[A bone cement containing methotrexate in an experiment and in clinical practice].

The authors submit their initial experience with the use of bone cement with methotrexate in the treatment of primary and secondary malignities of bones. As a basis they use the analogy of using bone cement with gentamycin the molecular structure of which is similar to that of methtrexate. In the experimental part of the work in vitro evidence was provided of methotrexate stability when exposed to heat or oxidation; and the kinetics of its release from pores of bone cement were revealed. In experimental dogs the effect of cement with methotrexate on neighbouring bone was investigated in the early period in the immediate vicinity necrosis of the trabeculae was found, during the late period healing processes. The clinical use of bone cement with methotrexate was started in 1986. In 17 surviving patients only in one case local progression of a tumour in the vicinity of cement with methotrexate was found. The aim of the work is to find a possible local treatment of bone malignities, in particular metastatic ones, which will be not only an emergency solution but will extend our therapeutic possibilities.

The submicroscopic structure of a chondroblastic osteogenic sarcoma.

Two patients are reported with a chondroid form of osteosarcoma, whose structure was also analyzed submicroscopically. The clinical and radiologic picture and the rapid lethal progress of the disease indicated an osteosarcoma, while both histologic and electron-microscopic investigation suggested the prevalence of malignant chondroid. Progress in the treatment of osteosarcoma makes it essential for diagnostic, therapeutic and prognostic reasons to reintroduce subclassification of these tumors in an attempt to correlate their morphological picture with their biologic properties. According to our experience to date, it seems that the cartilaginous component in no way ameliorates the rapid malignant penetration of the osteosarcoma. It is essential to distinguish these tumors from the usually much less malignant chondrosarcomas and to place the chondroblastic osteogenic sarcoma among in spite of the clear predominance of the malignant chondroid, and thus also introduce highly radical and combined therapy.

Cementoma-like tumours of bone.

The authors summarize the morphological findings on cementoma-like fibromas of long bones registered at the Reference Centre for Bone Tumours in Brno or referred to it. One of the cases registered is described in detail from the submicroscopic point of view. Their experience has shown that cementoma-like fibromas of long bones are genuine bone tumours sui generis, differing to some extent from cementomas of the jaws. The cementicles in such tumours are identical to acellular dental cement and structurally quite different from osteoid.
