R-wave voltage in the right precordial leads in anthracycline cardiomyopathy: a clinical study.

It was observed that the QS pattern in the right precordial leads of electrocardiograms of patients treated with anthracycline antibiotics resulted in congestive heart failure. In a review of the literature dealing with patients with anthracycline cardiomyopathy, terminal electrocardiograms, when available, showed the QS pattern in the right precordial leads in all patients. The relationship between a decreased R-wave voltage in lead V1 and an increase of the anthracycline dose was evaluated by a clinical study of ten patients. Analysis was subdivided into two categories (group A and B). In group A, there was a dose-response relationship. Electrocardiograms in group B, on the other hand, did not show low R-waves. The present study suggests that increased injury to myocardial cells in the regions of the anterior septum and anterior left ventricular wall might be important in the pathogenesis of anthracycline cardiomyopathy. Patients in group B seemed to be able to tolerate chronic anthracycline cardiac toxicity.

Endotoxin shock in newborn dogs: serial hemodynamic studies.

Using a newly modified dye dilution method for cardiac output determination, we successfully performed frequent serial hemodynamic measurements in newborn dogs to characterize hemodynamic changes in endotoxin shock in neonates. Sixty-seven mongrel newborn dogs (2 to 20 days old, 300 to 1500 gm) were divided into four groups: group 1 (2 to 20 days old, 300 to 1500 gm) received normal saline solution, group 2 (2 to 10 days old, 300 to 800 gm) received 1.5 mg/kg of Escherichia coli lipopolysaccharide (LPS), group 3 (2 to 10 days old, 300 to 800 gm) received 10 mg/kg of LPS, and group 4 (11 to 20 days old, 801 to 1500 gm) received 10 mg/kg of LPS. Cardiac output, heart rate, mean arterial pressure, systemic vascular resistance, and minute work were measured serially after endotoxin administration for 4 hours. Despite extensive manipulation, these measurements were stable in controls throughout the length of the study. Endotoxin, administered at two different doses of 1.5 mg/kg and 10 mg/kg, had profound effects on hemodynamic responses. These effects included a significant dose-related fall in cardiac output, minimal changes in heart rate, and a marked rise in systemic vascular resistance. The hemodynamic changes reported in this study lend additional support to the hypothesis that maturational factors are involved in the hemodynamic response to LPS.

R-wave voltage in the right precordial leads in anthracycline cardiomyopathy: experimental animal model.

Electrocardiographic abnormality, similar to that observed in cancer patients during chemotherapy with the anthracycline antibiotics daunomycin and adriamycin, was produced in rabbits. They were chronically treated with adriamycin, and before each administration of the drug an electrocardiogram was recorded. They were subdivided into two categories with respect to the R-wave voltage in lead V1, on the same basis as our previous investigation in humans. In group A, there was a dose-response relationship, and they ultimately showed a QS pattern in the right precordial leads with or without ST-T changes in the left precordial leads. In group B, the electrocardiograms did not show a low R-wave in lead V1, but ST-T changes in the left precordial leads. The similarity of these electrocardiographic changes to those produced by myocardial infarction that we observed in humans suggests that more severe injury to the myocardium at the anterior septum and anterior left ventricular wall may be important in the pathogenesis of anthracycline cardiomyopathy.

Congenital ventricular aneurysm and diverticulum in children.

Clinical profiles were analysed of 18 children with congenital ventricular aneurysm (CVA) and diverticulum (CVD) (nine with CVA and nine with CVD). Of 18 children, only six had any symptoms, consisting of chest discomfort, palpitation, or convulsion. Heart murmurs were heard in nine of the 18 children, and a nonspecific systolic ejection murmur in one. A systolic click was heard in only four children. Dyskinesia of an abnormal protrusion of the cardiac silhouette on the chest x-ray film was detected in only three. The ECG revealed abnormal findings in all children. In those with CVA, signs of myocardial damage or left axis deviation (LAD) and left bundle branch block (LBBB) were the main findings. Among those with CVD, multiple premature ventricular contractions (PVCs) were present in four; some signs of myocardial damage were present in three. None of those with CVD had LAD or LBBB. Among the four children with right ventricular diverticulum, multiple PVCs were present in three. The lesions were detected by two-dimensional echocardiography (2DE) before angiography in 13 (72%) of 18 children. The wall kinetics of the lesions, as seen on 2DE, were in agreement with the subsequent angiographic findings.

Rheumatic fever as a cause of myocardial infarction with coronary narrowing.

A 12-year-old boy with rheumatic fever associated with myocardial infarction is presented. As sequelae, left ventricular aneurysm and diffuse narrowing of the anterior descending branch of the left coronary artery were demonstrated by angiography. Given the high incidence of coronary arteritis in rheumatic fever, it is likely that arteritis contributed to the infarction.

Persistent wedge-shaped contrast enhancement of the kidney.

A case with a single wedge-shaped high-density area of the kidney that persists after urography was observed by CT scan. This might be an aberrant form of acute renal failure with multiple wedge-shaped contrast enhancement.

Sex differences in acquired cystic disease of the kidney on long-term dialysis.

Acquired cystic disease of the kidney in chronic glomerulonephritic patients who had been on hemodialysis more than 3 years (4.5 +/- 1.2 years at the base line examination), were followed for 3.6 +/- 0.5 years (at the follow-up examination) by computed tomography (CT scan). The renal volume in the 25 male patients was significantly greater at the follow-up examination than at the base line examination. On the other hand, the renal volume in the 12 female patients was similar on both examinations. The enlargement of kidney volume more than twofold during the follow-up was seen in 9 of 25 male and none of 12 female patients. The grade of cystic transformation was increased in male patients during the follow-up period. These results indicate that sex-related endogenous substances might play a role in the pathogenesis of acquired cystic disease.

Changes of the hepatic CT absorption value in hemodialysis patients.

The effect of hemodialysis on the CT absorption value of the liver was examined in 14 hemodialysis patients. In the high ultrafiltration group [10 patients with a weight loss of -1.7 to -3.5 (2.9 +/- 0.6) kg], the hepatic CT absorption value increased from 49.6 +/- 7.0 to 58.7 +/- 8.7 Hounsfield units (p less than 0.001) during hemodialysis. The increase in hepatic CT absorption value during dialysis correlated with the weight loss (p less than 0.001, r = 0.798). A 5.3% body weight loss by ultrafiltration resulted in an 18.5% increase in the CT absorption value of the liver. This study suggests that one should be careful in making an early diagnosis of hepatic disease, especially hemosiderosis, in hemodialysis patients based on the hepatic CT absorption value because it is affected by hemodialysis.

Aneurysms of the coronary arteries in Kawasaki disease. An angiographic study of 30 cases.

Thirty patients with coronary aneurysms associated with Kawasaki disease underwent coronary arteriography. Of 53 aneurysms, five were saccular, 24 sacculofusiform, 19 fusiform and five tubular. When tubular aneurysms were included in the fusiform type, the incidence of each configuration in the right coronary artery was almost the same as that in the left coronary artery. The left anterior descending coronary artery had the most aneurysms, followed by the right coronary, left main and circumflex arteries. Right coronary aneurysms always involved the bifurcation or the region from which a branch vessel arose; 13 of 31 left coronary aneurysms did not involve the bifurcation.

Renal cortex visualization and analysis of dynamic CT curves of the kidney.

Renal cortex visualization and dynamic renal computed tomography (CT) were carried out by bolus injection of contrast medium. In vivo renal cortex visualization showed that renal cortex volume and cortex thickness are both inversely correlated with age. The percentage volumes of cortex and columns of Bertin are correlated inversely in non renal-failure subjects. Percentage volumes of cortex tend toward low values in renal failure and status after renal transplantation. In analyzing dynamic renal CT curves, the corticomedullary junction time is a good index of mild impairment of renal function.

Evidence for patchy renal vasoconstriction in man: observation by CT scan.

We report the first demonstration of patchy vasoconstriction of the kidney in man, together with temporary impairment of renal function. Observations were made by computerized tomography (CT scan). Such vasoconstriction might occur during the mild form or the recovery phase of acute renal failure.

Eisenmenger syndrome: measurement of left atrial volume.

Three cases with Eisenmenger syndrome were reported. Two of then were infants and one was adolescent. In the infant cases, the left atrial volume was small and the left atrial volume change ratio was large. In the adolescent case, the left atrial volume was large and the left atrial volume change ratio was reduced. In the adolescent case, the decrease in the left atrial volume change ratio probably caused the pulmonary hypertension, but in the infant cases, the left atrial volume change ratio was not decreased and therefore this cannot be the cause of the pulmonary hypertension. The brain damage with microcephaly might play a role in the development of the high pulmonary vascular resistance.

Nonspecific ("pseudo-plasmal") dye-binding in the Feulgen nuclear stain and its blocking by azocarmin G.

In Feulgen nuclear staining nonspecific dye-binding due to the "pseudo-plasmal reaction" is intensified in isolated cells with intact cytoplasm, and cannot be eliminated by the post-irradiation method. Fluorescence intensity in the cytoplasm sometimes exceeds that of specific nuclear fluorescence, especially in brain and heart muscle cells, and it was almost impossible to perform cytofluorometric DNA quantification on such specimens. Various kinds of aldehyde-blocking agents such as sodium borohydride, 2,4-dinitrophenylhydrazine, aniline, and sodium pyrosulfite were effective in reducing the "pseudo-plasmal reaction". But the blocking effects were not complete because of additional release of reactive aldehyde groups during subsequent Feulgen hydrolysis. Acidic azocarmin G produced a complete block of all "pseudo-plasmal reaction" in acriflavine-Feulgen nuclear staining, allowing accurate DNA-cytofluorometry to be carried out.

Stokes-Adams attacks due to acute nonspecific myocarditis in childhood.

Six patients aged between 1 to 11 years (4 females and 2 males) developed Stokes-Adams attacks with complete heart block due to acute nonspecific myocarditis. Transvenous pacing was instituted in 2 patients, but in the other 4 patients the ECG returned to normal by isoproterenol. All ECGs in complete heart block showed QRS complexes of right bundle branch block with left posterior hemiblock pattern, except for 1 which showed QRS complexes of complete left bundle branch block pattern. The ECG improved sequentially in order and left anterior hemiblock pattern of QRS complexes remained to the last during the convalescent period. Normal atrioventricular conduction returned by 2 to 24 hours in all but 1 patient who was dead and 1 of the 2 patients with shock. In another patient bifascicular block has persisted.
