Antropyloroduodenal motility after resection of esophageal cancer.

OBJECTIVE: To study the mechanism of delayed emptying indigestible solids in thoracic stomach after the resection of esophageal cancer by monitoring the variety of antropyloroduodenal migrating motor complex (MMC) during interdigestion. METHODS: Esophagectomy and gastroesophagostomy in the neck was performed in 5 men with esophageal cancer. An eight-lumen manometric sleeve assembly was passed through a nostril into the duodenum during operation. The sleeve was astride the pylorus. Antropyloroduodenal manometry was performed for at least 300 min with a polygraphic system (PC POLYGRAF HR) 7 - 11 days after operation. RESULTS: Twenty-eight MMCs were recorded in the pylorus and duodenum, 12 MMC(s) in the antrum, and 33% (4/12) of the motility frequency of antral phases III started after that of pyloroduodenal phases III. The number of MMC in the antrum was lower than that in the duodenum after the operation. The mean duration of MMC of the antrum, pylorus and duodenum was (49.2 +/- 10.5) min, (46.5 +/- 10.4) min and (45.9 +/- 10.0) min respectively. The mean duration of phases III was respectively (6.7 +/- 3.5) min in the antrum, (10.0 +/- 3.5) min in pylorus, and (8.0 +/- 3.9) min in duodenum. The mean wave amplitude of phases III was respectively (83 +/- 30) mm Hg in the antrum, (60 +/- 12) mm Hg in pylorus, and (55 +/- 4) mm Hg in duodenum. The mean duration of MMC of the antrum and duodenum was shorter in patients than that in healthy volunteers, and the mean wave amplitude of phases III of the antrum was lower in patients than that in healthy volunteers. CONCLUSIONS: Diminution of the number of MMC and the mean wave amplitude of phases III in the antrum, incoordination of the antropyloroduodenal phases III should be the most important one of the mechanisms of delayed emptying of indigestible solids in thoracic stomach after the resection of esophageal cancer.

Work, social, and family disabilities of subjects with anxiety and depression.

BACKGROUND: Functional impairment scales are increasingly used to evaluate subjects with a variety of mental disorders. METHODS: We evaluated the work, social, and family disabilities of 228 subjects with 6 common anxiety and depressive disorders, as assessed by the Sheehan Disability Scale (major depression, panic disorder, generalized anxiety disorder, social phobia, obsessive-compulsive disorder, and mixed anxiety and depression). RESULTS: Subjects in the 6 diagnostic groups had significantly higher work, social, and family disability scores than control subjects, with the exception of the social phobia and panic disorder subjects' scores for family disability. Those with depressive disorders tended to have significantly higher family disability scores than the anxiety disorder subjects. CONCLUSION: Subjects with both anxiety and depressive disorders from primary care clinics and from chronic psychiatric and medical populations need further evaluation and treatment of the disabilities to decrease the personal, family, and economic burdens of these disorders.

[Cetyltrimethylammonium bromide for fluorescence enhancement of anhydrotetracycline hydrochloride and iso-tetracycline].

Fluorescence enhancement of anhydrotetracycline hydrochloride and iso-tetracycline has been described. The fluorescence intensities of anhydrotetracycline hydrochloride and iso-tetracycline with cetyltrimethylammonium bromide (CTMAB) enhanced by micellar solution have been examined. It is found that fluorescence enhancement of anhydrotetracycline hydrochloride and iso-tetracycline depends on the concentration of CTMAB and pH of the solution. It can be used to develop sensitive methods for the determination of tetracycline hydrochloride and its decomposition product.

Synthesis and characterization of biodegradable hyperbranched poly(ester-amide)s based on natural material.

A series of novel AB3-type monomers were prepared from nontoxic natural gallic acid and amino acids. These monomers were then melt-polycondensed in the presence of MgO as a catalyst via a transesterification process at 170-190 degrees C to yield the hyperbranched poly(ester-amide)s bearing terminal acetyl groups. FTIR and NMR spectra confirmed the structures of all the monomers and polymers. The degrees of branching, estimated from 1H NMR and quantitative 13C NMR spectra, were 0.50-0.68. These hyperbranched polymers displayed moderately high molecular weights. Hydrolytic and enzymatic degradation studies were carried out in vitro at 37.5 degrees C in NaOH hydrotropic solution and in Tris-HCl buffer (pH = 8.6) containing proteinase K, respectively. The results indicate that the hyperbranched poly(ester-amide)s are degradable hydrolytically as well as enzymatically, and the rate of hydrolytic degradation increases with the pH value of the solution.

Array-based multiplexed screening and quantitation of human cytokines and chemokines.

HydroGel-coated slide is a porous substrate based on a polymer matrix that provides a three-dimensional hydrophilic environment similar to free solution suitable for biomolecular interactions. This substrate has been used to develop fluorescence-based multiplexed cytokine immunoassays. Forty-three monoclonal antibodies (mAb) of cytokines and chemokines were printed at a volume of 350 pL per spot using a Packard BioChip Arrayer. For each probe, four replicates were printed at a pitch of 500 microm in the layout of a 13 x 16 pattern on a 12 x 12 mm2 HydroGel pad. Cytokines and chemokines that are captured by the arrayed mAbs are detected by using another biotinylated mAb, following by the addition of a Texas Red-conjugated streptavidin. The fluorescent images of arrays were recorded using a Packard ScanArray 5000 confocal slide scanner and quantitated using Packard QuantArray software. Experiments demonstrated that 43 cytokines and chemokines could be simultaneously screened and quantitated in conditioned culture media, cell lysates, and human plasma. Using this chip, we have examined cytokine expression in breast cancer cells and identified the chemokines associated with human cervical cancers.
