[Why is it worth to examine the biomechanical properties of cancellous bone?].

The study of biomechanical properties of cancellous bone is of growing interest in many fields of medicine especially in the aspect of osteoporosis. Biomechanical properties of bone have been investigated for over 100 years. It was and still it is caused by the necessity of an evaluation of fracture risk. Nowadays, the prediction of osteoporotic fracture is based on bone mineral density measurement which is the most important, but not the only, factor of bone strength. Based on fundamental rules of biomechanics, it is obvious that the strength of each material (cancellous bone too) depends not only on the amount of this material but also on internal structure, shape and biomechanical properties. Ideal method should assess all these properties in vivo and it is looked for. In vitro studies are the base of experimental studies of cancellous bone. The strength tests should be treated as a "gold standard" of them. They allow to improve a knowledge of cancellous bone and to explain the relationships existed between biomechanical properties and bone mineral density, ultrasound parameters and chemical composition. A good knowledge of these relations should help in the diagnostics and monitoring of metabolic bone diseases.

[Keratectomy for a malignant melanoma of the cornea].

PURPOSE: To present the possibility of malignant melanoma treatment of the cornea using keratectomy. MATERIAL AND METHODS: A 59-year old woman with a primary malignant melanoma of the cornea. Visual acuity before surgery 0.2. Malignant melanoma was removed using keratectomy. Follow-up 18 months. RESULTS: Satisfactory cosmetic result and visual acuity 1.0 was obtained. In 18 months follow-up no local or general symptoms of tumor recurrence are observed. CONCLUSION: Keratectomy should be considered as an alternative treatment for primary malignant melanoma of the cornea.

[Fetal hypotrophy: morphological pictures of placenta in light and electron microscopy of female residents of the Czenstochow region].

The 16 afterbirths samples obtained from pregnancies complicated by idiopathic fetal hypotrophy was examined. The criteria of hypotrophy were established: the duration of gestation above 36 weeks, the birth-weight was less than 2500 g. The control group included 8 afterbirth samples from physiological full-term pregnancies. It was performed macroscopic examination, light microscopy and transmission electron microscopy. Macroscopically, no changes were found in the study group apart from the small differences of the weight. Light microscopy showed the retardation of placenta maturation from fetus hypotrophy group, congestion of the villi and presence of the numerous calcifications. Transmission electron microscopy showed the changes of placental barrier elements: adaptation processes and chronic progressive damage of the cells (exfoliation of the endothelial cells from the basal membrane).

[The immunological and morphological aspects of chronic hepatitis caused by HBV].

The authors present in the study actual knowledge of the pathogenesis of the chronic viral hepatitis type B. Dividing of the disease into stages with description immunologic reactions connected with morphological features of the liver is used. Three stages of the disease have been reported: replication, elimination and integration of HBV in the liver. It present the results of immunohistological analysis of the incidence of the virus antigen in the tissue in the particular stages of the disease. Next it explains the mechanisms of immunological reaction leading to the incidence of particular forms of the inflammation activity and types of necrosis that occur in the chronic viral hepatitis type B.

[Difficulties in diagnosis of cytomegalovirus mononucleosis syndrome].

A case of a healthy 23-year-old woman is reported with cytomegalovirus mononucleosis as a result of infection of cytomegalovirus probably primary. The patient presented with symptoms of generalized adenopathy, migratory arthralgias and arthritis, hepatosplenomegaly, long lasting rash as well as complications of pneumonia and myocarditis. Because on histopathological examination of lymph node the Hodgkin-Reed-Sternberg-like cells were found a misdiagnosis of Hodgkin's disease was initially made. After about 8 weeks period there was a complete recovery. The current problems related to cytomegalovirus infection are presented.

[Selected elements of biological activity of circulating immune complexes in serum of patients with atopic diseases. I. Ability to injure guinea pig tissue by injecting atopic patient plasma].

Serum level was measured of circulating complexes (SpA binding method-CIC/SpA) in pollen and house dust mite sensitive patients during specific immunotherapy. Histopathological changes in guinea pigs skin after the intradermal injection of patients serum have been studied. Differences were demonstrated in the observed patho-morphological changes in relation to CICs/SpA serum concentrations.

[Angiosarcoma of the femur following chronic osteomyelitis].

Available literature concerning neoplastic metaplasia of the bone undergoing chronic osteomyelitis has been cited. The treatment of 62 years old patient with lasting 47 years chronic osteomyelitis of the femur after shotgun wound was presented in details. The diagnosis of angiosarcoma following chronic osteomyelitis has been established histologically. The extremity has been amputated with good result--no metastasis or recurrence was found during follow-up.

[Carcinoid of the larynx].

The case of carcinoid of the larynx is presented. Attention is focused on the difficulties with establishing the diagnosis. The principles of treatment this kind of neoplasm are reported.

[Value of endomyocardial biopsy in the assessment of the degree of the changes in myocarditis].

Right ventricular diagnostic biopsy was performed in 25 patients (12 females and 13 males) aged 19-48 (mean 32) with suspicion of acute or subacute myocarditis for definitive diagnosis verification and to determine indications for therapy with corticosteroids or azathioprine. 3-4 specimens taken from different sites of ventricle were estimated under the light microscope according to Dallas classification distinguishing active myocarditis with or without fibrosis from borderline myocarditis with inflammatory infiltration not causing myocytes damage. Routine clinical and laboratory tests, echocardiographic examination, ECG Holter monitoring and nuclear examination to evaluate left ventricular ejection fraction were performed in all patients. 6 patients underwent coronarography to exclude atherosclerotic changes in coronary arteries. Bioptic examination result confirmed myocarditis suspicion in all patients. Active inflammation was stated in 18 patients including 7 with coexisting fibrosis. Borderline myocarditis was stated in 7 cases. Comparison of clinical examination results with those of pathomorphologic study made possible to state, that tachycardia, cardiac murmur, arrhythmias and left ventricular function impairment were frequent observed in the group of active myocarditis. Ejection fraction less than 30% was observed only in patients with active myocarditis and mainly in those with coexisting fibrosis. Based on performed study we consider that myocardial biopsy allows not only to diagnose myocarditis but also is helpful for lesion extent assessment in myocardium with all clinical implications.

[Primary lung cancer from biopsy material of the Pathomorphology Department of the Silesian Medical Academy in Zabrze].

An analysis was carried out of material biopsied during fiberoptic bronchoscopic examination of 289 patients suspected of lung malignant disorders were present more often in males. In the group of patients older than 40 years the most often diagnosed malignant process was squamous cell carcinoma. A high correlation of clinical suspicion of malignant process and histopathological diagnosis was found. A low rate of diagnosis of operable cases was found.

[Cytologic prints of primary and metastatic melanomas. Preliminary report].

In 15 cases of melanoma (10 primary tumours and 5 metastatic ones) imprint examination was performed intraoperatively. In one case the material was non-diagnostic. Out of the remaining 14 cases six tumours consisted of epithelioid cells, one--spindle-shaped and seven were of mixed structure. Typical characteristics of the imprint included cellular dispersion, polymorphism, presence of binuclear and giant cells as well as intranuclear cytoplasmic inclusions. There features are the same as those described in other cytologic studies of these neoplasms. The imprints method turned out to be useful as an accessory and complementary study intraoperative histopathologic examination.

Polyamine levels in sarcoma-bearing rats in relation to growth and necrosis of the tumour cells.

Putrescine (PU), spermidine (SPD) and spermine (SPM) levels in blood serum of MC Sa 1828 P-bearing rats have been studied in relation to tumour weight and the histological picture. A statistically significant increase of PU and SPD was found in the course of tumour development. Decrease in the level of polyamines in the necrosis stage was also found. Only increased SPD level appeared in the period of intensified necrosis, accompanied by lymphocyte infiltration.
