[An initial assessment of therapeutic value of strontium 85 isotope in treatment of chronic osteomyelitis in adults].

An initial assessment of treatment of chronic osteomyelitis in 16 adults with Strontium 85 is presented. A dose 3.5-5 times greater than standard scintigraphic one was administered. In 3 cases the dose has been repeated because of the persistence of infection. The radionuclide was well tolerated. In 14 cases ceasing of osteomyelitis have been achieved. Laboratory tests showed no abnormalities.

[Value of "the Nissen phenomenon" in minimal osteotomy of the knee in experimental animals].

An attempt to assess duration of the biological stimulation after osteotomy known as the "Nissen Effect" has been made. An experiment has been performed on 5 adult rabbits injected with 100 Ci strontium 85. Biological stimulation of the osteotomy was measured by the incorporation of the isotope into the bone which has been increasing to reach the maximum 3 months after the operation. At 5 months postoperatively the incorporation ceased what indicated termination of the stimulating effect of the osteotomy.

[Analysis of knee arthrotomies performed during 14 years of department activity].

General analysis of 1075 knee arthrotomies in 1037 patients has been carried out. Young, physically active males prevailed 39 per cent of these procedures). In 20 per cent of the cases the injury to the structure other then the meniscus was responsible for the knee malfunction. A major damage to the articular surface has been found in 24 per cent of the cases. In 18 per cent it was associated with meniscal injury, in 6 per cent with the hypertrophied fad pad.

[Angiosarcoma of the femur following chronic osteomyelitis].

Available literature concerning neoplastic metaplasia of the bone undergoing chronic osteomyelitis has been cited. The treatment of 62 years old patient with lasting 47 years chronic osteomyelitis of the femur after shotgun wound was presented in details. The diagnosis of angiosarcoma following chronic osteomyelitis has been established histologically. The extremity has been amputated with good result--no metastasis or recurrence was found during follow-up.

[Injury of the acromial end of the clavicle imitating dislocation of the acromio-clavicular joint].

Two patients have been presented: a 15-years-old boy with rupture of the clavicular-coracoid ligaments as in total acromioclavicular dislocation, in whom dislocation of that joint did not occur but acromial epiphysiolysis of the clavicle (this patient, was successfully treated surgically) and 5-years-old boy with strain of the clavicular-coracoid ligaments and fracture of the acromial end of the clavicle; he was treated conservatively with remaining of angular dislocation of the distal fragment. The hope for rebuilding of the axis was fulfilled in that child. The anatomical and functional result in both patients is good.

Acromioclavicular Joint↗

[Fixation of multi-fragment fracture of the clavicle using an external stabilizer "Zespol"].

After a short presentation of the treatment methods of clavicle shaft fracture and the atypical use of Zespol stabilization in other than union of long bones shaft operation, the treated patient with ++multi-fragment clavicle shaft fracture with indications for operative reposition and fragments osteosynthesis has been presented. The Zespol plate and 4 screws were used above the skin as an external stabilizer for immobilization of the fragments. A good result was achieved: bone union and full mobility of the joints.

[Giant benign osteoblastoma of the first metacarpal bone. Late results of its removal together with the whole metacarpal bone].

A case that is a recurrence of the localization of the first described case: within the first metacarpal bone, has been presented in detail. The tumor was of enormous size, eliminating grip function of the hand. It was removed surgically together with the whole first metacarpal bone, which original borders could not be determined. In the place of the removed tumor, freeze-dried cortical-spongy bone was inserted. The follow-up at 5 years after operation revealed full healing of the graft, without any symptoms of tumor metastasis and excellent hand function.

[Treatment of multiple injuries of the pelvic bones (with rupture of the pubic symphysis) by stabilization of the latter using the Zespol method].

Fractures of the pelvis with disruption of the symphysis ossis pubis and, especially, the surgical treatment: stabilization of the symphysis ossis pubis by various kinds of the use of the ZESPOL stabilizers have been presented. The difficulties in the conservative treatment in 2 patients with multiple fracture of the pelvis, in whom the correct position of other injuries of the pelvis depended on the stabilization of the symphysis ossis pubis, have been presented in detail. In the discussed patients, it was obtained by the ZESPOL method employed in one patient subcutaneously and in the other externally. This is the first report about the use of the ZESPOL stabilizer in the treatment of symphysis ossis pubis disruption. It provides good conditions of healing of the symphysis ossis pubis ligaments, makes possible earlier mobilization and standing position of a patient. The presented employment of the ZESPOL stabilizer in the treatment of symphysis ossi pubis disruption widens the range of the application of this stabilization method.

[Surgical treatment of pain caused by external tibial abnormality].

The authors discuss in short the accessory bones of the foot and their clinical significance. The table and the X-ray pictures present in detail six patients with external tibial bone in ten feet, causing pain in 7 feet. In all the operated on feet (including 4 in children), degenerative-deforming changes in the articulations between the accessory bone and the navicular bone were found. It was confirmed six times by histopathological examination. The authors think that the degeneration causing pain is itself an indication for removal of the accessory bone and that too few patients with this defect are treated surgically.

[Isolated fractures of the pisiform bone].

The literature has been discussed and two patients followed up for this injury have been presented. The patients were treated conservatively. Good results were obtained.
