Vasodilation increases the threshold for bupivacaine-induced convulsions in rats.

UNLABELLED: Bupivacaine affects the vascular resistance by peripheral and central nervous system (CNS) mechanisms. As vasoconstrictors increase the CNS toxicity of IV bupivacaine, vasodilators may decrease its CNS toxicity. We examined the hypothesis that vasodilators decrease the CNS toxicity of bupivacaine in awake, spontaneously breathing rats. Male Sprague-Dawley rats were randomly divided into control (C), nicardipine (N), and phentolamine (P) groups (n = 12 in each group). Racemic bupivacaine was administered IV at 1 mg/kg/min until tonic/clonic convulsions occurred. Saline, nicardipine (0.4 microg/min), and phentolamine (10 microg/min within 5 min, 50 microg/min thereafter) were simultaneously administered with bupivacaine in groups C, N, and P, respectively. Mean arterial blood pressure was significantly increased by infusion of bupivacaine in group C and was maintained at baseline levels before the onset of convulsions in groups N and P. The convulsive dose of bupivacaine in group C was 5.8 +/- 1.5 mg/kg, but was significantly larger in groups N and P (7.6 +/- 1.5 and 8.1 +/- 1.1 mg/kg, P = 0.02 and 0.001, respectively). However, there were no differences in total or protein-unbound plasma concentration of bupivacaine or in concentration of bupivacaine in the brain at the onset of convulsions among the 3 groups. We conclude that nicardipine and phentolamine increase the cumulative dose but do not affect the threshold plasma or brain concentrations required for bupivacaine-induced convulsions. IMPLICATIONS: Bupivacaine, a long-acting local anesthetic, induces central nervous system toxicity when its plasma concentration is increased. Nicardipine and phentolamine increased the cumulative dose but did not affect the threshold plasma concentrations, required for bupivacaine-induced convulsions, suggesting that both nicardipine and phentolamine inhibited the increase in the plasma concentration of bupivacaine by inducing peripheral vasodilation.

Fatigue strengths of particulate filler composites reinforced with fibers.

The aim of this study was to evaluate the dynamic fatigue strengths at 10(5) cycles and the strains of particulate filler composite resins with and without reinforcing fibers. An UHMWPE (Ribbond), a polyaromatic polyamide fiber (Fibreflex), and three glass fibers (GlasSpan, FibreKor, Vectris Frame) were used to reinforce the particulate filler composite resins. The fatigue properties were measured in three-point bending mode using a servohydraulic universal testing machine at a frequency of 5 Hz, until failure occurred or 10(5) cycles had been completed. The fatigue strengths at 10(5) cycles were determined by the staircase method. The fractured aspects of specimens were evaluated by an optical and scanning electron microscope. The fatigue strengths of particulate filler composite resins were 49-57 MPa, and those of fiber-reinforced were 90-209 MPa. Unidirectional glass fibers showed higher reinforcing effects on the fatigue strengths of composite resins. The strain of UHMWPE-reinforced composite was largest.

Corrosion behavior and surface characterization of Ti-20Cr alloy in a solution containing fluoride.

The purpose of this study was to investigate the correlation between corrosion resistance and surface composition of an experimental Ti-20 mass% Cr casting alloy in a saline solution containing fluoride. The alloy had a greater resistance to corrosion in a fluoride-containing saline solution than did commercially pure titanium. However, with confirmed dissolution of titanium and chromium, it meant that the fluoride in the saline solution corroded the alloy slightly. X-ray photoelectron spectroscopy analysis revealed that the surface composition of the alloy consisted of titanium and chromium oxides containing hydroxide. The [Ti]/([Ti] + [Cr]) ratio in the surface oxide film decreased when immersed in fluoride-containing saline solution, that is, the surface oxide film became chromium-rich oxide. Therefore, the alloy obtained good corrosion resistance to fluoride due to formation of a chromium-rich oxide film.

Crystallization and preliminary X-ray diffraction analysis of O-acetylserine sulfhydrylase from Aeropyrum pernix K1.

Crystals of O-acetylserine sulfhydrylase from Aeropyrum pernix K1 were obtained by the hanging-drop vapour-diffusion method at 298 K. An X-ray diffraction data set was collected to 2.25 A resolution at 100 K. The crystal belonged to space group P42(1)2, P4(1)2(1)2, P4(2)2(1)2 or P4(3)2(1)2. The unit-cell parameters were a = b = 74.5, c = 276.0 A. The presence of two subunits of the enzyme per asymmetric unit gives a crystal Volume per protein mass (V(M)) of 2.28 A(3) Da(-1) and a solvent content of 46%(v/v).

The P-glycoprotein inhibitor quinidine decreases the threshold for bupivacaine-induced, but not lidocaine-induced, convulsions in rats.

PURPOSE: To examine whether inhibition of P-glycoprotein (P-gp) activity by quinidine affects the central nervous system toxicity of lidocaine and racemic bupivacaine (bupivacaine). METHODS: Forty male Sprague-Dawley rats were randomly divided into four groups (n = 10). Fifteen minutes following administration of 15 mg x kg(-1) of quinidine (QL and QB groups) or saline (L and B groups), lidocaine (L and QL groups, 4 mg x kg(-1) x min(-1)) or bupivacaine (B and QB groups, 1 mg x kg(-1) x min(-1)) was infused until convulsions occurred. Concentrations of lidocaine and its primary metabolite, monoethylglycinexylidide (MEGX) and bupivacaine in plasma and in the brain at the onset of convulsions were measured by high-performance liquid chromatography. RESULTS: There were no differences in the dose of lidocaine required to induce convulsions between the L and QL groups. There were no differences in the concentrations of total (L = 17.2 +/- 3.5, QL = 16.6 +/- 2.6 micro g x mL(-1)) or unbound lidocaine (L = 7.8 +/- 2.5, QL = 7.3 +/- 2.3 micro g x mL(-1)), total (L = 1.2 +/- 0.5, QL = 1.3 +/- 0.7 micro g x mL(-1)) or unbound MEGX (L = 0.9 +/- 0.5, QL = 0.8 +/- 0.4 micro g x mL(-1)) in plasma, total lidocaine or MEGX in the brain at the onset of convulsions between the L and QL groups. The dose of bupivacaine required to induce convulsions was comparable in the B and QB groups. At the onset of convulsions, plasma concentrations of both total (B = 4.9 +/- 1.1, QB = 4.0 +/- 0.6 micro g x mL(-1), P = 0.03) and unbound bupivacaine (B = 1.4 +/- 0.6, QB = 0.9 +/- 0.2 micro g x mL(-1), P = 0.02) were significantly lower in the QB group than in the B group. There were no differences in concentration of total bupivacaine in the brain between the B and QB groups. CONCLUSION: These results suggest that quinidine inhibited P-gp activity, resulting in increased brain/plasma concentration ratio of bupivacaine, but not of lidocaine, and decreased the threshold of plasma concentration for bupivacaine-induced convulsions.

Adhesion of 4-META/MMA-TBB resin to heated dentin: effects of pre-treatments with FeCl3 and/or HEMA.

The purpose of this study was to compare the tensile bond strengths (TBSs) and failure mode of 4-META/MMA-TBB resin to 60 degrees C-heated and unheated bovine dentin, especially to investigate the influence of ferric chloride contained in citric acid pre-conditioning. In addition, the effect of HEMA priming for heated dentin was also evaluated. The TBSs to heated dentin were significantly lower than those to unheated dentin. Adhesive failures were observed in most specimens of the heated and HEMA-non primed group. HEMA application to heated dentin significantly increased the TBSs in each acid conditioning, which were also significantly higher than those of the unheated and ferric chloride-contained citric acid-conditioned group. It was clarified that heating dentin decreased the bond strength without HEMA priming even if the dentin surfaces were acid conditioned with 10-3, while HEMA priming after acid conditioning recovered the bond strength.

The influence of albumin on corrosion resistance of titanium in fluoride solution.

Proteins can interact with corrosion reactions in several ways. In this study, we investigated the effect of albumin on the corrosion resistance of titanium in the presence of fluoride. The effects of the NaF concentration, albumin concentration, and pH on the corrosion characteristics of commercially pure titanium (CP-Ti) were examined by means of electrochemical techniques. The corrosion resistance of titanium decreased as the NaF concentration increased and as pH decreased. The corrosion resistance of titanium in NaF solutions was improved in the presence of albumin. The natural electrode potential was elevated, and the passive current density was reduced by albumin at a concentration of 0.01%. The polarization resistance rose with increased concentrations of albumin in fluoride solution. These results showed that the albumin in saliva and dental plaque affected the corrosion resistance of CP-Ti in fluoride solution.

Effect of tubule orientation and dentin location on the microtensile strength of bovine root dentin.

To investigate the mechanical properties of root dentin and to further clarify the cause of vertical root fracture (VRF), this study evaluated the effect of tubule orientation (parallel, perpendicular and oblique to the cross-section of dumbbell specimens in microtensile tests) and dentin location (cervical, middle, and apical location of the root) on the microtensile strength of bovine root dentin. Each specimen was stressed in tension at a crosshead speed of 1.0 mm/min. The results of the microtensile strength measurements were statistically analyzed with one-way ANOVA and the Fisher PLSD. The oblique group (95.18+/-23.80 MPa) was significantly (p<0.01) higher than the parallel group (38.93+/-5.28 MPa) or the perpendicular group (32.64+/-4.69 MPa). There were no significant differences among the different dentin locations within the parallel group (p>0.05). It was clarified that the VRF occurs frequently in practical situations due to the tubule orientation of root dentin.
