[Expression of E-cadherin as related to prognostic factors and survivals in breast cancer].

E-cadherin (E-CD) expression and its clinicopathological implication were investigated in 26 patients with breast cancer by immunohistochemical staining. 12 out of 26 primary lesions (46%) express strong and homogeneous expression of E-cadherin (preserved type), while in 14 cases (54%), degree of E-CD expression was reduced, i.e., 8 patients with heterogeneous staining, 2 cases with weak but homogeneous, and 4 cases with lost expression of E-CD (reduced type). However, there was no statistically significant correlation among decrease of E-CD expression, histological features, and advanced stages of breast cancer. Analysis on survivals showed better prognosis in the group with preserved E-CD expression than without E-CD (follow up was more than 96 months or until death). These findings suggests that the patients with decreased E-CD expression may be associated with metastasis resulting in poor prognosis, and E-CD expression could be one of the prognostic factors.

[Clinical study on the effect of intra-arterial injection of mitoxantrone-lipiodol emulsion for hepatocellular carcinoma].

To investigate the effect of intra-arterial injection of mitoxantrone emulsified with ethiodized oil, forty-eight patients with hepatocellular carcinoma were evaluated. After the treatment, ten of the patients underwent hepatectomy. In the thirty-eight unresected cases, there were 15 (39%) partial responses, which continued 1.2 to 11.8 months (mean, 5.8 months). The 1- and 2- year survival rates were 67% and 38%, respectively. The response rate was higher in cases with tumor under 5 cm in diameter. In the remaining 10 resected cases, the necrotic areas in main nodules ranged from 65% to 100% (mean, 85%). This treatment might be applied effectively to patients with small hepatocellular carcinoma.

Differential effect of host microenvironment and systemic humoral factors on the implantation and the growth rate of metastatic tumor in parabiotic mice constructed between young and old mice.

B16 melanoma cells were injected into the tail vein of young mice, old mice and parabiotic mice constructed between young and old mice, and the number and shape of pulmonary metastases were compared among three experimental groups. In unpaired mice, the number of metastatic colonies in the lungs was 10-fold larger in young than in old mice. In parabiotic mice, the number in young mice was almost comparable with that of unpaired young mice, but the number in old mice approached the level of young mice. Metastatic colonies on the pulmonary surface of young mice were mostly nodular in shape, while those of old mice were flat in shape. The shape of colonies reflecting the tumor growth rate did not change in parabiotic old mice in spite of an increase in number. In young parabiotic mice, the large and intermediate colonies decreased with a concomitant increase of small ones as compared with unpaired young mice. These results suggest that the implantation of metastatic colonies in the lung is mainly dependent on systemic humoral factors and their growth is mainly dependent on the host local factors in the microenvironment, and distinct age changes of both factors greatly influence the metastatic mode of tumors, respectively.

Intestinal ischemia resulting from Buerger's disease: report of a case.

A 42-year-old man who had undergone below-the-knee amputations for Buerger's disease 9 and 14 years earlier required emergency surgery for intestinal ischemia caused by arterial occlusive lesions in the mesentery. The cecum and proximal part of the ascending colon were resected, and marked hyperemic thickening of the wall and severe fibrous adhesions were found. Postoperative angiography revealed occlusions in the superior mesenteric and right external iliac arteries. The resected specimen showed mucosal necrosis of the cecum and circular ulcers at Bauhin's valve and at the ascending colon. Microscopically, small mesenteric vessels were occluded by organized and recanalized thrombi with preserved vascular architecture. The patient has remained free of any further intestinal symptoms for 1 year following his discharge from hospital.

An experimental study on intestinal vascular lesions: a comparison between the effects of arterial and venous occlusion in the acute phase.

In order to clarify what early ischemic changes are produced by venous interruption in the intestine, intestinal vascular lesions caused by peripheral venous ligation were compared with those caused by arterial ligation in rabbits. With less than 30 min ligation, there were only 4 specimens from a total 13 without injury following venous ligation, whereas there were 7 from a total 11 without injury following arterial ligation. After 30 min venous ligation, both the mucosal and submucosal layers of all 10 specimens were affected, while following arterial ligation, 3 of the 11 specimens showed no histological changes. Moreover, in 3 of the remaining 8 specimens from the latter group, the injury was confined to within the mucosal layer. Thus, in cases of venous interruption, intestinal injury might be evoked earlier with more severe damage than in cases of arterial interruption.

[Preoperative intra-arterial injection of mitoxantrone for locally advanced breast cancer].

Mitoxantrone (MIT) is a new anthraquinone anti-cancer agent. We successfully treated 3 patients with locally advanced breast cancer, including two inflammatory breast cancers, by pre-operative arterial injection of MIT. The treatment protocol was MIT 12 mg/m2 injected into both the internal mammary and the subclavian artery with oral administration of 5' DFUR 1,200 mg/day. In two cases of inflammatory breast cancer, the redness of skin was immediately reduced after the first course. After 2 courses, all tumors were decreased over 50% in size. Standard radical mastectomy could be carried out on all patients. Preoperative arterial injection of MIT might be the treatment of choice for locally advanced breast cancer. This preliminary result encourages further study.

[Successful preparation of mitoxantrone emulsion containing non-ionic contrast medium].

Mitoxantrone, a new anti-cancer agent, was successfully prepared for Lipiodol emulsion. The mixture of Mitoxantrone and non-ionic contrast medium, Omnipaque 300, was combined with Lipiodol at the ratio of 1:2. When the ratio of Mitoxantrone and Lipiodol was 1:4, microscopic study revealed stabilized water in oil emulsion, which could release the anti-cancer agent slowly. We applied it for a case of hepatocellular carcinoma with good result. Intra-arterial infusion of this emulsion might be considered effective for treatment of hepatocellular carcinoma.

Enterocolitis due to methicillin-resistant Staphylococcus aureus--report of two cases.

Two cases of postoperative enterocolitis due to methicillin-resistant Staphylococcus aureus (MRSA) after gastrectomy were experienced. Case 1: A 59-year-old male underwent subtotal gastrectomy for advanced gastric cancer. Diffuse peritonitis progressed after the first operation, so reoperation for drainage was required. Two days after the second operation, a profuse watery diarrhea developed. Case 2: A 46-year-old male underwent total gastrectomy for early gastric cancer. On the fourth postoperative day, frequent vomiting and cholera-like diarrhea started, followed by profound shock several hours later. Both cases were treated successfully by the administration of vancomycin. Stool cultures of both cases revealed MRSA and it had the same minimal inhibitory concentration, coagulase type and enterotoxin type, so that nosocomial infection was indicated.

[An experimental study on portal infusion of 5-fluorouracil (the second report)].

This study was conducted to clarify the role of continuous portal infusion with 5-fluorouracil (FU) from two different standpoints. Experiment I (Assessment of the ability to reduce hepatic metastasis): Male Donryu rats (180-210 g) receiving intraportal inoculation of ascites hepatoma AH 60 C (4 x 10(6) cells) were divided into 3 groups; portal infusion group, venous infusion group and control. 5-FU (20 mg/kg/day) was continuously infused via either the portal vein or the femoral vein with heparin (100 U/kg/day) for 5 consecutive days. The portal infusion group alone showed a significant decrease in hepatic tumor volume on day 21 and prolonged survival time. Experiment II (Evaluation of the immunological effects): Animals without inoculation of AH 60 C were also randomized in the same way as Experiment I. There were no statistical differences among the 3 groups in total leukocyte counts, lymphocyte subpopulations or NK activities in the spleen, whereas hepatic NK activity was significantly decreased in the portal infusion group. Continuous portal infusion with 5-FU is considered effective to prevent lodging of tumor cells in the intra-hepatic portal system but might be disadvantageous for hepatic cellular immunity.

Omental bleeding with spontaneously derotated torsion--a case report.

A case of omental torsion seen in a 16 year old male is reported herein. Abdominal pain in the right lower quadrant suddenly developed just after the patient twisted his waist and an emergency laparotomy revealed a hemorrhagic mass at the edge of the right omentum, which was excised. Histological examination of the resected specimen showed hemorrhage without any venous thrombosis or infarction, possibly suggestive of omental torsion with early spontaneous derotation. The patient was successfully treated as a case of omental torsion.

[The changes in tumor marker levels after chemolipiodolization in early phase].

Changes of the serum tumor marker levels were analyzed in 80 patients with hepatic malignancy treated with chemolipiodolization. In 52 cases, the levels of the tumor marker elevated on the 1st day after treatment with an exponential drop afterwards. The levels of the tumor marker on the first day compared with those before treatment were significantly higher in effectively treated cases. The exponentially declining curve dissociated about 10 days before reaching the lowest point, and it gradually rose again in 32 cases. The second treatment of chemolipiodolization should be applied before dissociation of the exponential curve.

Clinical trials of long-term RF local hyperthermia for advanced gastric cancer.

The effect of local hyperthermotherapy (HT) using 13.56-MHz radiofrequency (RF) capacitive heating was evaluated in 25 patients with unresectable or recurrent gastric cancer. HT was carried out once to 3 times a week for a duration of one hour at each session. Patients who underwent RF-HT frequently showed maintenance of performance status, symptomatic improvement and a reduction in tumor size. Moreover, the survival time of 9 patients who had numerous metastases to the distant peritoneum was significantly high (p less than 0.01), compared with 42 historical control patients who also had massive peritoneal dissemination, but had not received HT. The results of this study therefore indicate RF-HT to be a favorable modality in the palliative treatment of patients with far-advanced gastric cancer.

Ampullary carcinoma associated with familial polyposis coli--a case report.

We report herein a case of a 43 year old female who was found to have developed ampullary carcinoma seven years following a subtotal colectomy for familial polyposis coli. She presented to us with only a light fever and early stage ampullary carcinoma was subsequently diagnosed by endoscopic examination of the upper gastrointestinal tract, including an endoscopic retrograde choledochopancreatography. The patient is presently alive and well without any evidence of recurrence 33 months after undergoing a pancreaticoduodenectomy.

Histologic features of gastric cancer in relation to patterns of spread.

The modes for growth and spread of gastric cancer were investigated in a surgical series of 115 cases--88 of early disease, and 27 involving the muscular layer. In addition, 93 autopsy cases with advanced disease were examined in order to analyze metastatic spread. In cancers of differentiated type in the mucosal layer, changes in histologic features towards the undifferentiated type were observed in the deeper layers, but no histologic transformation was seen in cancers of undifferentiated type. At autopsy distant metastases were common in cases with primary tumor of differentiated type, whereas peritoneal dissemination predominated when the primary lesion was undifferentiated. From the clinical standpoint, follow-up and recommendations to the patient should focus mainly on risk of peritoneal dissemination when adenocarcinoma of the stomach is of undifferentiated type, whereas metastases to distant organs should be taken into consideration if the malignancy is histologically differentiated.

Malignant epithelioid hemangioendothelioma of the liver, spreading through the hepatic veins.

A case of malignant epithelioid hemangioendothelioma of the liver mimicking veno-occlusive disease is reported. The histological, ultrastructural, and immunohistochemical features of the present case indicate striking similarities to epithelioid hemangioendothelioma (EHE) of the soft parts described by Weiss and Enzinger. Tumour metastasis to the lung gave a picture closely resembling intravascular bronchiolo-alveolar tumour (IVBAT) of the lung. EHE of the liver is considered to be a unique type of hepatic endothelial neoplasm behaving as a low grade malignant tumour with a veno-occlusive process which has rarely been described, and had previously been classified as other diseases or neoplasms.
