[Initial results of hybrid chemotherapy of Hodgkin's disease using COPP/VBA].

19 patients with advanced, previously untreated Hodgkin's disease were treated with COPP/ABV (cyclophosphamide, vincristine, procarbasine, prednisolone, adriamycin, bleomycine, vinblastine) hybrid regimen. Complete response was achieved in 8 patients, partial response in 9 patients and primary treatment failure occurred in 2 patients. Two patients received involved field irradiation for the residual disease. 17 months after finishing the therapy one of the complete responders suffered a relapse. Of the 8 partial responders, 5 patients relapsed. 2 relapses occurred during the treatment and 3 relapses were observed in the first 7 months of follow-up. 2 of the partial responders and both nonresponders succumbed to their disease. There were no serious side effects and treatment-related death did not occur.

[Comparative study of advanced-stage Hodgkin's lymphoma patients treated with either COPP or ABVD drug combinations].

Between January 1985 and March 1988, 45 patients with advanced Hodgkin's disease were randomly assigned to receive either COPP (22 patients) or ABVD (23 patients). 9 and 16 patients achieved complete remission in the COPP and ABVD groups, respectively. One of the complete responders (COPP) relapsed during the follow-up. Patients treated with COPP experienced myelosuppression and neurotoxicity more frequently whereas alopecia and gastrointestinal symptoms were observed among those receiving ABVD. Present data confirm the favorable impact of primary ABVD treatment in advanced Hodgkin's disease.

Lack of regional specificity for connections formed between thalamus and cortex in coculture.

The mammalian cerebral cortex consists of many structurally and functionally specialized areas, with characteristic input from particular nuclei of the thalamus. Some localized external influence, such as the arrival of fibres from the appropriate thalamic nucleus before or around the time of birth, could trigger the emergence of committed cortical fields from an undifferentiated 'protocortex. The guidance of axons from each thalamic nucleus to its appropriate target area in the cortex could, then, be crucial in the regulation of cortical differentiation. Recently, Yamamoto et al. and Bolz et al. have demonstrated that cocultured explants of rat lateral geniculate nucleus and visual cortex can form layer-specific interconnections. We have now tested the possibility that each cortical area exerts a selective trophic influence on axons from its appropriate thalamic nucleus, and vice versa, by coculturing explants of different regions of the thalamus and cortex taken at various stages of development. Although thalamo-cortical and cortico-thalamic connections formed in vitro can be remarkably normal in many respects, they lack regional specificity.

[Possibilities of abdominal ultrasonic diagnosis in the clinical follow up of malignant lymphomas].

Authors performed follow-up abdominal ultrasound examinations in malignant lymphomas, with abdominal lymphoid masses, during chemotherapeutic treatment. It was measured the volume-change of the examined abdominal lymphoid masses. Authors elaborated a new method, basing upon pseudocolour-coding of the ultrasonic picture. With the use of this method it became possible to evaluate quantitatively the change of internal echo-structure of abdominal lymphoid masses during the treatment. The change in the time of the measured structural parameteres was represented graphically. In the opinion of authors, the change of structural parameteres shows a characteristic connection with the course of the disease. The clinically evident relaps can be proceded more weeks by an unfavourable trend in the change of ultrasonic structure parameters. According to the results of the authors, the abdominal ultrasound diagnostic, complemented with the method of pseudocolour-coding can successfully help the clinical follow-up of malignant lymphomas.

[Significance of ultrasonic studies of the splenic hilus in malignant lymphoma].

In the course of abdominal ultrasonography of patients suffering from malignant lymphoma the authors observed frequently morphological alteration of the splenic vein in the hilus of spleen. Three conditions were determined which when occurring simultaneously created cases of dilated vein in the hilus of spleen. The incidence rate of dilated vein of the hilus of spleen has been determined in patient group with lymphoma and "healthy" control group. It was studied whether in cases with morphological alteration of the vein in the hilus of spleen the occurrence of abdominal nodal manifestations or the alteration of the sonographic structure of the spleen were detectable at a higher rate in the group of patients suffering from lymphoma. On the basis of the results the authors are of the opinion that when the sonographic signs of the dilated splenic vein of the hilus of spleen are present the negative result of sonography must be considered more carefully than usual and other more sensitive diagnostic methods must be applied for the detection of the abdominal manifestations of lymphoma.

Effect of body posture on the serum potassium response to glucose in healthy subjects, diabetics and hyperthyroid patients.

Administration of glucose induces normally a decrease in serum potassium, though exceptionally a paradoxcial increase (PGIH) may occur. The effect of posture on glucose-induced serum potassium changes in health and disease has not been previously studied. We have investigated the influence of changes in body posture on the different serum potassium responses to glucose in various groups of 47 subjects. We have found a smaller decrease in serum potassium in the upright position than in the recumbent one in 20 healthy subjects and in 12 non-insulin dependent diabetics, but no significant difference in hyperthyroidism. The variable effect of posture on glucose-induced insulin secretion in the various groups of subjects did not provide explanation for the postural inhibition of the normal serum potassium response to glucose. A tendency to PGIH was found in a small number of diabetics. Although PGIH was completely reversed by DOC the normal postural difference between serum potassium curves was uneffected. It was concluded that change in the body posture has a definite influence on the glucose-induced decrease in serum potassium in normal people and is not mediated by insulin and/or aldosterone; PGIH develops in the upright position more easily than recumbency; exact serum potassium changes after glucose loading can be explored only by adequate standardization and carrying out both recumbent and upright measurements.

Influence of chemotherapy on the phagocytic activity of mononuclear cells in patients with Hodgkin's disease.

The phagocytic activity of mononuclear cells (MNC) was measured in 18 healthy blood donors and 16 patients with Hodgkin's disease (HD) before and after a new chemotherapeutic cycle (Cyclophosphamide - Vincristine - Prednisone - Procarbazine, CVPP; Adriamycin - Bleomycin - Vinblastine - Decarbazine, ABVD) using radiolabeled, opsonized yeast cells. The phagocytic activity of the MNC of HD patients was found to be significantly higher than that of the healthy controls. In patients in whom chemotherapy induced remission or partial remission, phagocytic activity of MNC was also reduced significantly, while phagocytic activity was increased in two patients, whose clinical course was not influenced by therapy.

Glucose-induced hyperkalemia developing in the upright position in captopril-treated hypertensives.

Paradoxical glucose-induced hyperkalemia (GIH) was elicited exclusively in the upright position during captopril treatment in four hypertensive patients (Group 3) without impaired glucose tolerance in three. Inhibited aldosterone production and inappropriate plasma aldosterone responses to increasing serum potassium may have been responsible for the captopril potentiated GIH. Similar GIH was found in four non-insulin dependent diabetic control patients with mineralocorticoid insufficiency (Group 1) but without captopril treatment, which could be prevented by desoxycorticosterone. Normal serum potassium responses to glucose, uninfluenced by desoxycorticosterone were shown in four untreated hypertensive control patients (Group 2).

Pancreatic tissue pH in experimental acidosis and alkalosis.

The pH was studied in pancreatic tissue and arterial blood in 22 dogs. Respiratory acidosis was induced by CO2 inhalations in 5 dogs, respiratory alkalosis by hyperventilation in 5 dogs, metabolic acidosis by intravenous(i.v.) infusion of 0.1 N HCl in 5 dogs, metabolic alkalosis by i.v. infusion of 4.2% Na bicarbonate in 5 dogs. The differences between the shifts of blood pH and tissue pH were not significant statistically. Infusion of Ringer's solution failed to affect the pH in blood and pancreatic tissue in two control animals. It is concluded, that with an adequate tissue blood flow the pancreatic tissue pH is uninformative unless the blood pH is known.

Postirradiation changes on the lymphatics studied by lymphography.

Repeat lymphography was done in 33 patients following radiotherapy between 8--73 months. Absorbed dose delivered by 60Co-teletherapy ranged between 30--45 Gy-s in subdiaphragmatic and 30,6--56 Gy-s in mediastinal region. The main sequel of radiation was decrease in nodal size (66,6%), lumen narrowing of lymphatic vessels (51,5%); number of lymph nodes and lymph vessels diminished rarely (6%). Blocking of lymph circulation never developed. Dilatation of thoracic duct with accelerated contrast flow rate was thought to be arteficial due to decreased pooling capacity of decreased nodal size. Radiation induced changes develop within 12 months and do not progress further on. It is concluded that changes are mainly affected by the level of absorbed dose.

[Significance of radiologic examinations for the determination of the extent of malignant testicular tumors].

Interpreting 61 cases with malignant tumors of the testicle, the authors comment techniques of diagnostic radiology as well as their reliability. The fundamental importance of a lymphographic examination of both inferior extremities for judgement on the state of aortic lymph nodes is accentuated. In case of need, the examination has to be supplemented by intravenous urography and, more rarely, by cavography. In accordance with the author's observations, the mediastinal lymph nodes may function as a source of lymphogenic pulmonary metastastes; a careful radiological examination of this region, therefore, is not to be neglected. The principles of radiotherapeutic treatment, for elective or curative purposes, based on the results of complex radiological examination, are discussed.
