Clinical and functional characterization of a human ORNT1 mutation (T32R) in the hyperornithinemia-hyperammonemia-homocitrullinuria (HHH) syndrome.

We studied two related families (HHH013 and HHH015) with the hyperornithinemia-hyperammonemia-homocitrullinuria (HHH) syndrome, a disorder of the urea cycle and ornithine degradation pathway, who have the same novel ornithine transporter (ORNT1) genotype (T32R) but a variable phenotype. Both HHH015 patients are doing well in school and are clinically stable; conversely, the three affected HHH013 siblings had academic difficulties and one suffered recurrent episodes of hyperammonemia and ultimately died. Overexpression studies revealed that the product of the ORNT1-T32R allele has residual function. Ornithine transport studies in HHH015 fibroblasts, however, showed basal activity similar to fibroblasts carrying nonfunctional ORNT1 alleles. We also examined two potential modifying factors, the ORNT2 gene and the mitochondrial DNA lineage (haplogroup). Haplogroups, associated with specific diseases, are hypothesized to influence mitochondrial function. Results demonstrated that both HHH015 patients are heterozygous for an ORNT2 gain of function polymorphism and belong to haplogroup A whereas the HHH013 siblings carry the wild-type ORNT2 and are haplogroup H. These observations suggest that the ORNT1 genotype cannot predict the phenotype of HHH patients. The reason for the phenotypic variability is unknown, but factors such as redundant transporters and mitochondrial lineage may contribute to the neuropathophysiology of HHH patients.

Lysosomal storage disorders.

Lysosomal storage disorders (LSDs) are genetic defects caused by lysosomal hydrolase deficiencies. These deficiencies lead to substrate accumulation affecting cells, tissues and organs. Detecting abnormal compound excretion and deficient enzymes assist diagnosis of these disorders for treatment and prevention. This mini review summarizes clinical presentations and diagnostic workup of LSDs and updates the new development in the area.

[Clinical observation on acupuncture by stages combined with exercise therapy for treatment of Bell palsy at acute stage].

OBJECTIVE: To find out a method for increasing clinical therapeutic effect on Bell palsy at acute stage. METHODS: Ninety cases of Bell palsy were randomly divided into an observation group, a control group I and a control group II, 30 cases in each group. They were treated respectively with acupuncture plus exercise therapy, simple acupuncture therapy, and simple exercise therapy, and their therapeutic effects were observed. RESULTS: The cured rate was 66.7% in the observation group, 53.3% in the control group I and 46.70% in the control group II, the observation group being better than the two control groups (P<0.05). CONCLUSION: Acupuncture by stage combined with exercise therapy can increase the therapeutic effect on Bell palsy at acute stage, and it is a better therapy for Bell palsy.

[Clinical observation on effect of fuzheng yiliu granule on cell cycle and nuclear transcription factor-kappa B in tissue of esophageal-gastric carcinoma].

OBJECTIVE: To observe the effect of Fuzheng Yiliu Granule (FZYLG) on cell cycle and nuclear transcription factor-kappa B (NF-kappa B) in tissue of esophageal-gastric carcinoma. METHODS: Seventy-six patients with esophageal gastric carcinoma were randomly divided into two groups, the FZYLG group and the control group. FZYLG was given to the former for 15 days. The tumor tissue in both groups was resected and cell cycle and apoptosis rate as well as NF-kappa B were determined by flowcytometry. RESULTS: Level of NF-kappa B in the treated group was significantly higher than that in the control group (P < 0.05). In the treated group, the percentage of G0/G1 stage cells were significantly increased and that of S stage significantly decreased (both P < 0.05). At the same time, obvious cell apoptosis was found in the treated group, the apoptosis rate of which was significantly higher than that in the control group (P < 0.01). CONCLUSION: FZYLG can increase the NF-kappa B expression, block the proliferation to promote the apoptosis of tumor cells.
