Mutans streptococci and the development of dental plaque.

A study of dental plaque was first documented in the literature of the 17th century. It has been described as a biofilm composed of bacterial populations living in a plaque matrix. The plaque development starts short time after a tooth surface is cleaned and covered by salivary proteins and glycoproteins conditioning film. Mutans streptococci was described as the most important bacteria related to the etiology of dental caries. This bacteria has the basic properties of cariogenicity required in the dental caries process. Current studies have suggested that the behaviour of the oral microflora, with all its components, should be the aim of future understanding of the process of dental caries. The aim of this paper is to review literature about initiation and development of dental plaque and the influence of mutans streptococci in the dental caries.

[Early childhood caries].

Early childhood caries is a severe form of dental decay that affects infants and toddlers. It is a chronic, multi-factorial, infectious disease. It should be considered as a symptom of paediatric disorders and lack of adequate care. The caries is particularly virulent, it develops rapidly, often soon after teeth erupt and it begins on tooth surfaces which are usually only affected to minor extent. The condition is particularly frequent in various minority (racial, cultural or ethnic) communities, single parent or low-income families etc. The manifestations include pain, infection, abscesses and chewing difficulties that affect a child's ability to eat and sleep. It thus has debilitating effect on the development, speech general health and self-esteem of infants. The problem should be addressed rather from the paediatric than dental point of view and the appearance of a single carious lesion an any tooth surface during the first 3 years of life must be considered a serious health problem. Early childhood caries is a preventable disease, that can be addressed by targeted preventive measures and professional health invention (provided by dentists as well as paediatricians), and by targeted community preventive programs.

Oral healthcare in transition in Eastern Europe.

Big changes have occurred in the oral healthcare delivery systems of most Eastern European countries since the fall of the Berlin wall in 1989 and the demise of communism in the former USSR in 1991. In the new situation it was necessary to reform the political and social systems including healthcare. Reforms were started to improve the economy and, in comparison with Western Europe, the generally lower living standards. It is difficult to obtain comprehensive data on oral healthcare in Eastern European countries but this paper reports data from nine countries and provides a 'macro' view of the current situation in these countries. Many countries seem to have adopted a Bismarckian model for the provision of oral healthcare based on a sickness insurance system.

[The clinical significance of bacteremia following tooth extraction].

A group of 61 patients underwent conservative and surgical dental treatment in general anesthesia and their blood was sampled for bacteremia before and after treatment. The antibiotic antibacterial prophylaxis was administered in 21 patients one hr. before treatment (both in conservatively and surgically treated patients). Immediately after the treatment the prevalence of bacteremia amounted to 80% in a group of ATB-free patients. In patients with ATB prophylaxis the prevalence of bacteremia after the treatment amounted to 17%.

Stimulation of human blood lymphocyte by different polyclonal B cell activators of bacterial and plant origin: production of IgM, IgG and IgA estimated by the ELISA method.

Lymphocytes isolated from peripheral blood of healthy donors were stimulated in vitro with pokeweed mitogen, concanavalin A, flagellin, Nocardia delipidated cell mitogen (NDCM) and heat-killed bacteria Escherichia coli and Actinomyces viscosus. A simple and sensitive technique, enzyme-linked immunosorbent assay (ELISA) was used for the detection of nanogram levels of IgM, IgA and IgC in media from lymphocyte cultures after polyclonal stimulation, Pokeweed mitogen, NDCM and E. coli were shown to stimulate a high production of IgM; after stimulation with A. viscosus a higher production of IgA was detected. No immunoglobulin production was observed after stimulation with polymerized flagellin.
