Bronchopneumonia with measles in infants and children at the Department of Child Health School of Medicine, University of North Sumatera/Dr. Pirngadi Hospital Medan (January 1985-December 1989).

A retrospective study was done on patients with bronchopneumonia hospitalized at the Dr. Pirngadi Hospital, Medan/School of Medicine, University of North Sumatera, during January 1985 through December 1989. The purpose of this study is to assess the morbidity and mortality of bronchopneumonia with measles and the nutritional status of the patients. Out of the 14.082 patients admitted, 1310 children suffered from bronchopneumonia (10.7%). Ninetynine of them were also with measles (7.6%). Most of the patients with bronchopneumonia without measles were in the age group 0-1 year (58.7%). The mortality was 24.8% and the highest mortality rate was in the age group of 4-5 years (34.6%). Most of the patients with bronchopneumonia and measles were in the age group of 1-2 years (30.3%). The mortality rate of bronchopneumonia with measles was 22.2% and the highest mortality rate was in the age group of 3-4 years (35.7%). Bronchopneumonia with or without measles occurred more in malnourished patients rather than in well-nourished cases. All of the patients who had bronchopneumonia with measles in the present study had never been immunized against measles.

Gastroenteritis at the pediatric ward of Tembakau Deli Hospital, Medan, in 1988.

In a period of 1 year (1 January-31 December 1988) a retrospective study on patients admitted to the Pediatric ward of Tembakau Deli Hospital, Medan, was done. This hospital is a referral hospital for patients from hospitals of Dept. of Plantation in North Sumatera and Aceh. During year 1988, there were 1339 infants and children hospitalized at the Pediatric ward Tembakau Deli, Medan. Of these patients, 183 had gastroenteritis; it was most frequently found in the age group of 0-13 months (46.4%). The patients consisted of 53.55% males and 46.45% females. Most of the patients i.e. 140 (76.51%), had good nutrition. Malnutrition were found in 31 patients where 16.94% among them were moderate malnutrition, and 12 patients (6.55%) had malnutrition. The total mortality rate of gastroenteritis in this study was 2.18% which was usually associated with severe complications while pure gastroenteritis showed no (0%) mortality.

Sydenham's chorea.

P, an Indonesian boy, 5 years old, 15 kg of body weight and 110 cm of body length, was admitted to the PTP IX Hospital on March 30, 1987 with cerebral palsy. The patient had fever since 10 days before admission which lasted 7 days; he showed restlessness by involuntary movements when catching, thus when holding something in his hand, it fell off. He had difficulty in walking and talking. On physical examination, were hyperemic the tonsils and pharynx; there was muscular hypotonia, flexed arms and metacarpophalangeal extension. Laboratory findings showed leukocytosis, increased ASTO titer, positive CRP and normal cerebral spinal fluid. Head CT scan showed no abnormality. Electrocardiogram and EEG were in normal limits. Throat swab culture produced Group A Streptococcus beta hemolyticus. The diagnosis of sydenham's chorea was established. Procaine penicillin, phenobarbital and vitamins were administered to the patient. After 5 weeks in the ward the patient was doing well, and now he is working and speaking normally; he is on ambulatory treatment with 1.2 million units/month of long-acting benzathine penicillin, at the Department of Child Health, Tembakau Deli Hospital, Medan.

Clinical features of severe malnutrition at the pediatric ward of Tembakau Deli Hospital, Medan.

Six hundred and fifty seven hospitalized patients under five in Child Health Department Hospital Tembakau Deli Medan, from January 1988 until December 1988 had been investigated retrospectively. Severe PCM are found 12 (1.8%), consisting of 7 (58.3%) boys and 5 (41.7%) girls. Most cases were found at the age of 1-2 years (33.3%). Marasmus were found in 5 cases (41.7%), Marasmic kwashiorkor in 4 (54%), while Kwashiorkor in 3 cases (25%). Clinical features of the patients are as follow: hepatomegaly 7 (58.3%), anorexia 6 (50%), old man face 5 (41.7%), subcutaneous fat decrease 5 (41.7%), thinsparse easily pick hairs 5 (41.7%), muscle hypotrophy 5 (41.7%), edema of the lower extremity 4 (33.3%), crazy pavement dermatosis 2 (16.7%). All patients were hospitalized combined with other diseases as chronic diarrhea 6 (50%), bronchopneumonia 5 (41.7%) and ascariasis 4 (33.3%). Mortality in 1 patient.
