[New method for detection of nonpalpable breast cancer].

OBJECTIVE: The objective of the study was to evaluate the feasibility of radio guided occult lesion localization (ROLL) in the cases of nonpalpable breast cancers with sentinel lymph node biopsy. DESIGN: Prospective clinical study SETTING: Department of Gynecology and Obstetrics, 2nd Medical Faculty and Teaching Hospital, Praha METHODS: Thirty-eight patients with nonpalpable breast cancer diagnosed by core cut biopsy underwent an injection of the radiopharmaceutical 18-20 hours before surgery. The dose of the radiopharmaceutical was 45 MBg into tumor and 15 MBg subdermaly. Surgical excision of radioactive breast tissue with nonpalpable tumor was carried out using the hand held gamma probe and the same technique was performed for biopsy of sentinel lymph node. Standard localization technique with hook-wire was performed in the control group of fifty-one patients. RESULTS: Radioguided surgery of nonpalpable tumor was successful in all cases. Surgical margins were clear in all cases, in four cases (10.5%) the margins were only 2 mm. The sentinel node was identified in all cases, in 3 patients (7.9%) the sentinel node was involved by tumor and axillary lymphadenectomy was performed. Infiltrating carcinomas were diagnosed in the "hook-wire" group in 21 cases (41.2%), all with clear margins over 3mm. Proportion of margins status between ROLL and hook-wire was the same. It seems that the health tissue loss around the tumor is in hook-wire technique bigger but it requires next study. CONCLUSION: Simultaneous performance of ROLL and sentinel node biopsy is useful and practicable methods in the management of nonpalpable breast cancer. In comparison to hook-wire, radioguided removal allows reduced excision volume and better lesion centering within the specimen.

Biochemistry of transmembrane signaling mediated by trimeric G proteins.

Many extracellular signals are at the cell surface received by specific receptors, which upon activation transduce information to the appropriate cellular effector molecules via trimeric G proteins. The G protein-mediated cascades ultimately lead to the highly refined regulation of systems such as sensory perception, cell growth, and hormonal regulation. Transmembrane signaling may be seriously deranged in various pathophysiological conditions. Over the last two decades the major experimental effort of our group has been devoted to better understanding the molecular mechanisms underlying transmembrane signaling regulated by G proteins and to the closely related process of desensitization of hormone response. This review provides general information about the basic principles of G protein-regulated transmembrane signaling as well as about our contribution to the current progress in the field.

[Hormone replacement therapy, proliferation in the breast and risk of carcinoma].

OBJECTIVE: The objective of the study was to investigate the changes of histopathological and immunohistochemical parameters of breast tissue among the HRT users. DESIGN: Prospective, pilot study. SETTING: Departments of Gynecology and Obstetrics, Pathology and Radiology of the 2nd Medical School, Charles University and the Teaching Hospital Motol, Praha 5, Czech Republic. METHODS: The samples of breast issue were obtained by core cut biopsy and during the breast surgery procedures in the study group of HRT users (no = 48) and in the control group of women without HRT (n = 22). Proliferation index (Ki-67), expression of ERB2 oncoprotein and hormonal receptors (ER, PR) were examined in the breast tissue in addition to routine histopathological examinations. RESULTS: We did not record increasing frequency of proliferative and precancerous lesions in the group of HRT users. Ki-67 expression was very low both in HRT users and in the control group of women. The values of estrogen receptors expression in breast tissue samples of women with HRT were similar to the findings in the normal breast. The values of progesterone receptor expression were higher among the HRT users then non-users, but it can be considered as a normal response of breast tissue to hormonal influence. Expression of ERB protein in HRT users was similar to that found in non-users. CONCLUSION: The findings indicate that HRT has not increased the proliferation rate of the breast tissue in our study group, so this mechanism certainly does not increase the risk of breast cancer.

[Cataract surgery in eyes treated with vitrectomy for idiopathic macular holes].

The authors evaluated the final functional results in 30 eyes where on account of nuclear and cortical cataract an intraocular lens was implanted. The operation was preceded by pars plana vitrectomy (PPV) with internal gas tamponade on account of an idiopathic macular hole (IMD). The cataract the development of which was recorded in 57 eyes was the manifestation of a late postoperative complication from a total number of 84 phakic eyes (67.9%) which were operated on account of IMD between April 1996 and June 1999. After cataract surgery the corrected visual acuity improved in 23.3% eyes. The investigation confirmed improvement and stabilization of the final corrected visual acuity in 24 eyes (80%) and confirmed the importance of these surgical procedures.

[Glaucoma after surgery for idiopathic macular holes].

UNLABELLED: The objective of the prospective randomized study is to evaluate the effect of surgery of an idiopathic macular hole (IMH) on intraocular pressure (IOP). The study comprised 60 primary pars plana vitrectomies (PPV) on account of IMH using 16% perfluoropropane (C3F8). Stage III according to Gass predominated in 65% and stage IV in 28.3%. Furthermore the authors enlisted in the study 15 secondary PPV on account of a patent IMH. A total of 75 primary and secondary PPV were performed. In 43 PPV an autologous thrombocyte concentrate (ATC) was used. With regard to the IOP after PPV the patients were subdivided into the following groups. Group A: IOP under 24 mm Hg was recorded throughout the investigation period in 24 of 75 PPV (32%). The total mean value of IOP in group A was 17.4 mm Hg. Group B: Early increase of IOP during the first week after surgery was recorded in 51 of 75 PPV (68.0%) This group was divided into two sub-groups according to the maximum postoperative deviation of IOP during that period. Sub-group B1 comprised a total of 34 PPV (45.3%) where the maximum postoperative deviation of IOP was between 25-35 mm Hg. The total mean value of IOP in sub-group B1 was 23.5 mm Hg. In sub-group B2 there was a total of 17 PPV (22.7%) and the maximum postoperative deviation of IOP was above 35 mm Hg. The total mean value of IOP in sub-group B2 was 28.6 mm Hg. Moreover the authors investigated the effect of ATC administration on the IOP after primary and secondary PPV. They did not reveal a significant relationship between administration of ATC and postoperative glaucoma. CONCLUSION: 1. Glaucoma after PPV in IMH can in individual cases cause deterioration of the functional results of the operation. PPV in IMH has an increased risk: a) in a monoculus, b) in advanced primary glaucoma with an open angle, c) in primary glaucoma with a narrow angle.

[The retinal nerve fiber layer in glaucoma. II. The status of the nerve fiber layer and development of changes in the visual field. Prospective study].

PURPOSE: To determine whether retinal nerve fiber layer (RNFL) photographs can predict future glaucoma visual field damage. METHODS: In a prospective study 158 red free photographs of the RNFL were made from November 1994 to June 1995. 28 eyes of 19 patients with ocular hypertension (repeated intraocular pressure over 21 mmHg and normal visual fields--Peristat 433 Rodenstock--at first examination) could be re-examined for an average time of 42.3 (SD 4.5) months. RESULTS: Only in one eye with normal RNFL at the first examination typical glaucoma visual field defects appeared (6%), but in 6 of 12 eyes (50%) with photographically demonstrable RNFL defects. The difference was statistically significant. CONCLUSION: There is only a minor probability, that visual field defects would appear during 3-5 years in eyes with normal RNFL photographs (6%), but about 50% of eyes with originally found RNFL defects.

[Vitrectomy and autologous thrombocyte concentrate in the treatment of idiopathic macular holes].

OBJECTIVE: To evaluate the results of surgery of idiopathic macular holes (IMH) using or not using autologous thrombocyte concentrates (ATC). METHOD: Fifty eyes with IMH, stage 2-4, were divided into two groups. Group 1 comprised 27 eyes with an average persistence of complaints of 12 months where pars plana vitrectomy (PPV) and tamponade with 16% C3F8 were combined with the use of ATC. Group 2 comprised 23 eyes with a mean duration of complaints of 14 months which were treated by PPV and 16% C3F8 without ATC. In all IMH stage 2 and 3 the posterior vitreous membrane was detached from the retina. The epimacular membrane was removed, if present. The mean follow-up period in group 1 was 17.2 months and in group 2 12.3 months. RESULTS: In group 1 anatomical success was achieved in 21 eyes (78%), visual acuity improved by two lines and more in 16 eyes (59%). In group 2 anatomical success was recorded in 15 eyes (69%) (p = 0.326) and visual acuity improved in 10 eyes (44%) (p = 0.265). Nine eyes were reoperated with ATC. Reoperations improved the anatomical success in the whole series to 90% and the functional success to 62%. The visual acuity improved in 70% operated eyes within 12 months from the onset of complaints and in 47% eyes with a longer case-history (p = 0.1182). Complications included retinal tears in 5 eyes (10%), detachment of the retina in one eye (2%), minor facete-like defects in the pigmented epithelium in 22 eyes (44%), disciform maculopathy in 5 eyes (10%) and atrophy of the optic disc in 3 eyes (6%). CONCLUSIONS: PPV is beneficial in IMH in the majority of patients. ATC can improve anatomical and functional results. Functional success is more likely after surgery in the early stage of the disease. The results of surgery are jeopardized by various complications. It is important to inform patients on the limitations and risk of surgery.

[The retinal nerve fiber network layer in glaucoma. I. Introduction and methods].

In the retinal nerve fiber layer diffuse and localized defects can be seen in glaucoma. Photographs made with a green filter on black and white high contrast film allow to follow these alterations. This method will be used together with colour photographs for the statement of intra- and interobserver agreement.

[Bilateral retinal detachment].

In 1977-1990 a total of 2,554 patients with rhegmatogenic retinal detachment were operated by the cryosurgical method, using silicone episcleral implants. The total number of patients comprosed 36 (1.4%) where the authors detected during the preoperative examination retinal detachment, usually in an initial stage, on the other eye. In 30 patients the authors operated bilateral detachment of the retina simultaneously and in the remaining six during the same hospitalization. The poorer eye was characterized by 1. more extensive retinal detachment, 2. a higher grade of proliferative vitreoretiopathy (PVR). As to the preoperative findings, the authors consider the following points important: 1. Almost 70% of the patients did not report subjective complaints of the better eye. 2. With regard o the pathogenesis the patients were divided into three groups: lattice degenerations in 61.1%, idiopathic oral dialysis 11.1%, other factors 27.8% of the patients. 3. Symmetry of the retinal defect up to 90 in two-thirds of the patients. 4. Type of operation. On the poorer eye the authors performed segmental plombage in 12 eyes (33.3%), cerclage in 5 eyes (139%) and reinforced cerclage in 19 eyes (52.8%). For the better eye the authors indicated segmental plombage in 12 eyes (77.8%), cerclage in 4 eyes (11.1%) and reinforced cerclage in 4 eyes (11.1%). Subretinal fluid was punctured on the poorer eye in 19 eyes (52.7%) and on the better eye in 7 eyes (19.4%). After an average observation period of 40 months the retina became anatomically attached in 86.0% on the poorer eye, and in 100% on the better eye. Bilateral retinal detachment in our group can be considered a model situation of retinal detachment which developed roughly at the same time and under equal endogenous conditions of the given organism. It may be thus assumed that a different extent of detachment and grade or proliferative vitreoretinopathy before operation was determined above all by the period of retinal detachment.
