[Cesarean section for the second twin during the course of delivering twins].

We have analysed 6 cases of twin pregnancies with vertex presentation of the first foetus. In this cases after delivery of the first twin by vaginal route caesarean, section was made. Caesarean section of the second twin was made because of: transversal presentation with fetal distress syndrom (four cases), umbilical cord drop (one case), and premature placenta ablation (one case). We have determined acid base balance and Apgar score. We have noted worse results for the second twin, independently too of the time between deliveries both twins. Caesarean section of the second twin is the rarely clinical situation, but in motivated situation is accepted and reasonable solution.

[Triplet gestation--analysis of pregnancy course, delivery and neonatal outcome, based on personal material].

UNLABELLED: Triplet gestation appears in 0.1-0.3% of all pregnancies and it is high risk pregnancy for mother and foetus. It appears more frequently in Afroamerican women, rarely in Japan women. In multifetal pregnancy early prenatal diagnosis and management are very important. AIM: Analysing course of pregnancy, way of delivery, condition of newborns, influence of environmental factors, and concomitant diseases in triplets gestation. MATERIAL: 30 women treated between 1989-1998, in Division of Perinatology, University of Medical Sciences in Poznań, Poland. RESULTS: 21 pregnancies were ended by caesarean section, 9 by vaginal delivery. Apgar score for II and III foetus decreases significantly. pH value of umbilical artery was without significant differences. CONCLUSIONS: Almost all triplets have ended preterm. Route of delivery of triplets have to be considered individually. Environment factors could play an important role in multifetal pregnancy.

[Thyroid hormone levels during pregnancy, labor and puerperium].

Concentration of thyroxine triiodothyronine and reverse-triiodothyronine in serum blood of 30 pregnant women were estimated, like a longitudinal study from 5 to 41 week of pregnancy. Significant increase of concentration of all hormones was observed. We presented also estimation of iodothyronines in blood serum in labor and non-complicated puerperium. We found increase of T3 concentration immediately after delivery, and decrease of level of all hormones in puerperium.

[Levels of thyroid hormones during pregnancy and in the perinatal period. II. Levels of iodothyronine in blood serum during cesarean section and puerperium].

Analysis of T4, T3 and rT3 concentration in blood serum of 27 women during and after caesarean section was performed. Estimation of hormones using RIA was done. Changes of levels of analysed hormones differed from this found during normal delivery. Periferral conversion of T4 is discussed as a potential source of different levels of T3 and rT3.

[Delivery in adolescence].

We analysed structure of age within 16,602 deliveries between 1947 and 1977 and data collected from 17,056 deliveries between 1989 and 1992 in Institute of Obstetrics and Gynaecology in Poznań. Percent of women under age 18 deliveries increased from 0.58% in 1947 to 1.3% in the last years. We observed a significant difference in group of teenage mothers: the higher risk of prematurity, lower average birth weight of neonates, lower frequency of cesarean section and higher frequency of surgical assisted vaginal deliveries. There was no significant differences in the frequency of birth weight under 2500 g.

[Vaginal delivery after previous cesarean section].

Data concerning frequency of cesarean section as well as frequency of vaginal delivery after cesarean section in Poznań region in years 1990-1991 were presented. During last years we observed significant increase of spontaneous deliveries after cesarean section. The indications for the previous cesarean section and the outcome of the present pregnancy are discussed.

[Prevention of immunization of women with Rh group antigens in the years 1970-1988 at the Institute of Gynecology and Obstetrics Medical Academy in Poznan].

The number of serological incompatibility cases in relation to the number of labours was analysed for the years 1970-1988 in the Perinatology Department, Institute of Gynaecology and Obstetrics, Medical Academy in Poznań, that is in the period when prevention of serological Rh incompatibility conflicts had been introduced and applied. The prevention methods applied in pregnant women and in women in labour made possible a reduction of Rh immunization of pregnant women by about 70%. On the basis of the results and clinical analysis it was found that a further rise of the effectiveness of prophylactic measures could be achieved by a wider use of immunoprophylaxis after artificial abortions and spontaneous abortions, by the use of adequate doses of anti-Rh-D IgG after labour, and introduction of immunoprophylactis methods already in a proper period of pregnancy.
