Comparison between the early use of skin flap and skin graft for the correction of large tissue loss at the elbow.

Six patients with soft and hard tissue injuries of the elbow region during the 1973 Yom-Kippur war are discussed. These six patients were treated with two types of procedures. One group was initially treated with application of a split-thickness skin graft followed by a thoracoabdominal, abdominal, or chest skin flap. The second group was initially treated with the application of a thoracoabdominal flap. There are few descriptions in the literature or in textbooks of the plan design method, nor are there comparisons of two types of procedures. In both groups the end results were the same. The differences were the time of hospitalization and the cooperation of the patients during treatment. In both groups the flap was transferred before further surgical reconstruction of bones and joints.

Adult

Treatment of extensive avulsions of skin and subcutaneous tissues.

This article deals with the treatment of extensive avulsion injuries. Five out of twenty-eight cases of extensive avulsion injuries are presented and illustrated. All of these injuries were caused by machinery or automobile accidents and were encountered in the three-year period from 1972 to 1975. Etiology, pathology, and treatment with mechanical and chemical debridement, porcine skin grafts, fasciotomy, delayed primary closure, stamp or mesh skin grafts, and skin flaps are detailed and discussed.

Experience in reimplantation of amputated digits.

Reimplantation was attempted in 24 of 30 amputated fingers in 16 patients; 17 attempts were successful. The indications for surgery, the operative technique postoperative complications and results are discussed. It should be stressed that revival of the amputated digits does not necessarily mean functional success, and a careful evaluation of the results is needed. Organization of the reimplantation service is very important for achieving better results in this branch of surgery.

Dermoid cyst of the floor of the mouth in an infant. (Case report).

Dermoid cysts of the floor of the mouth in the infant are rarely encountered although the aetiology is mainly embryological. Meyer proposes the classification of the cysts based on the histological differentiation of the elements of the cyst wall. A case is reported which shows differences in the epithelial lining of the cysts found followed by the differential diagnosis and treatment.

Cytology of myeloma cells.

A cytological, cytochemical, and cytometric study of plasma cells from 195 cases of multiple myeloma showed that, contrary to earlier reports, flaming cells, thesaurocytes, and intranuclear inclusions are not confined to IgA-secreting cases but are common also in IgG and Bence Jones varieties of myeloma. IgA-secreting cells are not larger, nor do they have a lower nuclear cytoplasmic ratio than other myeloma cells. On average, for a given mass of tumour, Bence-Jones, IgG, and IgA varieties of myeloma produce amounts of paraprotein in the ratio 1 to 1-6 to 2-7.
