The ursodeoxycholic acid-p-aminobenzoic acid deconjugation test, a new tool for the diagnosis of bacterial overgrowth syndrome.

OBJECTIVE: To determine the possible complementary role of the ursodeoxycholic acid-p-aminobenzoic acid (UDCA-PABA) loading test in the diagnosis of intestinal bacterial overgrowth. DESIGN: A prospective clinical study. PATIENTS AND METHODS: The hydrogen breath and UDCA-PABA tests were performed simultaneously in 68 patients with suspected contaminated small bowel syndrome (CSBS), and in 10 healthy control subjects. The hydrogen breath test was performed by oral loading of 25 g of lactose and/or 10 g of lactulose. The UDCA-PABA test was carried out by oral loading of 250 mg of UDCA-PABA conjugate, followed by measurement of the amount of PABA excreted in the urine. The diagnosis of bacterial overgrowth was considered to be established when either the hydrogen breath test or the UDCA-PABA test produced abnormal results. RESULTS: Thirty-five of the 68 patients proved to have CSBS. In 13 of these 35 patients, only the enhanced urinary PABA excretion (11.7 +/- 1.42 mg vs. 3.6 +/- 0.68 mg) indicated bacterial overgrowth, 15 of the 35 patients gave only a positive hydrogen breath test, and in the remaining seven cases the results of both tests were abnormal. In eight CSBS patients, the urinary excretion of PABA was decreased significantly following 10-day tinidazole treatment (5.5 +/- 1.29 mg vs. 13.1 +/- 2.07 mg). CONCLUSION: The UDCA-PABA test is a valuable clinical method for the detection of bacterial overgrowth, especially in cases where hydrogen production alone fails to reveal CSBS. It is also a useful procedure for evaluating the efficacy of antibacterial treatment.

[Laparoscopic cholecystectomy: analysis of 1122 operations].

The results of 1122 laparoscopic cholecystectomies have been analyzed which were performed within a two year period of time. In 1097 (97.8%) cases the authors managed to carry out the laparoscopic procedure, however in 25 patients (2.2%) the operation had to be converted to an open surgery. There was no hospital mortality, however 1 patient died at home due to pulmonary embolism. (Mortality: 0.09%) According to their survey 758 patients exhibited concurrent medical risk factors and 413 previous abdominal operations increased the "surgical" risk of laparoscopic procedures. 27 early and 46 late postoperative complications have been observed. In 3 instances (0.26%) bile duct injury occurred and in 5 cases (0.44%) reoperations had to be carried out secondary to postoperative complications. Forty patients (3.6%) have had recognized common bile duct stones, these were removed, in part preoperatively (28pts), in part postoperatively (11pts) by endoscopic sphincterotomy (EST). In one case a laparoscopic retrieval of common bile duct stone was accomplished via the dilated cystic duct. 47% of their patients left the hospital on the first day and 78% of them were discharged within 2 days.

Dermatitis herpetiformis: relation between circulating immune complexes, small-intestinal mucosal status, and immunohistopathological findings.

Some aspects of humoral immunity and the status of the jejunal mucosa were investigated in 22 patients with classical dermatitis herpetiformis (DH). The mucosal status was characterized on the basis of immuno- and histopathological findings and functional analysis of malabsorption and disaccharidase activity. There was no significant abnormality in the serum immunoglobulins, whereas the IgA and C3 contents of the circulating immune complexes (established by polyethylene glycol precipitation and inverse radial immunodiffusion techniques) were significantly elevated. Polyorgan-specific autoantibodies of IgG type were present in the sera of 95% of the 22 patients. Functional analysis of the gastrointestinal tract revealed some abnormality in all of the 18 cases examined, while direct immunofluorescence studies demonstrated an increased number of subepithelial IgA lymphoid cells in all of the 15 cases examined. These findings did not correlate well with the jejunal mucosal morphology. This study supports the view that IgA deposited in the skin is formed in the gut.

Effects of prostaglandin E2 and F2 alpha on the absorption and portal transport of sugar and on the local intestinal circulation.

With the isolated jejunum loop technique investigations of prostaglandin E2 and F2 alpha were made on canine intestinal absorption and transport of glucose and on the circulation of the intestinal loop. These compounds decreased glucose absorption; intra-arterial prostaglandin administration decreased the portal transport of glucose, but intraluminal administration caused an increase. PGE2 enhanced the circulation of the intestinal loop; intra-arterial PGF2 alpha diminished this circulation, whereas on intraluminal PGF2 alpha had no significant effect.

Primary Sjögren's syndrome with secondary hyperparathyroidism.

The case history of a 43-year-old woman with primary Sjögren's syndrome is presented: in 1970, xerostomia and keratoconjunctivitis sicca; in 1980, arthritis; in 1982, chronic tubulointerstitial nephritis with renal tubular acidosis and vasopressin-resistant hyposthenuria. The renal function gradually deteriorated. Chronic atrophic gastritis with vitamin B12 deficiency and chronic duodenitis with diminished disaccharidase activity in the mucosa were also diagnosed. From 1985, repeated multiple fractures of the ribs occurred, with secondary hyperparathyroidism in the background. The renal and intestinal involvement suggests that, besides the elevated parathyroid hormone level, an acquired vitamin D deficiency plays a pathogenetic role in severe osteopenia. The patient is being treated at present by haemodialysis, and subtotal parathyroidectomy and renal transplantation are planned.
