[Strategy study of harvesting total facial flap and donor choice for allograft transplantation in cadaver].

OBJECTIVE: To explore the operational strategy of harvesting total facial allograft by autopsy. METHODS: Twelve fresh human cadavers were dissected. They were divided into two groups randomly. The total facial-scalp flap of the group I was elevated by the bi-pedicle method, the group II was operated with single-pedicle method. Both were dissected at the deep plane of the SMAS. : the time of facial flap harvesting, length of the artery vein and nerve pedicles of the donor were measured and marked, after operation, in each group we transferred one facial allograft to another. Then the free graft of group I was poured through artery by methylthioninium chloride to study vascular territories. RESULTS: Mean harvesting time of the group I (46 +/- 11) minutes, group II (111 +/- 7) minutes, P < 0.01. Perfusion result shows that unilateral superficial temporal artery and the opposite side of the facial artery can supply blood for whole face. The pedicle was long enough for anastomosis. Post-operation appearance: the face looks like neither the donor nor the recipient primarily, It's mainly due to the characteristics of the skeleton and the soft tissues. CONCLUSIONS: The bi-pedicle method of the harvesting total facial allograft is concise, fast, safe can be widely applied in clinical.

[Research of the preservation of the composite facial allograft].

OBJECTIVE: To investigate the effective method of preserving composite facial allograft so as to attenuate ischemic injury. METHODS: The composite facial allografts were harvested from dog, perfused and preserved with 4 degrees C physiological sodium chloride and UW solution respectively. Immediately after the removal of the flap, after 12, 24, 48 h of preservation, MTT assay was used to determine the viability of several kinds of tissue, including skin, mucosa, muscle, bleed vessel, nerve and gland. The results of the two groups were compared in term of viability percentage. The pathology of several tissues were observed after 24 and 48 h of storage. RESULTS: The viability percentage of every tissue conserved in UW solution for 48 hours was more than 75%. There was significant difference between physiological sodium chloride group and UW group (P < 0.05). Some changes, including Porous arrangement of fibers in connective tissue of skin and mucosa, hyalinization of tissue around the hair follicle and edema of cell in hair follicle, enlargement of space between muscle bundles and unclearness of boundary of acinus could be seen in physiological sodium chloride group while no significant change in UW group. CONCLUSIONS: UW solution could be considered as preservation solution for composite facial allograft.

[Electrochemical reduction of nitrobenzene under mild conditions].

Effect of cathode materials (S1, S2, AB5, graphite, Ni plate, Fe plate) on the electrochemical reduction of nitrobenzene of light concentration under mild conditions(current density less than 1.0 mA/cm2, pH in the range of 4 - 9), was experimentally studied. In aqueous solution, S1, S2 and AB5 electrodes showed high selectivity of nitrobenzene reduction to aniline, with the nitrobenzene conversion and aniline yield being over 85% and 75%, respectively. The results obtained by cyclic voltammograms and single sweep voltammograms showed that nitrobenzene was directly reduced at S1 and S2 electrodes, while reduced by hydrogen at AB5 electrode. The products electroreduced from nitrobenzene on S1, S2 and AB5 cathodes could be easily degraded by aniline degrading strain AN3 under aerobic condition.

[Prognostic analysis of lung function and chest X-ray changes of 258 patients with severe acute respiratory syndrome in rehabilitation after discharge].

OBJECTIVE: To analyze the clinical characteristics and prognostic changes of rehabilitating severe acute respiratory syndrome (SARS) patients through regular lung function tests and lung imaging studies after discharge and to retrospectively analyze the treatment data of these patients. METHODS: 258 discharged SARS patients received regular SARS-Co virus IgG test, lung function test and chest X-ray and/or high resolution computerized tomography (HRCT) examination at General Hospital of PLA two months after discharge, and the treatment data of these patients were retrospectively analyzed. RESULTS: 80.6% patients (208 of 258 patients) were positive for SARS-Co virus IgG. 21.3% patients (55 of 258 patients) showed lung diffusion abnormity (D(LCO) < 80%pred). Compared to 155 SARS-Co virus IgG positive patients without lung diffusion abnormity and 50 SARS-Co virus IgG negative patients, the 53 SARS-Co virus IgG positive patients with lung diffusion abnormity had longer fever course, higher dosages of glucocorticoid therapy, higher percentage of oxygen therapy and non-invasive ventilation. 51 of the 53 patients with lung diffusion abnormity received lung function test after one month, and the results of D(LCO) improved in 80.4% patients (41 of 51 patients). 40 of 51 patients with lung diffusion abnormity showed lung fibrosis, and the fibrosis decreased in 55% patients (22 of 40 patients) after one month. CONCLUSIONS: This finding suggests that lung fibrosis caused by SARS mostly occurs in severe patients, and it can resolve spontaneously. D(LCO) may be more sensitive than HRCT in evaluating the fibrotic changes.

[Alveolar damage of pneumocystis carinii pneumonia in renal transplantation recipients].

OBJECTIVE: To study the alveolar ultrastructural changes and the interaction between Pneumocystis carinii (PC) and alveoli in Pneumocystis carinii pneumonia (PCP) patients after renal transplantation. METHODS: Twenty-seven patients with suspected PCP after renal transplantation were examinated by bronchoscopy with bronchoalveolar lavage (BAL) and transbronchial lung biopsy (TBLB). The BAL fluid was centrifuged and the sediments were stained for PC. Cases for which electron microscope showed alveolar tissue in TBLB specimen were included. Then the clinical features, PC, alveolar epithelial damage, exudate in alveolar space, inflammatory cell infiltration, and fibrous tissue in the interstitial space were analyzed and evaluated. RESULTS: Twenty-three cases were studied. The mean time from renal transplantation to onset of illness was 5.6 months, and that from onset of illness to hospitalization was 5.5 days. Clinical features included fever, dyspnea, unproductive cough, and scanty chest signs, to hypoxemic respiratory failure. Chest CT showed diffuse lung interstitial changes in 22 of the 23 cases, 9 with consolidation. After treatment with SMZco, the fever resolved in 1 - 5 days, and the general state of the patients became better, and 19 patients were dischaged within 1 month. PC in BAL fluid was found by special staining in 18 patients, while PC was found by electron microscope in 14 patients. In most cases PC was few in the lung tissue, but in 3 cases abundant PC filled the alveolar space. PC was seen in two forms, the cyst and the trophozoite. Electron and light microscopes showed alveolar exudate, inflammation in interstitium and alveolar space, interstitial fibrosis, and alveolar epithelial damage in all patients. CONCLUSION: In PCP patients after renal transplantation there was marked alveolar damage, which was the major pathological change in the lung.

[Determination of impurities in K2Cr2O7 by an inductively coupled plasma spectrometer].

The trace elements Si, Al, Fe, Ca, Mn, Mg, and Cl in the K2Cr2O7 produced by chromate cleaner production were determined by a sequential inductively coupled plasma spectrometer in this paper. Matrix effect was studied and eliminated through matrix matching. The analytical conditions for the elements were optimized. Incident power of the RF generator was 0.95 kW. Pressure of carrier gas flow was 0.13 MPa, and its flow rate was 1.2 L x min(-1). Auxiliary gas flow rate was 0.2 L x min(-1). The element Cl was indirectly determined by the method of AgCl precipitation. The ion Cl- can be completely precipitated in 6% nitric acid system, and then determined after keeping the solution at 50 degrees C for two hours. The recoveries of the studied impurities range from 95% to 104%, and the relative standard deviations are lower than 4% (n = 10). Detection limits of these impurities are in a range 0.0001%-0.00001% (w/w). The analytical results of random samples agree well with the results using the standard-adding method, and the relative error varies from -4.27% to +5.26%. The developed method has been applied to the determination of impurities in K2Cr2O7 product with satisfactory results.

[Flame temperature distribution measurement of solid propellants].

Many high temperature bodies such as flame, in which chemical reactions are very complex, emit their own spectra. These emission spectra usually consist of the spectral lines, spectral bands and the continuous spectra. In some cases, the spectral lines gather together. It is very difficult to find the right single spectral line when the spectral line intensity method is used. To deal with this problem, the idea that the single spectral line intensity is replaced by the total intensity of many spectral lines to measure the temperature is mentioned. And the relative intensity method is also changed to deal with this idea. The measurement of the temperature distribution based on this improved method is successful, and the measurement results are compared with the results of the thermocouple method.

[Application of emergency hysterectomy on severe postpartum hemorrhage].

OBJECTIVE: To investigate the value of emergency hysterectomy on arresting severe postpartum hemorrhage. METHODS: A retrospective analysis was performed on 10 cases of hysterectomy by reason of postpartum hemorrhage between Jan 1990 and Dec 2000. RESULTS: All the patients were severe postpartum hemorrhage that couldn't be controlled by conservative treatment. Among them, there were 5 cases of DIC, 2 cases of placental adherence, 1 case each of uterine atony, uterine rupture and infection of uterine incision. Blood loss averaged 2,427 ml. All the 10 patients were recovered. Emergency hysterectomy was related to times of gestation and delivery and also to delivery pattern. CONCLUSION: Emergency hysterectomy is one of the effective methods on arresting severe postpartum hemorrhage.

Patterns of failure after primary local therapy for prostate cancer and rationale for secondary therapy.

The timing and type of treatment for patients with biochemical disease recurrence after local therapy for prostate cancer remains controversial. This is because of many unresolved issues surrounding the natural history of disease progression in such patients, including the limited ability of clinical measures to accurately define local versus distant disease recurrence. Clinicians generally rely on clinical tumor characteristics, such as tumor stage, grade, and prostate specific antigen (PSA) kinetics after local therapy, to distinguish local from distant recurrence. This determination is important, because patients with local recurrence may be candidates for a second, potentially curative treatment, whereas those with distant recurrence are generally treated with androgen deprivation therapy (ADT). Data from a national disease registry of patients with prostate cancer, the Cancer of the Prostate Strategic Urologic Research Endeavor (CaPSURE), suggest that the use of secondary cancer treatment after local therapy for prostate cancer is common. For patients initially treated with radical prostatectomy, secondary treatment appears to be nearly equally divided between postoperative radiation and ADT, whereas >90% of patients receiving a secondary treatment after radiation are treated with ADT. Serum PSA at diagnosis, Gleason score, and type of initial treatment appear to be predictors of secondary treatment use in this setting. Patient age, lymph node status, and margin status appear to be predictors of secondary treatment with ADT or radiation for patients initially treated with radical prostatectomy.
