The possible preterm birth preventive effect of ampicillin during pregnancy.

OBJECTIVE: To study the possible association between oral ampicillin treatment during pregnancy and pregnancy complications, in addition to birth outcomes, particularly preterm birth. METHODS: We evaluated newborn infants without congenital abnormalities born to mothers with or without ampicillin treatment during pregnancy in the population-based large data set of the Hungarian Case-Control Surveillance of Congenital Abnormalities. RESULTS: Of 38,151 newborn infants, 2,630 (6.9%) had mothers with ampicillin treatment during pregnancy. Some pregnancy complications, particularly preeclampsia, showed a higher prevalence in pregnant women with ampicillin treatment. Mean gestational age was slightly longer (0.2 week) but it resulted in a significant reduction in the prevalence of preterm birth (7.1 vs 9.3%; adjusted POR with 95% CI: 0.8, 0.7-0.9). The preterm birth preventive effect of ampicillin was most obvious after the use of the drug in the first trimester of pregnancy. Similar difference was not seen in the reduction of low birth weight. The rate of preterm birth was lower in newborn infants born to mothers with acute infectious diseases of both the genitourinary and respiratory systems if they were treated by ampicillin. CONCLUSIONS: Ampicillin treatment, particularly in the first trimester of gestation, can reduce the proportion of preterm birth in pregnant women with acute infectious diseases of the genitourinary and respiratory system.

Data to the clinical manifestation of the Krukenberg tumour.

Ovarian cancer is one of the most frequent malignant tumours in the female population. The screening of this tumour type is unsolved. The tumours are usually diagnosed in the advanced stage; thus the results of survival are unfavourable. Their histopathological appearance has a wide variety, with the occurrence of numerous metastatic forms. In these metastatic cases the choice of treatment is more difficult and prognosis is also worse. Among the metastatic tumours, the primary tumours of the gastrointestinal tract occur the most frequently. They are known as the Krukenberg tumour. Authors present two cases of Krukenberg tumours in order to summarize our knowledge on this rare tumour type and to give some practical advice.

Quantitative comparison of serum steroid and peptide hormone concentrations in male and female fetuses in the maternal-fetoplacental system during the 28th-40th weeks of pregnancy.

There are a great deal of data concerning the role of testosterone in the formation of the sex of the fetus. The synthesis of the testosterone is influenced by the hypophysis and the fetoplacental unit. In this study the authors conferred the progesterone, DHAS, cortisol, oestradiol, oestriol, prolactin and HPL levels of male and female newborns in the maternal vein and in the umbilical vein and artery in the 28th-32th weeks (31 parturients, 15 male and 16 female fetuses), in the 33rd-36th weeks (43 parturients, 20 male and 23 female fetuses) and in the 40th week (34 parturients, 11 male and 23 female fetuses) of pregnancy. The determination of serum hormone concentration was carried out with the RIA method in 2084 samples. There was no significant difference between the serum hormone concentrations of the male and female sex in the 28th-40th weeks of pregnancy. They assume that in the 28th-40th weeks there is no difference in the activity of steroid and peptide hormone secretion caused by the sex of the fetus.

Peridural morphine analgesia by means of a subcutaneously tunnelled cannula.

Peridural morphine analgesia is a well tested method of pain relief for patients with gynecological tumors which are in an advanced stage. Traditional methods of fixation can slightly inconvenience the patient when cleaning and dressing procedures are being carried out, and the cannula can be displaced or damaged. Fifty patients were treated with subcutaneously tunnelled peridural cannula. The procedure and its favorable results are described.
