kinase II phosphorylation site, and three N-myristoylation sites. PSORT II software analysis predicted that putative GR6 protein was located in nucleus (reliability: 76.7%). At the level of mRNA, the expression of GR6 gene was high in normal mucosa, moderate in mucosa adjacent to cancer and adenoma tissue, low in colorectal carcinoma tissue. Significant differences were demonstrated between normal mucosa and adenoma (P<0.05), normal mucosa and carcinoma (P<0.01). CONCLUSION: The putative GR6 protein, encoded by GR6 gene may predictably function as an important nuclear signal transduction molecule. Decreased expression of GR6 gene may play an important role in the initiation and promotion of colorectal neoplasia.

[Expression of beta-catenin in human colorectal adenoma and carcinoma].

OBJECTIVE: To investigate the expression of beta-catenin and its significance in colorectal neoplasms. METHODS: Tissue specimens of normal colorectal mucosa, mucosa adjacent to carcinoma, colorectal adenoma and adenocarcinoma were examined for beta-catenin with immunohistochemistry. RESULTS: Beta-catenin was mainly expressed in the cytomembrane of normal mucosa and mucosa adjacent to cancer (the positive rates were 94.6% and 86.5%, respectively) and also in the cytoplasm (the positive rates were 38.7% and 55.0%, respectively), while its expression was negative in the cell nucleus. In adenoma and adenocarcinoma, beta-catenin was mainly expressed in the cytoplasm (the positive rates were 85.1%,and 93.7%, respectively) and partially in the cell nucleus (the positive rates were 12.8% and 23.4%, respectively). Compared with normal mucosa and mucosa adjacent to cancer, the expression of beta- catenin in the cytomembrane of adenoma and adenocarcinoma was significantly lower (P<0.05), while its expression in the cytoplasm and cell nucleus of adenoma and adenocarcinoma was significantly higher (P<0.05). The positive rates of cytoplasm in highly-and moderately differentiated adenocarcinoma were significantly higher than that in poorly-differentiated adenocarcinoma (the positive rates were 100%, 95.5% and 68.8%, respectively). Beta-catenin expression rate in cytoplasm was correlated with Dukes'stages of adenocarcinoma, which was significantly lower in stage A than in stage B/C. CONCLUSION: The expression of beta-catenin is significantly correlated with differentiation and Dukes'stages of colorectal carcinoma and it can be used as an indicator for the prognosis of colorectal carcinoma.

[Laparoscopic examination and treatment of abdominal cocoon with infertility: a report of 6 cases].

OBJECTIVE: To assess the abdominal cocoon complicated with infertility using laparoscope and its clinical management. METHODS: Six cases of abdominal cocoon treated in our hospital from January 1998 to December 2002 were retrospectively reviewed. RESULTS: Of the 6 patients all with primary and tubal infertility, 3 received routine closed laparoscope operation, and other 3 failed for laparoscopy and were transferred to laparotomy. During the surgery it was found that the abdomen and pelvis were filled with multiple layer fibrous tissue which enveloped the bowel and reproductive organs, making exploration nearly impossible. It was difficult to ablate the envelope. CONCLUSION: Abdominal cocoon can be diagnosed in those primary and tubal infertile patients when the charge is hindered in the process of laparoscope. The optimal treatment of the infertility for those patients is in vitro fertilization-embryo transfer.

[A case-control study on the polymorphisms of methylenetetrahydrofolate reductases, drinking interaction and susceptibility in colorectal cancer].

OBJECTIVE: To investigate the relationship between methylenetetrahydrofolate reductases (MTHFR) polymorphisms and colorectal cancer susceptibility. METHODS: A case-control study of 126 patients and 343 healthy controls was conducted to investigate the roles of MTHFR C677T and A1298C polymorphisms in colorectal cancer development. Genotypes of C677T and A1298C polymorphisms were analyzed by polymerase chain resction-restriction fragment length polymorphism (PCR-RFLP) methods. RESULTS: The frequencies of MTHFR 677T and 1298C allele were 39.7% and 17.1%, respectively. After adjustment for age and sex, the MTHFR 1298C alleles seemed to have reduced association on the risk of colorectal cancer comparing to wild types. Among those with 677T and 1298A alleles, a decreased risk of colorectal cancer was observed: a 4-fold decrease in colorectal cancer risk (OR = 0.552, 95% CI: 0.265 - 1.150) in those with 677T and 1298C alleles. Men who were ex-drinkers and with MTHFR 1298C allele had a 2-fold increase in risk of colorectal cancer (OR = 3.307, 95% CI: 0.521 - 17.698) while no increased risk was seen among those current-drinkers. CONCLUSIONS: This study suggested that certain MTHFR C677T and A1298C might be associated with the risk of colorectal cancer development. The interaction between MTHFR 1298AC polymorphisms and ex-drinking might also serve as a risk factor of colorectal cancer.

[Quantitative assessment of individual risk and population screening program on gastric carcinoma].

OBJECTIVE: To set up a mathematic model for determining individual risk and population risk of gastric carcinoma. METHODS: Based on principles and methods of probability and fuzzy mathematics, a case-control study was quantified and a mathematic model for the screening of gastric carcinoma was set up. Using two different calculation methods of weight coefficients, the thresholds were identified as 0.20 and 0.17, respectively. This mathematic model was then used to examine 63 cases and 693 normal persons. RESULTS: The sensitivities and specificities were 69% and 63%. The different calculative methods of weight coefficients did not affect the results of the identification. CONCLUSION: The sensitivities and specificities were satisfactory indicating that it was convenient, feasible, economic and could meet the contented screening rate. It could be used to determinate the risk for gastric carcinoma both on individual and on population.

[Screening the differentially methylated DNA sequences of colorectal cancer by methylated CpG islands amplification coupled with representational difference analysis].

OBJECTIVE: To screen the differentially methylated DNA sequences between mucosa adjacent to colorectal cancer (MACC) and normal colonic mucosa. METHODS: The methylated DNA sequences were enriched by methylation CpG islands amplification (MCA), and the differentially methylated DNA sequences between MACC and normal colonic mucosa were isolated by representational difference analysis (RDA). Similarities between the separated fragments and the human genomic DNA were analyzed with BLAST program system in GenBank. With the separated fragment 1A12 as probe, dot blot was used to study its distribution between RDA products (No. 1-4 rounds), MACC (tester) and normal colonic mucosa(driver). RESULTS: Twenty-five differentially methylated DNA sequences were obtained. Preliminary studies indicated that 1A01 fragment was concerned with two different genes (LOC256866 and CECR7), it was located in the first exon of CECR7. 1A12 fragment was located in 5 flanking region of GR6 gene. By dot blot with 1A12 probe, hybridized signals were detected in MCA product of MACC and RDA products of No. 1-4 rounds, respectively. No signal was detected in MCA product of normal colonic mucosa. CONCLUSION: The differentially methylated DNA sequences can be isolated effectively between two different tissues with MCA coupled with RDA. Different methylated DNA fragments exist between MACC and normal colonic mucosa and these fragments may be concerned with colorectal cancer.
