[Bilateral neonatal adrenal hemorrhage associated with hypoadrenalism].

A newborn is described, who was born in term with 3250 g weight, after intrauterine hypoxia. She was born with vacuum extraction, and suffered from birth trauma and intrauterine infection. At age of one day manifested an eclampsia, which was hardly diminishable. The serum Na level was 118 mmol/l. Bilateral adrenal apoplexy was visible with ultrasound. The newborn was treated with infusion of sodium chloride and glucose, with antibiotic and glucocorticoid hormone. Her condition improved rapidly. Her serum cortisol level at 8 and 15 day after stopping of 12 days prednisolon therapy was below 2.5 micrograms/dl, marking hypadrenia. At age 3 months the function of adrenals was normal: the serum cortisol level was 11.4 micrograms/dl, and after ACTH stimulation it run up to 44.5 micrograms/dl. By screening of 1491 newborns, 13 adrenal apoplexy was found. The other 12 were one-sided and symptomless.

[Fracture of the lower thoracic spine in ankylosing spondylitis].

Authors report on experiences gained during the treatment of the injury of the lower thoracal spine in a 64 year old male patient and review the relating literature. As a consequence of the basic disease fractures may occur following rather trivial traumas. As all ligaments are calcified a particular unstable state will develop following the fracture.

Production of 6.5 h 82mRb via the 82Kr(p, n)-process at a low-energy cyclotron--a potential substitute for 82Rb.

The production of 6.5 h 82mRb, a potential substitute for generator-produced 1.2 min 82Rb in myocardial blood flow studies, is discussed. Using a 82Kr gas target and a low-energy cyclotron it is possible to obtain greater than 100 mCi (greater than 3.7 GBq) of high purity 82mRb. Results of radiation dose calculations for 81Rb/81mKr, 82Rb and 82mRb are reported.

Hyperlipemia and hyperlipoproteinemia [HLP] screening among the children from premature myocardial infarction risk families.

Serum lipids and lipoproteins were investigated in the premature myocardial infarction (PMI) risk families before 45 years of age with the aid of screening for hyperlipemia and hyperlipoproteinemia (HLP): in the case of 174 persons from Csongrád County from the Departments of Internal Medicine I and II and of 42 patients (fathers) suffering from PMI and their 79 "high risk" children from Heves County. In the investigated three groups of "high risk" children the genetically determined antiatherogenic HDL-Ch level diminished in 34.8, 52.3, 40.5 per cent. Significant negative correlation was detected between the serum HDL-Ch and beta-lipoprotein; significant positive correlations were found between the HDL-Ch and the serum lipase activity; between the beta-lipoprotein and the phospholipid level; significant negative correlation was proved between the HDL-Ch and the phospholipid level in the group of PMI patients and their offsprings. The Ch/Tg, and the HDL-Ch ratios were significantly diminished in the PMI patients' group against the risk children' group, while the Ch/HDL-Ch rate was significantly elevated.

Gonadotropin release in infertile men following ketoconazole administration.

The pituitary gonadotropin releasing capacity of ketoconazole, (KC, p.o. 800 mg/day for 5 days), GnRH (i.v. 100 micrograms) and clomiphene, (CL, p.o. 100 mg/day for 7 days) was compared in 25 infertile male patients. It was found that KC resulted in a significant increase in serum LH (from 8.94 +/- 1.05 to 17.55 +/- 1.32 U/l), FSH (from 6.68 +/- 1.05 to 9.82 +/- 1.28 U/l), 17-OH-progesterone (from 4.51 +/- 0.34 to 11.35 +/- 0.68 nmol/l) and a decrease in serum testosterone (from 16.62 +/- 1.28 to 11.66 +/- 1.59 nmol/l) and dehydroepiandrosterone (from 6.56 +/- 0.39 to 4.82 +/- 0.37 mumol/l) levels. GnRH was effective in all patients, while CL failed to induce considerable amount of LH release (less than 2 U/l) in 7 patients and ketoconazole in 2 patients. It is important to note that KC was also effective in the CL resistant cases. Furthermore, it has been established that the amount of LH released by KC was midway between that of GnRH and CL. Almost 50 per cent of the patients failed to show a significant FSH release (less than 2 U/l), and in this respect, the three substances were similar. The KC test with its different mode of action can be recommended as a reliable diagnostic method in male patients.

[Rudimentary ovary syndrome].

21 patients with rudimentary ovary syndrome were studied with a review of the literature. Rudimentary ovary syndrome is one of the syndromes of primary ovarian failure characterised by limited ovarian function around puberty. Some cases are associated with chromosomal abnormalities, some others may be due to immunologic defect. Treatment considerations are discussed.

CA 125 levels in patients with hypoestrogenic ovarian failure.

The source of CA 125 in the sera of women is not known. Serum CA 125 concentrations were measured in 36 women with hypoestrogenic ovarian failure of whom 30 had hyper-(group A), 6 had hypo- or normogonado-tropic amenorrhea (group B). Group A consisted of 16 women with streak gonad syndrome, 6 with rudimentary ovary syndrome, 5 with premature menopause syndrome and 3 with gonadoblastoma. Serum CA 125 levels were normal in 34 patients irrespective of the presence of associated sex chromosome anomalies, and their mean values did not differ significantly from those of the 9 age-matched controls. We also measured normal CA 125 values in 2 females with müllerian agenesis. These data indicate that the derivatives of müllerian ducts do not contribute significantly to the presence of CA 125 in the sera of nonmenstruating adult women, and that the abnormalities of the X chromosome do not seem to influence CA 125 levels.

Effect of simultaneously administered iopanoic acid and thyrotropin releasing hormone (TRH) on thyroid hormone metabolism in euthyroid subjects.

The cause of serum T4 elevation after iopanoic acid administration was searched in euthyroid subjects. 3 gr iopanoic acid was given per os to 9 healthy volunteers (group I), in other 5 volunteers a TRH test was performed prior to iopanoic acid administration (group II). In group I TSH, T4, FT4I and rT3 levels increased, in group II T3 and FT3I decreased, but no TSH, T4, FT4I increase was seen, obviously, because of the exhaustion of pituitary TSH content by TRH. On the ground of the findings T4 elevation caused by iopanoic acid is predominantly consequence of TSH release and not that of inhibited T4 conversion rate.

Somatostatin-induced inhibition of lipolysis: in vitro studies.

Incubation of epididymal fat tissue slices with somatostatin (SS) led to the inhibition of epinephrine-induced release of free fatty acids (FFA) and glycerol in a dose-dependent manner. The SS administration did not suppress the lipolysis evoked by dibutyryl cAMP. The experimental findings indicate that SS exerts an inhibition of catecholamines-induced lipolysis at the level of adipocytes although the mechanism of action requires further investigations.

Superoxide dismutase activity in thymus cell suspension: effects of methionine-enkephalin and morphine.

Superoxide dismutase activity of rat thymus-derived cells was studied by the inhibition of L-adrenaline auto-oxidation oxidation in vitro. The incubation of cells in the presence of methionine-enkephalin (Met-Enk) or morphine (2-20.10(-7) M and 2-8.10(-6) M, respectively) was performed in Krebs-bicarbonate buffer at 37 degrees C for 180 min. After a lag period of 30 to 60 min of incubation, both Met-Enk and morphine decreased the inhibitory activity of cell suspension on the adrenaline autooxidation. Naloxone blocked the effects of opioids in near equimolar concentrations. The observations suggest the interaction of opioids on superoxide anion production of T cell lymphocytes.

Glucocorticoid-induced low molecular mass anti-inflammatory factors which do not inhibit phospholipase A2.

The peritoneal lavage fluid of dexamethasone (DXM)-pretreated rats was filtered through an Amicon YM-10 membrane (cutsize 10 kD). The retentate inhibited dextran oedema. Its 40 kD fraction (lipocortin) obtained by molecular sieving on Sephadex (SG)-75, suppressed carrageenin-induced foot swelling and phospholipase A2 (PLA2) activity, but it had no effect on the dextran response. The Amicon filtrate was chromatographed on SG-25 gel and yielded 6 and 2 kD fractions suppressing dextran and serotonin (5-HT) oedema but not carrageenin inflammation and PLA2 activity. These fractions may be active fragments of vasocortin, a glucocorticoid-induced mediator regulating vascular permeability.

Homologies between small nuclear RNAs and LAV/HTLV-III sequences and their possible role in the cytopathic effect of the virus.

The mechanism of action of LAV/HTLV-III resulting in a pronounced cytopathic effect is still unknown. We demonstrate that the long terminal repeat (LTR) sequence and env gene of LAV/HTLV-III contain regions of 70-80% homology and/or complementarity with regions of small nuclear (sn) RNAs U1 and U2. On this basis, we hypothesize that the cytotoxic effect of LAV/HTLV-III is due - at least partly - to the inhibition of processing of cellular hnRNAs by competing for splice junction sites and/or by complexing with U1 and especially U2 RNAs.

Morphine induced thyroxine release from rat thyroid gland in vitro.

Male rat thyroid glands were incubated for two hours in Krebs-Ringer bicarbonate buffer with different amounts of morphine and/or naloxone. Five micrograms/ml morphine produced a significant increase in the T4 concentration of incubation medium, and resulted in an accumulation of cAMP in the tissue. Naloxone did not change the T4 release but its incubation with morphine prevented the morphine-induced changes. Similarly, naloxone inhibited the morphine-induced accumulation of cAMP in the thyroid tissue.

Homologies between a brain-specific identifier (ID) sequence and regions of Harvey murine sarcoma virus and Rous sarcoma virus genomes. Putative role of identifier sequences in the tissue specificity of malignant transformation by RNA tumor viruses.

As a step toward understanding of the tissue specificity of cellular transformation by RNA tumor viruses were looked for the presence of a putative brain specific regulatory (identifier) sequence (C82B) in the genome of various oncornaviruses. The genomes of Harvey murine sarcoma virus and Rous sarcoma virus contain sequences flanking the viral oncogenes with greater than 80% and greater than 60% homology to C82B, respectively. We suggest that identifier sequences acquired by oncoviruses may determine the potential target cells of malignant transformation after virus penetration.
