Detectable C-Peptide and Diabetic Ketoacidosis Risk in Type 1 Diabetes.

OBJECTIVE: To investigate whether detectable C-peptide levels in type 1 diabetes is associated with a lower risk of diabetic ketoacidosis (DKA). RESEARCH DESIGN AND METHODS: We analyzed the Diabetes Control and Complications Trial publicly available data repository for the association between detectable stimulated C-peptide (>0.2 nmol/mL, measured annually) and DKA incidence over an average 6.5-year follow-up. We used crude and adjusted Andersen-Gill models for recurrent DKA events with time-dependent covariates. RESULTS: Of the 1,441 participants (53% male, median age 27 years), 129 (9%) experienced 180 DKA events. Of these events, 179 (99.44%) occurred after C-peptide was ≤0.2 nmol/L in the prior year and only 1 event occurred with C-peptide >0.2 nmol/L. C-peptide >0.2 nmol/L was associated with a DKA hazard ratio of 0.07 (95% CI 0.01-0.48; P = 0.007), consistent across adjusted models. CONCLUSIONS: Endogenous insulin production was significantly associated with lower DKA risk, suggesting that treatments preserving insulin production could decrease long-term DKA risk.

Virtual sources for a cosh-Gaussian beam.

On the basis of superposition of beams, a group of virtual sources that generate a cosh-Gaussian wave is identified. A closed-form expression is derived for this cosh-Gaussian wave, which, in the appropriate limit, yields the paraxial approximation for the cosh-Gaussian beam. From this expression, the paraxial approximation and the nonparaxial corrections of all orders for the corresponding paraxial cosh-Gaussian beam are determined.

Propagation of the Wigner distribution function for partially coherent nonparaxial beams.

On the basis of the Rayleigh-Sommerfeld diffraction integral, a closed-form propagation expression for the Wigner distribution function of partially coherent nonparaxial beams in free space is derived for what is to our knowledge the first time. The propagation of spatially fully coherent nonparaxial beams is treated as a special case of our general result. Application of the result is illustrated with the nonparaxial propagation of partially coherent anisotropic Gaussian-Schell-model beams and TEM11-mode Hermite-Gaussian beams.

[A comparative study on safety and immunogenicity of an inactivated hepatitis A vaccine in HBsAg carriers and healthy children].

OBJECTIVE: To evaluate safety and immunogenicity of inactivated hepatitis A vaccine in HBsAg carriers and healthy children. METHODS: One hundred and twenty-one healthy children and ten HBsAg carriers, aged 1-10 years HAV susceptible were enrolled in the study. The inactivated hepatitis A vaccine was produced by Tangshan Biogenetic Company. The dosage of the vaccine was 1000 U/Dosage and 500 U/Dosage. The vaccination schedule was six month apart for two injections. The serum anti-HAV level was detected with EIA at one month after first injection and at one and six month after the booster injection, respectively. RESULTS: The anti-HAV appeared in all the children. One month after the booster injection, the serum anti-HAV level in children vaccinated 500 U/Dosage was 4684.9 mIU and 4535.6 mIU, respectively and in the children vaccinated 1000 U/Dosage, 5399.8 mIU and 7347.1 mIU, respectively. The anti-HAV level was not statistically different between the two groups of children. There was no adverse reaction after the vaccination. The anti-HAV level was still high one year after first injection. CONCLUSIONS: The data indicated that the safety and immunogenicity of the domestic inactivated hepatitis A vaccine were excellent in both groups of children.
