[Scientific consideration and topic selection in the field of andrology].

The improvement of diagnosis and treatment of andrological disease depends on scientific advances in the corresponding domain of andrology. In this article, the author thought topic selection was of great significance for the improvement of diagnosis and treatment based on his clinical and scientific experience for years in the field of andrology. Topic selection should be closely combined with clinical practice and follow the principles of importance, innovation, feasibility and reality, which meets the demands of basic, therapeutic and epidemiologic aspects in andrology research. Several common approaches to topic selection were discussed as well in the article.

[Comparison of four methods for sperm counting].

OBJECTIVE: To evaluate the accuracy and precision of 4 methods including Hemacytometer, Makler chamber, Cell-VU chamber, and computer-aided semen analysis for determining sperm concentration. METHODS: Latex bead solutions with concentrations known as (35 +/- 5) x 10(6)/ml and (18.0 +/- 2.5) x 10(6)/ml and semen samples (n = 54) were counted by the above 4 methods and the results were then compared. RESULTS: Mean bead concentrations for Hemacytometer, Makler, Cell-VU chambers and CASA were (44.84 +/- 4.86), (52.36 +/- 7.78), (39.70 +/- 4.76), (28.53 +/- 2.06) x 10(6)/ml respectively for the standard solution containing (35 +/- 5) x 10(6)/ml, and (21.04 +/- 1.87), (24.54 +/- 3.67), (19.09 +/- 2.02), (14.62 +/- 0.95) x 10(6)/ml respectively for a standard solution containing (18 +/- 2.5) x 10(6)/ml. The results of Cell-VU chamber were consistently similar and close to the standard solutions, while those of Hemacytometer, Makler chambers were overestimated, and those of CASA were underestimated. The coefficients of variation for Hemacytometer, Makler, Cell-VU chambers and CASA were 10.81%, 14.86%, 12.80%, and 7.22% respectively for a higher standard solution, while 8.89%, 14.96%, 10.58%, and 6.50% respectively for a lower standard solution. CASA has the lowest CV%. When semen samples were counted, the results of Hemacytometer, Makler, Cell-VU chambers and CASA were (76.98 +/- 59.90), (63.89 +/- 53.84), (45.28 +/- 34.52), (41.96 +/- 31.93) x 10(6)/ml respectively. There wasn't any significant difference either between Cell-VU chamber and CASA (P = 0.71), or between Hemacytometer and Makler chamber (P = 0.14), while there was significant difference between Cell-VU chamber or CASA and Hemacytometer or Makler chamber (P < 0.05 or P <0.01). CONCLUSION: When counting semen sample, there wasnt any significant difference between Cell-VU chamber and CASA. Each laboratory can select its own proper method for manual or computer-aided analysis.

[Clinical observation of the treatment of male sexual dysfunction by yangjingjiaonang].

OBJECTIVE: To observe the therapeutic effect of Yangjingjiaonang on the treatment of male sexual dysfunction. METHODS: Two hundred and ten patients with male sexual dysfunction were divided into 2 groups, the treatment group orally delivered with Yangjingjiaonang, and the control group administrated with Wuziyanzongwan. The treatment lasted for 60 days, and the changes of sexual function were compared and measured by the total scores of IIEF. RESULTS: After taking orally drugs for 60 days, the total scores of IIEF-5 and the total scores of the questions 11 and 12 of IIEF in treatment group elevated from (11.26 +/- 2. 72) score and (5.12 +/- 1.16) score to (16.84 +/- 3.12) score and (8.50 +/- 1.02) score, respectively, showing a significant improvement in the treatment group compared with the control (P <0.01). There was no significant difference for the treatment of premature ejaculation between the 2 groups. CONCLUSION: Yangjingjiaonang has been demonstrated to have apparent therapeutic effect on improving male sexual function.

[Construction and identification for cell strain of anti-human sperm protein 22 monoclonal antibodies].

OBJECTIVE: To prepare monoclonal antibodies of mice anti-human sperm protein 22 (SP22) and to identify their specificities. METHODS: BALB/c mice were immunized with human SP22, monoclonal antibodies were prepared by hybridoma technique and the sensitivity and specificity of SP22 McAb were investigated by ELISA and Western-blot assay, respectively. The distribution of human SP22 in sperm were shown by immunohistochemistry. RESULTS: Three strains of hybridoma cells were obtained, with the affinity constant (K) of 1.0 x 10(7) L/mol and the titers were 1:10(3) and 1:3,200 in the mixed supernatant of cell cultures and abdominal dropsy, respectively. IgG isotype of the antibody was identified as IgG1. Western blot demonstrated that there was a specific recognition between human SP22 and the obtained monoclonal antibody. Immunohistochemistry displayed that human SP22 mainly distributed on the acrosome surface of human sperm. CONCLUSION: The monoclonal antibodies of anti-human 22 was prepared by the technique of hybridoma cell has higher titer and specificity, which can combine specially with the SP22 protein on the surface of human sperm.

[Detection of peripheral blood Th1/Th2 cell ratio in patients with chronic abacterial prostatitis/chronic pelvic pain syndrome].

OBJECTIVE: To evaluate the change of peripheral Th1/Th2 cells in patients with CAP/CPPS and its significance in various clinical types of CAP/CPPS. METHODS: Interferon-gamma (IFN-gamma) and interleukin-4 (IL-4) secreted by CD3+ CD8(-)T cells were detected by flow cytometry in 35 patients with CAP/CPPS and 12 healthy volunteers, and significance of Th1/Th2 cells ratio in the etiology of CAP/CPPS was analyzed. RESULTS: Compared with control group, peripheral Th1 cells were significantly increased in patients with IIIA and III B CAP/CPPS, the Th1/Th2 cell ratio was apparently increased (P <0.05), while Th2 cells was not different from that in the control(P>0.05 ); Th1, Th2 cells and Th1/Th2 in patients with IIIA CPPS were not statistically different from those in III B CPPS respectively (P > 0.05) . CONCLUSION: There is a Th1/Th2 imbalance in CAP/CPPS patients with an increase of Th1 type cytokines. Th1 cell may play an important role in the pathogenesis of CAP/CPPS.

[Study on fetal SRY gene in maternal plasma using nested polymerase chain reaction].

OBJECTIVE: To develop a nested polymerase chain reaction (PCR) technique for fetal SRY gene identification using cell-free fetal DNA in maternal plasma. METHODS: Peripheral blood samples were obtained from 30 pregnant women and cell-free DNA was extracted by the phenol/chloroform method from plasma. The nested PCR was carried out to amplify the fragment of SRY gene by two sets of PCR primer pairs. Direct sequencing analysis was then performed on the PCR product. RESULTS: Among the 17 women bearing male fetuses, SRY sequences were detected in 15 plasma samples after nested PCR amplification, while none of the 13 women bearing female fetuses had the positive results. The accuracy and sensitivity were 93.3% (28/30) and 88.2% (15/17), respectively. CONCLUSION: The phenol/chloroform extraction for fetal DNA in maternal plasma was effective and simple. And the nested PCR amplification of SRY sequence is a convenient and low-cost approach for the non-invasive early prenatal diagnosis of sex-linked inheritant diseases.

[Preliminary experience in treatment of chronic prostatitis/chronic pelvic pain syndrome with Longjintonglin capsule].

OBJECTIVE: To explore the effectiveness of Longjintonglin capsule in treatment of chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS). METHODS: One hundred and thirty-six patients accorded with the demands of chronic prostatitis type III of National Institute of Health (NIH), among them there were 84 cases of chronic prostatitis type IIIA and 52 cases of type IIIB. All of the patients accepted oral Longjintonglin capsule for 3 tablets, 3 times per day, for 3 months. NIH- chronic prostatitis symptom index (NIH-CPSI) and counting leukocytes in expressed prostatic secretions (EPS) were used as target to evaluate the effectiveness. RESULTS: One hundred and thirty-one cases accomplished treatment, in which there were 81 cases of type IIIA, and 50 cases of type IIIB. Overall scores of NIH-CPSI before and after treatment were(24.8 +/- 5.6) and (13.3 +/- 6.8) points respectively, which dropped significantly (average 11.5 points), (P < 0.01). The symptom scores before and after treatment were (15.4 +/- 4.3) and (8.7 +/- 3.9) respectively, which also decreased significantly (average 6.7 points), (P <0.01). The scores for quality of life before and after treatment were (9.4 +/- 3.7) and (4.6 +/- 2.7) respectively, which dropped significant (average 4.8 points) (P < 0.01). The leukocytes counts before and after treatment in patients with CP/CPPs type IIIA were (21.7 +/- 14.5)/HP and (8.8 +/- 12.6)/HP, respectively. Twenty-four cases(18.3%) got cured, 41 cases (31.3%) notable effect, 40 cases (30.5%) some effect, 26 cases (19.8%) no effect, and total effective rate was 80.1%. No apparent side effect was observed in all patients accepted treatment, especially for their kidney and hepatic function. CONCLUSION: Longjintonglin capsule was a safe and effective medicine in treatment of CP/CPPS without significant side effect.

[A brief introduction to MEDLINE and a guide for authors].

Medical literature analysis and retrieval system online (MEDLINE) established by National Library of Medicine, USA (NLM) is the most important biomedical database all over the world. This article introduces the database of MEDLINE and Chinese periodicals indexed in MEDLINE in 2006. Several suggestions are made as well, with the aim of providing a guide for authors to contribute to periodicals indexed in MEDLINE so as to extend the international influence of the articles, authors and related hospitals.

[Etiologic analysis of globozoospermia: a 1-case report].

OBJECTIVE: To investigate the etiologic factors of globozoospermia. METHODS: Routine semen analysis, sperm DNA special staining and chromosomal karyotype detection of peripherical blood lymphocytes were performed in a globozoospermia patient. RESULTS: Round-headed spermatozoa were lack of acrosome and the acrosin activity was low. Meanwhile, there was an additional band located in the Y chromosomal short arm. CONCLUSION: Lack of acrosome, low acrosin activity and abnormality of chromosome may be the main reasons for globozoospermia.

[Application of distilled water in sperm counting and hypoosmotic swelling test].

OBJECTIVE: To evaluate the application of distilled water in sperm-counting and hypoosmotic swelling test. METHODS: Thirty-seven semen samples were collected and each was diluted by distilled water and sodium acid carbonate-formaldehyde solution, respectively. Then the hemacytometer was used for sperm counting. Meanwhile, the percentage of swelled sperm diluted by distilled water was compared with the result of hypoosmotic swelling test recommended by WHO. Another 26 semen samples were diluted by distilled water and hypoosmotic swelling solution respectively, and the percentages of the swelled sperm were compared. RESULTS: There was no significant difference either between the sperm concentrations obtained by distilled water and sodium acid carbonate-formaldehyde solution (P > 0.05) or between the percentages of the swelled sperm diluted by distilled water and hypoosmotic swelling solution. CONCLUSION: Distilled water can not only replace sodium acid carbonate-formaldehyde solution for sperm-counting dilution but also be used as a hypoosmotic swelling solution.

[Progress in researches on the relation between chronic prostatitis and cytokines].

Chronic prostatitis is a common andrologic disease. The etiology and pathogensis of the disease are not yet completely understood. This article reviews the origin and the molecular strucure of cytokines and their role in the pathogenesis of prostatitis, especially chronic nonbacterial prostatitis/chronic pelvic pain syndrome.

[A list and analysis of reproductive medicine periodicals indexed in MEDLINE in 2006].

MEDLINE established by National Library of Medicine (NLM), USA, is the most commonly used biomedical literature retrieval system, whose indexed literature generally represents the top of the medical world. Of the total number of 4959 journals listed in the Lists of Journals Indexed in MEDLINE (2006) published by NLM, 46 are periodicals of reproductive medicine, mainly including andrology, fertility, and sterility. Researchers of reproductive medicine should be familiar with the journals indexed in MEDLINE, closely follow their published frontier researches in this field, and consciously contribute to them in English. Editors of reproductive medicine journals should endeavour to have their journals included in such internationally authoritative systems as MEDLINE, SCI, etc by publishing them in English in compliance with international standards. This article presents a list of the reproductive medicine periodicals indexed in MEDLINE (2006) together with a brief analysis, which is meant to help researchers in reproductive medicine, andrology and other relevant fields with their contribution, subscription, reading and retrieving, as well as to share some more knowledge about MEDLINE and its indexed journals with Chinese editors of reproductive medicine journals.

[Evaluation of sperm mitochondrial function using Rh123/PI dual fluorescent staining].

OBJECTIVE: To investigate the feasibility and clinical significance of detecting sperm mitochondrial function by using Rh123/PI dual fluorescent staining and flow cytometry analysis, and to explore the relationship between the results of Rh123/PI dual fluorescent staining and seminal parameters. METHODS: Sixty-three semen samples were classified as normal (n=31) and abnormal (n=32) according to the World Health Organization guidelines. Rh123/PI dual fluorescent staining was then carried out to evaluate sperm mitochondrial function by flow cytometry analysis. RESULTS: Significant differences in Rh123+ PI-, Rh123- /PI+ and Rh123- /PI- were detected between the normal and abnormal semen samples (P < 0.05). There was a significant positive correlation between the Rh123+ PI- sperm and sperm motility and a significant inverse correlation between Rh123+ PI- and immotile sperm. But the Rh123- PI+ sperm showed a contrary relationship with Rh123+ PI-. A significant inverse correlation was also observed between the Rhl23- /PI- sperm and sperm concentration in the abnormal group. CONCLUSION: Rh123/PI dual fluorescent staining and flow cytometry analysis can readily and quickly detect sperm mitochondrial function and be used to evaluate semen quality.

[Progress of researches on carnitines in the clinical therapy of andrology].

Carnitine, an important compound in the beta-oxidative process of mitochondrial fatty acid, plays a significant role in the cardiovascular, nervous, and reproductive system. Recently, carnitine has been used as a therapeutic in the treatment of male infertility, erectile dysfunction, Peyronie's disease, etc. Accordingly, the objective of this review is to summarize the progress in researches on carnitine as a clinical therapy in andrology.

[Integrated treatment for azoospermia caused by radiotherapy after surgical treatment of spermatocytoma: a case report].

OBJECTIVE: To investigate a therapeutic method for male infertility caused by radiotherapy after surgical treatment of spermatocytoma. METHODS: A case of azoospermia caused by radiotherapy after surgical treatment of spermatocytoma was reported and the Chinese medicine Jiaweishuiluerxiandan was used as a major therapy for 3 years. RESULTS: The patient's health condition was improved dramatically two years after being treated by the Chinese medicine but no sperm was found in his semen. However, three years after the treatment, his spermatozoon density was recovered from zero to 2.0 x 10(6)/ml with normal morphology. His sperm was subsequently used for intracytoplasmic sperm injection, which made his spouse pregnant successfully, and an healthy male infant was born by caesarean birth. CONCLUSION: Chinese medicine is a successful try at treating male infertility caused by radiotherapy after surgical treatment of spermatocytoma. For those who have failed to get their sperm frozen before surgery, Chinese medicine is a choice for remediation.

[Preparation and identification of anti-human epididymal sperm protein P34H monoclonal antibody].

OBJECTIVE: To prepare and identify anti-human epididymal sperm protein P34H monoclonal antibody (McAb). METHODS: The previously purified recombinant P34H protein was used as an antigen to immunize BALB/c mouse. Cell strains secreting anti-P34H McAb were established by hybridoma technique and then ascitic fluid-type McAb prepared. The sensitivity and specificity of McAb were detected by ELISA and Western blot, respectively. RESULTS: One strain (2C4) of IgG1 Kappa anti-P34H McAb was harvested. The titer detected by ELISA technique was 1:10(3) - 1:10(5). Western blot of healthy human sperm samples and purified recombinant P34H antigen probed with the prepared McAb were immunoreactive at 34,000 and 27,000, respectively. CONCLUSION: Anti-P34H McAb has been prepared successfully by the above methods, which may provide a powerful tool for exploring the relationship between P34H and male fertility.

[Androgen deficiency and erectile dysfunction].

Androgen not only indirectly affects penile erection by enhancing sexual desire and arousal, but also play an important role in maintaining the normal structure of the erectile tissue. Testosterone deficiency may result in a reduction of trabecular smooth muscle content concomitant with increased accumulation of extracellular matrix and deposition of adipocytes in the subtunical region. All this can lead to corporal veno-occlusive dysfunction. In addition, testosterone can affect the expression and activity of nitric oxide synthases (NOS) and RhoA/Rho kinase and influence penile erection directly. Androgen administration may restore erectile function for hypogonadal patients with erectile dysfunction who fail to respond to PDE5 inhibitors alone.

[Biological function of CoQ10 and its effect on the quality of spermatozoa].

Oxidative damage is one of the important factors inducing the regression of sperm quality. CoQ10 is a liposoluble antioxidant that exists in mitochondria and its levels in semen and spermatozoa have an important role in the oxidation resistance of the male genital system. Exogenous administration of CoQ10 can improve the sperm quality and reproductive ability of infertile patients as well as exert the effects of an adjunctive therapy on male infertility. This review describes the biological function of CoQ10 and its effect on the quality of spermatozoa and its effect as an adjunctive therapy on male infertility.
