[Localization of primary and recurrent bladder tumors].

The location distribution of the primary and recurrent bladder tumours was studied in 417 patients. The authors decided that the ureteral orifices, the lateral walls are the most common places of primary superficial bladder tumours. The favoured places of the primary invasive bladder tumours are the posterior walls. They found not so much disparity between the site of primary and recurrent invasive bladder tumours, but the difference between the site of the primary and recurrent superficial bladder tumours is remarkable. They decided, according to their patients material, that the recurrent superficial bladder tumours take places more frequently on the posterior walls, the dome and the anterior walls. The explanation of this phenomenon may be the tumour cells implantation during the TUR. The authors survived all the other processes can yield the local and distant recurrent bladder tumours and understanding of this processes can result in a better understanding of the location distribution of the recurrent bladder tumours on the one hand and it can help us to avoid or decrease the recurrences on the other hand.

The efficacy of intravesical BCG in the treatment of patients with high risk superficial bladder cancer.

Sixty-two patients with superficial bladder cancer stage T1 with or without dysplasia or carcinoma in situ and 34 patients with stage TA associated with dysplasia or carcinoma in situ were treated by TUR and TUR + BCG, respectively, and were followed up for an average of 46 months (6-192) and 32 months (6-72), respectively, after therapy. The recurrence rate (61%), progression (37%) and tumour death (19%) were unfavourably high in the group of patients treated by TUR only. On the contrary, the effect of intravesical BCG therapy was excellent. Recurrence rate was as low as 20.5%, and progression and tumour death were not detected in any patient. Intravesical BCG seems to be the best choice for treatment of high risk superficial bladder cancer. The authors recommend their effective modified treatment schedules.

Histopathological "self control" in transurethral resection of bladder tumours.

Between 1979 and 1989, 462 bladder tumours were resected in 319 patients using the differentiated technique. After transurethral resection (TUR) that was considered optically to be complete, histopathological examination of the specimen revealed residual tumour in 35% of cases. The danger of insufficient resection increases in relation to the depth of infiltration (pT) and is closely associated with the growth pattern of the tumour. The differentiated form of TUR is important in determining the depth of infiltration, peripheral expansion of the tumour and the completeness of removal.

[Results of BCG immunotherapy in the management of bladder tumors].

The authors reported their results of BCG immunotherapy in the treatment of the bladder cancer. between 1984-1989 60 superficial and 4 muscle infiltrative bladder tumors were treated. The results of 53 BCG treated superficial bladder cancer group were compared to control groups. The recurrence cases three-four times, the progressive cases eight-ten times were more in the control groups than it was found in the BCG group. They could not establish the value of BCG in the treatment of muscle infiltrative bladder cancer, because they had not appropriate number of patients and they could find only few articles in the literature. The authors surveied the last literature of BCG immuntherapy and verified that BCG had an important role in the treatment of the bladder cancer and searching the action of antitumor effect of BCG would add more useful information to cancer immunology.

[Histopathologic characteristics and clinical behavior of superficial bladder tumors].

The authors analyse the histopathological features and clinical behaviour of 325 superficial (Ta, T1) tumours of 232 patients with bladder cancer. Each of the tumours was treated by TUR using differentiating resection technique and multiple cold biopsy was performed in 197 patients. The recurrence and progression of superficial tumours are examined during the 3-192 months follow up time. It is stressed the role of incomplete TUR in the "recurrence" (tumour persistence). The opinion of the authors is, that superficial bladder tumours can not be considered a homogen group, there are significant differences between Ta and T1 stage in recurrence, neoplastic progression and histopathological features as well.

[Radical transurethral resection of bladder tumors].

During the last ten years the authors treated 462 bladder tumors of 319 patients in the expectation of radicality. On the basis of histopathological examination following TURs with differenciating technique the tumor-removing proved true in the cases of 360 tumors (78%). The first TUR was radical only in 65% of the tumors, in 10% the second, in 2% the third and in 1% the fourth intervention resulted full radicality. The authors analyse the number of TUR's needed to complete removing of tumors in connection with the depth of infiltration and the configuration of tumors. The exploration of the causes of failure makes possible the improvement of the results.

[Significance of seemingly intact urothelium in patients with bladder cancer].

During 9 years (from 1979 to 1988) the authors performed 412 multiple cold biopsies taken from 9 different points of the cancerous bladder in 263 patients. In 42 per cent of the patients the hystopathological examination of biopsy materials discovered dysplasia, carcinoma in situ or tumor of the grossly normal urothelium. The frequency of these proliferative lesions is closely connected with number, configuration, grading and depth of infiltration of overt bladder tumor. These lesions indicate the probable recurrence-rate as well as danger of the more progressive recurrence.

[Dysplastic urothelial changes in patients with bladder cancer].

The authors treated 238 patients with urinary bladder tumours in 4 years. In each case the transurethral resection formed an integrae part of the treatment plan in the form of a diagnostic, curative or palliative TUR. The transurethral intervention in 152 cases was carried out with differentiated resection technique according to Bressel, in 137 cases mapping was also made of 9 different points. By this technique detailed information was obtained about the pathological lesion of the mucosa which appeared healthy by endoscopy in 122 patients. Precancerous lesions which can form the basis for later malignant epithelial proliferation were found in 50,9% of the patients. The number and severity of the dysplastic lesions showed close correlation with the histological differentiation grade of the tumour.

[Bladder neck perforations caused by TURP (transurethral prostatectomy)].

In 511 successive transurethral resections of the prostate the postoperative cystography has been performed as a routine. A total of 66 perforations of the bladder neck have been observed, 68 per cent of which had been diagnosed at the transurethral adenectomy. In 45 cases the perforation of the capsule has been discovered only with the help of the cystography. If you aim at the total transurethral ablation of the adenom tissue, a certain number of perforation of the capsule is unavoidable: the causes, diagnostic and therapy of which as well as its postoperative control are outlined. It is recommended to perform the routine cystography after transurethral resections of the prostate.
