Clinical outcomes and direct hospital costs of reduced-intensity allogeneic transplantation in chronic myeloid leukemia.

A reduced-intensity conditioning allogeneic stem cell transplantation was given to 19 patients (aged 15-59 years) in the first chronic phase and one patient in the accelerated phase with chronic myeloid leukemia (CML) after a regimen consisting of fludarabine (Flu), busulfan (Bu) and ATG Fresenius. The median follow-up was 27 months. Until day +100, no transplant-related mortality was recorded. The incidence of acute and chronic graft-versus-host disease (GvHD) was 55 and 75%, respectively. Two patients (10%) died from GvHD. Fourteen (70%) patients achieved molecular remission. Additional post-transplant intervention (donor lymphocyte infusion, imatinib) was necessary, however, in 10 patients (50% of the patients; non-achievement of stable molecular remission or later relapses). The total direct cost of the transplantation treatment for all of the patients came to 1,572,880 euro. If the patients had been treated with imatinib and followed-up with the same time period as they were following a transplantation, the direct cost of the imatinib treatment would have been 2,005,117 euro. The transplantation treatment appears to be less expensive after approximately 2 years of follow-up. Flu+Bu+ATG is a low-toxicity regimen for patients with CML. However, a close follow-up is necessary and about 50% of the patients require further therapeutic intervention.

Cardiac function and cardiopulmonary performance in patients after treatment for non-Hodgkin's lymphoma.

Authors conducted a one-year prospective study to determine whether CHOP regimen (cyclophosphamide, doxorubicin, vincristin, and prednisone), used in the treatment of aggressive non-Hodgkin s lymphoma, is associated with the presence of an early impairment of cardiac function. Forty seven patients were prospectively examined (27 male and 20 female) aged 49+/-14 years who were treated with CHOP regimen. Rest echocardiography was performed at baseline and one-year control. Cardiopulmonary exercise test was carried out at one-year control examination. The ejection fraction (EF), parameters of diastolic function, myocardial performance index (MPI), and pVO2 were used as parameters of cardiopulmonary performance. The cumulative dose (CD) of doxorubicin was 277+/-56 (300 mg/m(2)) was given. The baseline EF 64+/-5% (64%) decreased to 58+/-7% (57%) at the one-year control (p<0.0001). 23% of patients exhibited a drop in EF >10% during the follow-up. 43% revealed a pathologically increased value of MPI >0.55, and 47% impaired diastolic function compared to the baseline values, respectively. 21% of patients exhibited a decrease of pVO(2) < 20 ml/kg/min, and 17% pVO(2) < 80% of the reference value, respectively. None of the patients developed signs of heart failure. The Doppler parameters of both diastolic and global LV function were the most affected measures and significantly influenced the cardiopulmonary performance. Multivariate analysis showed that CD > or =300 mg/m(2) (OR=8.08; p<0.05) and the presence of risk factors (OR=9.48; p<0.008) are the best predictors of cardiotoxicity. The results show that subclinical cardiac impairment was frequent in patients receiving the CHOP regimen with safe cumulative doses of doxorubicin. The value of described changes for the development of heart failure has to be assessed during the prospective follow-up.

Clinical utility of selected disease activity markers in patients with systemic lupus erythematosus.

The objective of this study was to assess the utility of measurement of thrombomodulin, antinucleosome antibodies, sVCAM-1, sICAM-1, neopterin, fas ligand, IL-10 and sIL-2R in patients with systemic lupus erythematosus (SLE) and to compare them with traditional markers of SLE activity (anti-dsDNA antibodies, C3, C4) and the ECLAM index of disease activity. The measurement was performed over a 6-month period at three consecutive time points after 3 months in each of the 52 patients with SLE. Anti-dsDNA antibodies, thrombomodulin, antinucleosome antibodies, sVCAM-1m sICAM-1, neopterin, fas ligand, IL-10 and sIL-2R were tested by ELISA technique, while C3, C4 components of complement were tested by nephelometry. Fas ligand and IL-10 did not correlate with the ECLAM index. The rest of the markers showed significant correlation with the disease activity index. Thrombomodulin and anti-dsDNA antibodies reflect in the best way the changing trend in disease activity. Antinucleosome antibodies seem to be a promising marker useful in early diagnosis. Soluble VCAM-1, sICAM-1, neopterin and sIL-2R are interesting molecules with a role in disease pathogenesis, but their practical utility is limited.

[Osteoarthrosis].

The locomotor system, especially its functional important areas, joints and spinal column, undergo degenerative changes of various range during the age and owing to endo- and exogenous factors. Osteoarthrosis affects peripheral joints, spondylosis and spondylo-arthrosis affect axial skeletal joints. The problem of osteoarthrosis becomes actual both owing to the increasing incidence of this illness and in the connection with the prolongation of human life. The problem of osteo-arthrosis is significant all over the world. That's why the attention is devoted to this illness from the point of view of etiology, pathogenetic mechanisms and pathologic morphology. A special attention is devoted to joint's medium. We deal with clinical signs, diagnostic possibilities and therapeutic principles. We emphasize the necessity of complex treatment and we report its single parts. In this connection we emphasize psychological and sociological aspects of the access to the patient with this serious chronic illness. Last but not least we deal with osteoarthrosis as the economic and social problems.

[Coagulation analytical studies in coli enterotoxemia in swine].

Store pigs with spontaneous outbreaks and experimental endotoxin shock were kept under observation in the context of coagulation analysis. Heparin was applied to some of the animals to disrupt the plasmatic coagulation system. The thrombocyte count in animals with endotoxin infusion declined by some 50 to 65 percent of the original level. No statistically secured difference was found to exist between heparinised animals, on the one hand, and non-heparinised, on the other. The aggregation and adhesion of thrombocytes in all shock animals was more pronounced than that in the controls. The fibrinogen levels were lowered in both the animals with spontaneous outbreaks and the experimental animals. Thrombocyte alteration was not found to have been dependent on activation of the plasmatic coagulation system. In endotoxin shock cases activation of plasmatic coagulation proteins was found to be preceded by rise in thrombocyte aggregation.
