Epidemiological survey on the relationship between different iodine intakes and the prevalence of hyperthyroidism.

OBJECTIVE: To investigate the effect of different levels of iodine intake on the prevalence of hyperthyroidism and the impact of universal salt iodization on the incidence of hyperthyroidism. DESIGN: A comparative cross-sectional and longitudinal survey was conducted in three areas with borderline iodine deficiency, mild iodine excess (previously mild iodine deficiency) and severe iodine excess. Universal salt iodization had been introduced 3 years previously except in the area with borderline iodine deficiency. METHODS: In total 16 287 inhabitants from three areas answered a questionnaire concerning the history of thyroid disease. Among them 3761 unselected subjects received further investigations including thyroid function, thyroid autoantibodies, thyroid ultrasonography and urinary iodine excretion. RESULTS: Among areas with median urinary iodine excretion of 103 microg/l, 375 microg/l and 615 microg/l (P<0.05), the prevalence of hyperthyroidism did not differ significantly (1.6%, 2% and 1.2%). The prevalence of subclinical hyperthyroidism was higher in areas with borderline iodine deficiency and mild iodine excess than in the area with severe excess iodine intake (3.7%, 3.9% and 1.1%, P<0.001). The prevalence of Graves' disease and its proportion in hyperthyroidism did not differ among areas. The incidence of hyperthyroidism did not significantly increase after the introduction of universal salt iodization. CONCLUSION: Different iodine intakes under a certain range do not affect the prevalence and type of hyperthyroidism. Subclinical hyperthyroidism is more prevalent in the iodine deficient area than in the severe iodine excessive area. In the area with mild iodine deficiency, the introduction of universal salt iodization may not be accompanied by an increased incidence of hyperthyroidism.

The study of thyroid diseases in a community not using iodized salt.

OBJECTIVE: To investigate the prevalence of thyroid diseases in a community which did not use iodized salt. METHODS: The survey was conducted in Panshan, Liaoning Province. 1 103 inhabitants aged 14 years or more attended the examinations, which included questionnaire, physical examination and serum analysis. Iodine in the urine and thyroid B ultrasound examination were also conducted. RESULTS: The prevalence of overt hyperthyroidism and hypothyroidism was 16.3 per thousand and 2.7 per thousand, respectively. Subclinical hyperthyroidism and hypothyroidism were detected in 37.2 per thousand and 9.1 per thousand of the subjects, respectively. Serum autoantibodies to thyroid were detected in 10.9% of the entire population. The prevalence of goiter was 20.7% (diffuse goiter 16.8% and nodular goiter 3.9%). CONCLUSION: In the iodine deficient areas, perhaps autoimmununization is not only related to the development of goiter but is also the main cause of subclinical hyperthyroidism and hypothyroidism.

[A linkage study of cytotoxic T lymphocyte-associated antigen 4 gene and Graves' disease in northern Chinese Han ethnic].

OBJECTIVE: To determine if the cytotoxic T lymphocyte-associated antigen 4 (CTLA-4) gene, which is located on chromosome 2q31-q33, is the major susceptibility gene for Graves' disease (GD) of northern Chinese Han nationality. METHODS: Five highly polymorphic microsatellite markers spanning the entire region of chromosome 2q31-q33 were employed to screen 54 families with multiple incidences of GD (322 individuals) of northern Chinese Han nationality. Tow-point and multi-point Lod scores were calculated under different levels of penetrance, assuming both dominant and recessive models. Multipoint nonparametric linkage (NPL) scores were also calculated. RESULTS: Two-point Lod scores (theta = 0) and multipoint Lod scores of less than -2 were observed for all the markers tested, at all levels of penetrance, in both the dominant and recessive models of inheritance. P values of greater than 0.05 were observed for all the multipoint NPL scores obtained. CONCLUSION: We obtained evidence against the linkage of GD with chromosome 2q31-q33, thus we are of the opinion that CTLA-4 gene is not the major susceptibility locus for Graves' disease of northern Chinese Han nationality.

[Maintaining Growth of Long-Term Culture Initiating Cells from Human Cord Blood on Feeder Layers of Bone Marrow Stromal Cells Transfected with FL and/or TPO Genes]

Long-tem culture initiating cells(LTC-IC), and in vitro assay of hematopoietic stem/progenitor cells, still represent a heterogeneous population in terms of proliferative capacity and sensitivity to different growth factors. Human umbilical cord (CB) is rich of hematopoietic progenitor cells measured by clonogenic assays and stem cells capable of reconstituting the marrow after transplantation. The influence of culture conditions on the in vitro behavior of LTC-IC from CB was evaluated. First, by using IRES sequence, FL and TPO cDNA were recombined with retroviral vector pLXSN by gene recombination technology. The recombinant plasmid pLFSN, pLTSN, pLFTSN were transfected into human stromal cell line HFCL. Then, LTC-IC were evaluated in long term cultures, comparing five types of stromal feeder layers: human bone marrow stromal cell, human stromal cell line HFCL, and stromal cell lines HDF tranfected with FL gene, HLT transfected with TPO gene or HFT co-transfected with FL and TPO genes. The results were demonstrated that after 8 weeks of coculture, three types of stromal cell lines that supported the maintenance of CFU-C for up to 3 weeks in vitro were identified. However, cocultivation of human bone marrow stromal cell and CB CD34(+) cells on HFT, CFU-C production continued up to 6 weeks or longer on these stroma. The absolute LTC-IC frequency in CD34(+) cells on human bone marrow stromal cell (2.65 +/- 0.76/1 000 cells) was no significant difference with on HFT (3.65 +/- 0.58/1 000 cells). Thus, HFT acts by direct contact to maintain the phenotype and function of the most primitive and quiescent human progenitors. Furthermore, HFT cell line was selected as the optimal one for supporting long-term culture feeder. It was concluded that LTC-IC progenitors from cord blood maintain growing upon the FL/TPO gene-modified stromal feeder layers in vitro.

Differences in optical properties between healthy and pathological human colon tissues using a Ti:sapphire laser: an in vitro study using the Monte Carlo inversion technique.

The purpose of the study is to analyze and compare differences in the optical properties between normal and adenomatous human colon tissues in vitro at 630-, 680-, 720-, 780-, 850-, and 890-nm wavelengths using a Ti:sapphire laser. The optical parameters of tissue samples are determined using a double integrating sphere setup at seven different laser wavelengths. The inverse Monte Carlo simulation is used to determine the optical properties from the measurements. The results of measurement show that the optical properties and their differences vary with a change of laser wavelength for normal and adenomatous colon mucosa/submucosa and normal and adenomatous colon muscle layer/chorion. The maximum absorption coefficients for normal and adenomatous human colon mucosa/submucosa are 680 nm, and the minimum absorption coefficients for both are 890 nm. The maximum difference of the absorption coefficients between both is 56.8% at 780 nm. The maximum scattering coefficients for normal and adenomatous colon mucosa/submucosa are 890 nm, and the minimum scattering coefficients for both are 780 nm. The maximum difference of the scattering coefficients between both is 10.6% at 780 nm. The maximum absorption coefficients for normal and adenomatous colon muscle layer/chorion are 680 nm, and the minimum absorption coefficients for both are 890 nm. The maximum difference of the absorption coefficients between both is 47.9% at 780 nm. The maximum scattering coefficients for normal and adenomatous colon muscle layer/chorion are 890 nm, and the minimum scattering coefficients for both are 680 nm. The maximum difference of the scattering coefficients between both is 9.61% at 850 nm. The differences in absorption coefficients between normal and adenomatous tissues are more significant than those in scattering coefficients.

Changes of higher order aberration with various pupil sizes in the myopic eye.

PURPOSE: To investigate the effect of pupil diameter on higher order aberration in myopic eyes. METHODS: One hundred and two eyes of 51 normal subjects were evaluated with the Nidek OPD-Scan. RESULTS: All types of aberration increased significantly with increasing pupil size (P<.001). However, optical aberrations had a less pronounced increase in C3(-1) and C3(+1) , more pronounced increase in C4(0) with pupil area increased (P<.05), 2nd coma (C5(-1) and C6(+1)) and high order astigmatism (C4(-2), C4(+2) C6(-2), C6(+2) with larger pupil diameter. Compared with the aberrations of each high order aberration at 4 mm, the average increase root mean square values were 1.54, 1.59, 1.71, and 1.87 on S3, S4, S5, and S6 respectively, with a 5-mm pupil, whereas increased root mean square values were 1.46, 1.88, 1.51, and 1.60 for a 6-mm-diameter pupil. CONCLUSION: For an equal increase of pupil size, not all Zernike polynomial coefficients induced equivalent increase of values. Coma-like aberrations increased less with pupil dilation. Spherical-like aberration showed only a small increase from 4 mm to 5 mm pupil size, but a larger increase from 5 mm to 6 mm pupil size. Other higher order aberrations (S5 and S6) increased slightly with pupil dilation. Coma-like aberration was larger than spherical aberration, and larger than other higher order aberrations for all pupil sizes.
