Combination of FSH priming and hCG priming for in-vitro maturation of human oocytes.

BACKGROUND: The purpose of this study was to determine if there is any additional benefit from FSH priming in addition to hCG priming on in-vitro maturation (IVM) programmes. METHODS: Sixty women with polycystic ovary syndrome (PCOS) who underwent 68 IVM cycles were randomized by computer-generated numbers to receive FSH stimulation or not. Thirty-five cycles were pretreated with 75 IU rFSH for 6 days, and 33 cycles were not. Every cycle was given hCG 10,000 IU 36 h before oocyte retrieval. Immature oocytes were matured in vitro and fertilized by ICSI, and the resulting embryos were replaced on day 2 or 3. RESULTS: A total of 1528 immature oocytes were recovered. The overall maturation and fertilization rates were 74.2 and 72.8% respectively. After embryo transfer, 23 pregnancies resulted (33.8%). The oocyte numbers and endometrial thickness were similar between FSH-primed and non-FSH-primed groups. Serum estradiol level on the day of hCG injection was significantly higher in the FSH-primed group than in the non-FSH-primed group (377.2 pmol/l versus 143.8 pmol/l, P=0.001). The maturation rate, fertilization rate and pregnancy rate were 76.5, 75.8 and 31.4% respectively for FSH-primed group, and 71.9, 69.5 and 36.4% respectively for non-FSH-primed group (all not significant). CONCLUSIONS: IVM is a feasible treatment for women with PCOS. FSH priming has no additional beneficial effect on IVM.

Successful laparoscopic management of a huge ovarian tumor in the 27th week of pregnancy. A case report.

BACKGROUND: Laparoscopic surgery in the second trimester is very rare. The enlarging uterus impedes surgical procedures and increases the risk of surgery. We report on a woman in the 27th week of pregnancy who developed a huge ovarian tumor. CASE: A woman underwent laparoscopic surgery in the 27th week of gestation because of a rapid growing ovarian tumor. After aspiration of its contents, the tumor was extracted, and cystectomy was performed extracorporeally. The postoperative course was uneventful and the woman delivered a healthy infant at 42 weeks. CONCLUSION: Laparoscopic gynecologic surgery can be performed through the 27th week of gestation, and extracorporeal manipulation makes the procedure much easier.

Heterotopic triplet pregnancy: report of a case with bilateral tubal pregnancy and an intrauterine pregnancy.

The first report of an ectopic pregnancy following IVF was published in 1976, and since then heterotopic pregnancies (HPs) have been reported at an increasing rate. Although cases of the co-existence of a bilateral tubal and an intrauterine pregnancy following IVF-embryo transfer have been reported, a case of heterotopic triplet pregnancy caused by unilateral tubal embryo transfer has not yet been published in the literature. Here we report on a 38-year-old women (gravida 3, para 1) with a history of infertility who presented to our infertility clinic for evaluation. Hysterosalpingography revealed bilaterally patent Fallopian tubes and stricture of the cervical canal. She conceived after receiving HMG combined with pure FSH, followed by IVF-tubal embryo transfer. Four embryos were replaced into the right tube. Approximately 5 weeks after tubal embryo transfer, the patient presented with lower abdominal tenderness and shock due to internal bleeding. She underwent an emergency laparotomy under the impression of HP. Bilateral tubal pregnancy with right tubal rupture was noted during the operation. The post-operative course was uneventful. Early intervention and thorough inspection of the peritoneal cavity in patients with haemodynamic instability can prevent jeopardizing the life of the mother as well as the ongoing pregnancy.

Amniotic membrane grafting to treat refractory labial adhesions postpartum. A case report.

BACKGROUND: Labial adhesions are common in prepubertal children, but their occurrence postpartum is extremely rare. CASE: A woman developed labial adhesions after a vaginal delivery. Topical estrogen cream was ineffective. Simple excision was performed, but the adhesions reformed two weeks later. Finally, amniotic membranes were used to cover the labia minora after they were separated by excision. The wound healed without adhesion formation. CONCLUSION: Excision plus amniotic membrane grafting is effective for recurrent labial adhesions.

Use of sublingual buprenorphine for pain relief in office hysteroscopy.

STUDY OBJECTIVE: To assess the efficacy of sublingual buprenorphine in the relief of pain associated with office hysteroscopy. DESIGN: Prospective, randomized study (Canadian Task Force classification I). SETTING: Tertiary medical center. PATIENTS: One hundred sixty-four women referred for office hysteroscopy from September 2003 through March 2004. INTERVENTION: Before hysteroscopy, 80 women received a tablet of buprenorphine (group A), and 84 women received a placebo (group B). Their pain sensations were evaluated on a 10-cm visual analog scale, and they were asked about the adverse reactions and level of satisfaction on the following day. MEASUREMENTS AND MAIN RESULTS: The pain score in group A was 3.3 +/- 1.1, which was similar to 3.2 +/- 1.3 in group B. The pain scores in subgroups of women also were similar within the same group and between the two groups. Thirty-one women (38.8%) in group A reported adverse reactions, including nausea, vomiting, and drowsiness, while none in group B reported any adverse reactions. CONCLUSION: Office hysteroscopy with a 3.1-mm flexible hysteroscope is a well-tolerated procedure. Sublingual buprenorphine is not helpful in relieving the pain associated with hysteroscopy but is associated with significant adverse reactions.
