[A rabbit model of cervical spondylosis established by stimulation of wind, cold and dampness].

OBJECTIVE: To make an animal model of cervical spondylosis (arthralgia syndrome type) with stimulation of wind, cold, and dampness. METHODS: Twenty-four 8 months old male New Zealand white rabbits were randomly allocated into four groups: normal control group, light stimulation group, moderate stimulation group and severe stimulation group. The wind speed was 10.8-13.8 m/s, the temperature was (5+/-0.5)degrees centigrade, and the humidity was 100%. The rabbits of light, moderate, and severe stimulation groups were kept in the above-mentioned environments for 4 hours everyday, and for a total of 32, 64, and 128 hours, respectively. The intervertebral discs were stained with HE method, and observed with a light microscope. Prostaglandin E(2) (PGE(2)), 6-ketone-prostaglandin F1alpha (6-K-PGF(1alpha)) and thromboxane B(2) (TXB(2)) contents were measured by ELISA. Fas and Bcl-2 expressions were examined by immunohistochemical avidin-biotin peroxidose complex technique. Interleukin-1beta (IL-1beta), tumor necrosis factor alpha (TNF-alpha), and transforming growth factor beta (TGF-beta) mRNA expressions were examined by reverse transcription-polymerase chain reaction. RESULTS: The nucleus pulposus of rabbits in the light and moderate stimulation groups shrunken, and in the severe stimulation group, the anulus fibrosus loosed or ruptured, and the cartilage end-plate became proliferated. Compared with rabbits in the normal control group, the PGE(2) content rose in the light stimulation group, the contents of PGE(2), 6-K-PGF(1alpha), and TXB(2) increased, the expressions of IL-1beta and TNF-alpha mRNAs and Fas were up-regulated, and the expressions of TGF-beta mRNA and Bcl-2 were down-regulated in the moderate and severe stimulation groups. The expression of Fas was up-regulated mostly and Bcl-2 was down-regulated mostly in the severe group. CONCLUSION: Moderate and severe stimulations of wind, cold and dampness can lead to degeneration of cervical intervertebral discs of rabbits. The model corresponds to the theory of traditional Chinese medicine about arthralgia syndrome caused by wind, cold and dampness.

[The clinical manifestations and assessment of post stroke dysphagia].

OBJECTIVE: To find the relationship between the clinical manifestations of dysphagia resulting from stroke and dysfunction of swallowing organs. It is helpful to understand the mechanism of dysphagia so as to guide its diagnosis and therapy. METHODS: Oral facial examination experimental swallowing methods were used to identify the abnormal manifestations of dysphagia. Spearman correlation analysis was used as statistic method. Sensitivity (SN), specificity (SP), predictive values (PV+/-) were calculated. RESULTS: There were about 20 or so abnormal manifestations after stroke and swallowing organs dysfunction include abnormal lip closure, decreased tongue motility, weakness of palate, decreased or disappeared gag reflex, abnormal lift of larynx and insufficient opening of cricopharyngeal muscle. The sensitivity and negative predictive values were high in all the dysfunction organs with the highest sensitivity, specificity, positive and negative predictive value in abnormal lip closure (84.62%), decreased tongue motility (67.75%), insufficient opening of cricopharyngeal muscle (80.96%) and abnormal lip closure (90.91%). CONCLUSIONS: It is comparatively accurate for clinical evaluation to judge dysfunction in lip closure, lingual movement and larynx elevation.

[Correlation between plasma level of homocysteine and cerebral large-artery atherosclerosis].

OBJECTIVE: To identify whether high level of plasma homocysteine is an independent risk factor for potential cerebral large-artery atherosclerotic stenosis determined by digital subtraction angiography (DSA) in patients with ischemic stroke. METHODS: 276 adult patients with ischemic stroke experienced DSA were selected, and 224 patients with potential large-artery atherosclerotic stenosis (grade > 50%, not less than one of vascular) and 52 controls without stenosis or mild grade (grade < or = 50%) were included. Plasma homocysteine level was analyzed and conventional risk factors of cerebral stroke such as sex and age, hypertension, diabetes, long-smoking, long-alcohol and plasma levels of triglyceride, high-density lipoprotein, low-density lipoprotein was evaluated in 276 subjects. RESULTS: The proportion of subjects with high level of plasma homocysteine (> 15 micromol/L) was significantly higher in patients with potential atherosclerotic stenosis than those without (38.8% versus 15.4%; OR 3.49, 95% CI 1.57 - 7.77, P = 0.001). Based on multivariable stepwise logistic regression model, the odds ratio of homocysteine levels was 4.10 (95% CI 1.80 - 9.32, P = 0.001) with additional adjustment for conventional risk factors. The effect of homocysteine was more pronounced in the presence of hypertension (OR = 4.89), old age (OR = 4.79) and low level of plasma high density lipoprotein (OR = 10.46). CONCLUSIONS: It is suggested that high level of plasma homocysteine is an independent risk factor for cerebral large-artery atherosclerosis. The effect of homocysteine is more pronounced in the presence of other risk factors of ischemic stroke.

[Neuroprotection by resveratrol against cytotoxicity of oxidized low density lipoprotein in PC12 cells].

OBJECTIVE: To investigate neuroprotection of resveratrol against cytotoxicity of oxidized low density lipoprotein (oxLDL) in PC12 cells. METHODS: PC12 cells were pretreated with resveratrol for one hour as a neuron model and then exposed to oxLDL at varied concentrations. Effects of resveratrol on cell viability, permeability of cell membrane, cell nucleus and activity of caspase-3 were evaluated with MTT assay, lactic acid dehydrogenase (LDH) release assay, DNA fragmentation (TUNEL) assay and assay for caspase-3 activity (caspase assay). RESULTS: Cell viability, LDH release rate, percentage of cells with positive TUNEL and activity of caspase-3 were (62 +/- 3)%, (23 +/- 3)%, (26 +/- 5)% and (0.811 +/- 0.049) mol.min(-1).microg(-1), vs (84 +/- 7)%, (13 +/- 4)%, (12 +/- 4)% and (0.553 +/- 0.048) mol.min(-1).microg(-1) in PC12 cells treated with 10 mg/L oxLDL vs in those treated with 10 mg/L oxLDL plus 50 micromol/L resveratrol, respectively, with statistically significant difference. CONCLUSION: Resveratrol could attenuate cytotoxicity induced by oxLDL in PC12 cells with neuroprotection.

[A study on drug compliance in secondary prevention of ischemic stroke].

OBJECTIVE: The aim of the present study is to determine the rate of compliance with secondary stroke prevention one year after ischemic stroke and to identify the reasons for poor compliance. METHODS: The proportion of patients compliant with prevention of risk factors (hypertension, diabetes, hyperlipidemia) was evaluated through structured telephone interviews. RESULTS: Of the consecutive patients admitted to the neurological department of Tiantan Hospital between Oct. 2002 and Apr. 2003, 374 patients were involved in this investigation and 296 (79.1%) were finally evaluated. In the 296 (79.1%) patients, the rate of antithrombotic drug compliance was 35.1%. Of the patients with hypertension, diabetes, and hyperlipidemia, 77.9%, 80.2%, and 48.4% were still treated for their respective risk factors. Influence factors for drug compliance, such as medical insurance and free medical care (OR 2.117, 95% CI 1.174-3.821) can promote the drug compliance in stroke patients, while the use of antithrombotic drugs other than aspirin (OR 0.352, 95% CI 0.153-0.812) and a lower living ability (62.5 +/- 13.3) (Mann-Whitney-P = 0.000) could decrease the drug compliance. CONCLUSIONS: Long-term compliance with secondary prevention in patients with ischemic stroke is poor in the setting of our study. Poor compliance with antithrombotic therapy such as incorrect discontinuation or change and reduction of dosage was noted. Means of payment, income levels, categories of antithrombotic drugs and personal living ability of patients are associated with compliance. Doctors should attach importance to secondary prevention and provide stroke patients detailed guidelines for the use of preventive drugs.

[The reliability of ischemic stroke subtype classification using the TOAST criteria].

OBJECTIVE: The aim of this study was to assess the reliability of the classification of acute cerebral infarction based on the TOAST system. METHODS: 300 patients with ischemic stroke were recruited into this study. Ischemic stroke was classified into five subtypes according on clinical manifestations, MRI, MRA and DSA. The investigators classified all the patients into five etiologic subtypes. The data was analyzed by the statistics software of SPSS 11.5. RESULTS: The value of Kappa about the classification's reliability was 0.8, P = 0.028. CONCLUSIONS: The reliability for the classification of acute cerebral was excellent. This value could be improved with the development of the diagnosis tools and the physicians' recognition.

[The impact of stroke unit on early outcome of cerebral infarction patients].

OBJECTIVE: To investigate the early therapeutic effect of stroke unit on cerebral infarction patients. METHODS: 285 of acute cerebral infarction were admitted into our department from December 2001 to January 2003. All the patients were assigned either into stroke unit (SU) or general ward (GW) randomly to receive treatment and the data of the patients of the two groups were compared. The key indexes recorded were mortality and hospitalization time and other aprameters observed were Barthel Index (BI), National Institutes of Health Stroke Scale (NIHSS), Oxford Handicap Scale (OHS). All the patients or their family members were asked to have the Visual Analogue Scales (VAS), which reflects patient satisfaction, at the time of discharge. All these data, incorporated with cost and effect, were analyzed with SPSS soft packs. RESULTS: There was no statistical difference between the two groups, but the mean gap scores between the day of admission and discharge demonstrated significant difference (P < 0.01) in BI (SU 21.01 +/- 24.4, GW 8.49 +/- 22.39), NIHSS (SU -2.53 +/- 5.73, GW 0.27 +/- 6.80) or OHS (SU -0.79 +/- 0.95, GW -0.23 +/- 0.97). Mortality rate was less (P < 0.05) and patient satisfaction higher in SU than in GW (P < 0.01). The ratio of rehabilitation was higher (P < 0.01) and complications less in SU than in GW (P < 0.01). Moreover, the cost of hospitalization was less in SU. CONCLUSION: For cerebral infarction patients, SU is able to improve the daily activities in early stage, reduce neurological defects and restore social abilities with less expenses.

[Protective effect of continuous veno-venous hemofiltration on tissue and organ damage in patients with severe acute pancreatitis].

OBJECTIVE: To explore the protective effect of continuous veno-venous hemofiltration (CVVH) on tissue and organ damage induced by severe acute pancreatitis (SAP). METHODS: Thirty-seven SAP patients were randomly divided into CVVH group (n=22) and control group (n=15). Patients in control group were treated with conventional therapy, while patients in CVVH group were treated with conventional therapy combined with CVVH. Serum amylase (AMS) and lipase (Lip), the plasma levels of endotoxin, tumor necrosis factor-alpha (TNF-alpha), interleukin-1beta (IL-1beta), and C-reactive protein (CRP), and faecal occult blood test and blood serum leukocyte were determined. Functions of brain, kidney, liver, heart and lung were observed. RESULTS: The treatment effect was better in CVVH group than that in normal group. The mortality rate was lower and the number of hospitalization day was decreased in CVVH group than those in normal group, they were 18.2 percent vs. 33.3 percent and (18.3+/-5.7) days vs. (27.5+/-8.6) days. CONCLUSION: CVVH has a significant protective effect against tissue and organ damage induced by SAP.

[Biodiv Ysio stent for symptomatic stenosis of middle cerebral artery].

OBJECTIVE: To assess the safety and feasibility of Biodiv Ysio stent for patients with symptomatic M1 stenosis of middle cerebral artery (MCA). METHODS: Forty-three patients with forty-five M1 stenoses, ranged from 50% to 99% in diameter reduction and refractory to medical therapy, were enrolled in this study between March 2002 and November 2003. The lesions were situated at M1 trunk (n = 19), M1 origin (n = 12) and M1 bifurcation (n = 14), respectively. RESULTS: The technical successful rate was 97.8% (44/45) for all the lesions. The rate of complicating subarachnoid hemorrhage was 7.0% (3/43 patients) and the rate of death was 2.3%. During a follow-up of 7.0 months (median), there was no recurrence of transient ischemic attack or stroke in 41 available patients. Six-month angiographic follow-up was obtained in 6 patients (7 vessels), demonstrating good patency in 6 stenting vessels and restenosis in one vessel. CONCLUSIONS: Angioplasty associated with Biodiv Ysio stent appears to be a safe and feasible for the patients with symptomatic M1 stenosis of MCA, under strict control of periperformeral project. Further study is, however needed.

Assessment of the early effectiveness of a stroke unit in comparison to the general ward.

BACKGROUND: Stroke unit is the most effective treatment method to benefit stroke patients. Our study is to evaluate the early effectiveness of a hospital stroke unit (SU). METHODS: Three hundred and ninety-two patients who had suffered from acute strokes and who were admitted to our hospital between December 2001 and January 2003 were recruited for this controlled study. All patients were sent at random to either the SU or the general ward (GW) for treatment. The following indices were measured by: Barthel Index (BI), National Institute of Health Stroke Scale (NIHSS), Oxford Handicap Scale (OHS). RESULTS: The mean change in BI score between the day of admission and the day of discharge was 20.00 +/- 24.36 for the SU group and 10.63 +/- 23.59 for the GW group. A difference that is statistically significant (P = 0.000). The mean change in NIHSS score was -2.01 +/- 6.61 for the SU group and 0.55 +/- 7.44 for the GW group. A difference that is also statistically significant (P = 0.000). Finally, the mean change in OHS score was -0.74 +/- 1.04 for the SU group and -0.28 +/- 0.98 for the GW group, also a statistically significant difference (P = 0.000). Among SU patients, patient satisfaction was higher (P = 0.000), the rehabilitation success rate was higher (P = 0.000), and there were fewer complications (P = 0.000). CONCLUSION: Compared to GW patients, stroke patients treated in a special SU were able to return to normal daily activities earlier, with better social abilities, and have reduced neurological defects, without increasing the overall economic burden.

[TOAST subtyping of acute ischemic stroke].

OBJECTIVE: To determine the etiological characteristics of ischemic stroke in Chinese people on the basis of Trial of Org 10 172 in Acute Stroke Treatment (TOAST) criteria. METHODS: 300 patients with ischemic stroke were recruited into this study. Ischemic stroke was classified into five subtypes based on clinical manifestations, MRI, MRA and DSA. RESULTS: Of all the ischemic stroke patients, 12.3%, 40.0%, 31.3%, 5.0%, and 11.4% were classified as cardio embolism, large-artery atherosclerotic, lacunare, of other demonstrated etiology and of undetermined etiology. CONCLUSIONS: The statistical comparison between male and female of each subtype is not significant. The relative incidence of large-artery atherosclerosis in Chinese patients is higher than that of all the other four subtypes.

[Flow cytometric detection and significance of four cyclins in esophageal cancer].

OBJECTIVE: To study the expression and significance of cyclin E, cyclin D1, CDK4 and p27 protein in esophageal squamous cell cancer (ESCC) and their correlation with tumor differentiation and lymph node metastasis. METHODS: Expressions of cyclin E, cyclin D1, CDK4 and p27 protein in 65 patients with ESCC were quantitatively detected by flow cytometry. RESULTS: The expressions of cyclin E, cyclin D1, CDK4 in poorly-differentiated ESCC were higher than those in well-differentiated ESCC (P = 0.0275, 0.0001, 0.0174). The expression of p27 in poorly-differentiated ESCC was lower than that in well-differentiated ESCC (P = 0.0042). There was positive correlation between cyclin E and cyclin D1, cyclin D1 and CDK4, but negative correlation between cyclin D1 and p27. The expressions of all four proteins were not correlated with lymph node metastasis. CONCLUSION: The expressions of cyclin E, cyclin D1, CDK4 and p27 are closely related to tumor differentiation of ESCC. An imbalance between positive and negative control of cell cycling might be critical in the carcinogenesis of esophageal squamous cell cancer.

[Infrequent X chromosome abnormality and X-linked syndromic deafness].

OBJECTIVE: To make clear the relationship between the X chromosome abnormality and sydromic deafness through genetic analysis of a pedigree with X-linked syndromic deafness. METHODS: The chromosome number and structure of the family members were analyzed by the standard and high resolution banding with Giemsa, and fluorescent in situ hybridization. The allelic number of the DNA segment in X chromosome was studied with genetic markers. RESULTS: The 2 probands, their mothers and grandmother with normal phenotype all had X(p22-pter) duplication. The whole X chromosome of both the proband III-3 and his mother could be stained with X chromosome staining probe. The proband III-3 had 2 copies of DXS7108. CONCLUSION: The abnormal X chromosome occurring in this pedigree of X-linked syndromic deafness derives from partial Xp duplication, which will guide further research to identify the breakpoint of this abnormal chromosome.
