[Poliomyelitis and the post-polio syndrome].

In developed countries as well as in Hungary polio virus related disease disappeared completely due to the extensive administering of vaccine. As a result, young and middle-aged doctors have no experience of encountering acute polio virus infection but instead they meet its resultant impairments and disabilities. Persons who had suffered the onset of poliomyelitis 3 or 4 decades earlier, may to develop a new set of symptoms and functional declines. The criteria for post-polio syndrome were identified and its impairments and disabilities were described. It is only within the last decade that the adaptive changes in muscular tissue, insufficiently compensed denervation, together with the effects of physiotherapy, have become better known. The aim of this paper is to review the present situation concerning the struggle for keeping the polio virus in check, to give a short summary of the post-polio syndrome and to draw attention to the importance of rehabilitation.

[Rehabilitation of anoxic brain injured patients after reanimation].

Authors report rehabilitation outcomes of anoxic brain injured patients, following reanimation. In the past 12 years (1988-1999) they treated 54 patients. The patients were selected from other hospitals after successful reanimation, but sustained diffuse anoxic brain injury. The CT and MR examination was generally negative, but small hypodensities were shown rarely (18/54). At the time of admission 21 patients were quadriparetic, 5 patients were hemiparetic, 2 were paraparetic and there were no any paresis in 26 cases. The mean age of the patients was 32 years (7-61). At the time of admission 31 patients were dependent in basic activities of daily living, the mean Barthel Index score was 15 (0-100) but at the time of discharge it was 45 (0-100). 41 patients were discharged into their home and 13 patients had to be transmitted to another hospital for further treating, but later 5 of them died. At the time of discharge only 7 patients had score 100 of the Barthel Index, but they were not able for independent living. Family support, mainly guidance was necessary. Memory deficits, apraxia, attention disturbance, ataxia were the main problems during the rehabilitation process.

[The relationship between the length of orthodontic treatment and patient compliance].

Based on the results of the psychological tests and their clinical experience, the authors conclude as follows: 1. At the beginning of the orthodontic treatment the fear of the unknown is dominating, but later on the aesthetic desires gain more importance than the first reaction. 2. The compliance and the length of the orthodontic treatment show an indirect proportional relationship. The compliance is stronger in case of a shorter and more successful orthodontic treatment.

[The development and action-mechanism of the quadhelix in orthodontic treatment].

Authors present a historical review of the development of quadhelix appliance. In conclusion, slow maxillary expansion with the quadhelix appliance provides an alternative approach to rapid maxillary expansion that is particularly beneficial for correction of transverse and arch-length discrepancies during the mixed dentition. There are numerous indications for this approach with two major advantages. First, a lower force that is within the elastic limits of maxillary sutural and periodontal tissue is generated. Second, a low separation rate of 0.4-1.1 mm per week allows for more physiologic adaptation to the expansion that may result in a more stable correction with less sutural relapse. The quadhelix is a versatile appliance that provides practitioners a convenient means to normalize developing malocclusions.

[Relationship between dental status and salivary microbiology in adolescents].

The authors studied caries prevalence in connection with salivary caries related findings, in 349, 14-16 years old Hungarian adolescents, living in two different cities. DMF.T, DMF-S means, stimulated salivary flow, buffer capacity, Streptococcus mutans, Lactobacillus and Candida albicans counts in saliva were determined. The ratio of cariesfrre adolescents was 4.6% in the total population sample, DMF-T mean values were 7.24 +/- 4.86, DMF-S means 10.50 +/- 8.35. Mean secretion rate of stimulated saliva was 0.84 +/- 0.50, a low buffer capacity was found in 6.3% of the examined children. Ratio of carriers of Streptococcus mutans, Lactobacilli and yeasts in saliva was 89.7%, 73.9% and 47.7% respectively. DMF-T and DMF-S values, as well as S. mutans counts were lower in the capital than in the other city. Statistically significant correlations were found between DMF-S, DMF mean values and salivary microbiological counts.

Rehabilitation of patients after hip disarticulation.

We review rehabilitation of patients after hip disarticulation operated on over the past 5 years. Sixty-two patients underwent 63 hip disarticulations: 24 had malignancies, 23 arteriosclerosis, 11 Buerger's disease, 3 diabetes and 1 uncontrollable infections. The mean age of tumour patients was 39 years (range 6-69 years), that of the vascular patients was 55 years (range 33-78 years). The postoperative mortality rate of the vascular patients was 16/37, and 0/24 of those with malignancies. There was one bilaterally operated patient with bilateral purulent coxitis from decubitus ulcers, who died on the 32nd post-operative day. Complicated wound healing was observed in 5 of the 24 tumour patients, and in 24 of the 37 vascular patients. The strategy of prostheses fitting has a two-stage concept: temporary prostheses in the 1st or 2nd month, permanent prostheses in the 6th month. All 24 tumour patients were fitted and could walk, while only 2 of 37 vascular patients were able to walk with prostheses. In our experience the outcome is significantly dependent upon the primary illness: in vascular cases it is poor, while in malignancies it is fairly good.

[Psychosomatic correlations in orthodontics--the impact of dentition anomalies on the mental status].

Authors present the essence of psychosomatics with particular emphasis on the impact of dento-occlusal and cranio-facial anomalies on the mental constitution. In their representation--after giving a historical review--they show the main features of the present state of knowledge in this branch of science based on literary publications by completing it with their own clinical experience.

[Clinical examination of the effect of the "Corega Junior" cleaning tablets for removable orthodontic appliances].

According to the authors' macroscopic and microbiological examinations Corega Junior prevents plaque formation on the orthodontic appliances and the establishment and multiplication of microorganisms and therefore has a favourable effect to the oral flora and last but not least to the general state of health of the patients. The usage of the Corega Junior cleaning tablets incites young people for the regular use of orthodontic appliances and indirectly for mouth care.

[The immobilization syndrome].

Prolonged inactivity, bed rest causes pathological changes in most organs and systems of the body generally known as immobilization syndrome. These changes became known in the past 50 years in addition to scientific experiences. The author reviews pathological changes that occur with inactivity, discusses the predisposition of the elderly to the complication of bed rest and makes recommendations for treatment and prevention. While the effects of immobilization are mostly reversible in young subjects, older persons have considerably more difficulty recovering, it can even lead to the loss of independence. Minimizing duration of bed rest, early ambulation, good nursing care and physiotherapy can prevent many of the complications of inactivity and bed rest.

[Effect of various mouthwashes on wound healing after tooth extraction].

The results of the clinical tests involving 200 patients show that after extraction the use of Meridol, NaF and Chlorhexidine mouthrinses along with careful mouth hygiene helps healing to a great extent. The antiseptic affect of solutions in case of solutions of fluoride content are manifested in an antibacterial way, while solutions of chlorhexidine content act in an antiseptic way. Furthermore the solutions foster good oral hygiene and in this way lessen the number of complications. This is shown by the decreased frequency rate of gingivitis and also the opinions of the patients. Their reports confirm that there is less foetor ox ore, pain and functional insufficiency in mastication. Based on these facts we can state that any of the three mouthrinses considerably helps complication-free healing.

[Indications for preventive removal of impacted third molars].

The impacted third molars are clinically important due to the pathological alterations caused by them. It is important from orthodontic viewpoint, that during their development the anomalies combined with the constriction, or unsuitable length of the dental arch may lead to recrudescence. For this reason the preventive removal of impacted third molars is suitable at the proper stade of their development.

Loneliness in old age.

The author describes, in connection with 100 cases, a pathological syndrome of isolation, the absolute loneliness. This pathological condition is deemed to be dangerous for old people. Most typical is the superposition or total lack of emotional responses. Loneliness is a disease leading ultimately to death. 48 old subjects aged 65 to 82 were examined for immuno-globulin status (IgG, IgA, IgM), they were divided into groups of lonely and partly lonely subjects. In the case of lonely old subjects a more vigorous decline of immuno-globulin level was found than with non-lonely ol people - a fact pointing to negative psychic nervous influences have an adverse influence on immuno-globulin production. Over a period of 5 years, the group of lonely old subjects exhibited a higher mortality rate than the group of non-lonely people. The author puts forward a proposal of therapy to mitigate the absolute loneliness: social and individual care and therapy by physicians of gerontological outlook (psycho-pharmacons, procain, products, multivitamins etc.).
