An assessment of the quality and usefulness of the EuroTransMed satellite programmes in continuing medical education.

The quality and usefulness of the EuroTransMed (ETM) international satellite medical educational programmes were assessed. Medical educational sessions are broadcast once a week from London in English to over 140 sites in Europe. The aim is to provide an update of medical information to physicians as a part of their continuing medical education (CME). A questionnaire was distributed to two groups of postgraduate medical trainees, internists and family doctors. The groups viewed two video-recordings of the satellite broadcasts, translated into Polish. The first programme was watched by 219 doctors and the second by 265. There was a 96% response rate to the questionnaires. Results suggest that the ETM satellite programmes are both relevant and of good quality, but are of limited usefulness to the target audience. These limitations result mainly from a shortage of time available for such a form of education, as well as from difficulty understanding the English broadcasts, because of a lack of fluency among general practitioners in Poland.

Ultrastructure of the primitive neuroectodermal tumors (PNET).

We report a prospective series of consecutive cases of primitive neuroectodermal tumor (PNET) studied by electron microscopy. Virtually all specimens showed a differentiation along neuroblastic lines as evidenced by the presence of neurites, dense-cored vesicles, microtubules and adhesive plaque junctions. We observed also numerous intracytoplasmic cilia and autophagic vacuoles. Synaptic specializations were only rarely seen. We conclude that PNET is a tumor category which is not undifferentiated ("primitive") and clearly exhibits features of neuroblastic differentiation.

[Familiarity with diseases of the esophagus, stomach and pancreas among trainees who applied for specialization in general surgery].

The range of knowledge of the diseases of oesophagus, stomach and pancreas amongst 277 trainees in general surgery has been investigated by means of the 180 multiple-choice question examination. Analysis of the results revealed that the knowledge of the above subject with constituted 19% of the questions, does not differ from that of the rest of general surgery problems, distributed in the remaining 81% of the test's content.

Problem of objective performance assessment of primary health care physicians.

Training programmes for primary health care physicians often lack methods for valid and reliable performance assessment of the trainees. A set of rating scales has been developed with the aim of increasing the objectivity of measurement of practical skills required for the management of cardiac failure. To prove the validity of the content and structure, as well as the inter-observer reliability, the scales were tested by the method of "competent judges" with the use of video-recorded clinical simulation. Calculated Kendall's coefficient of concordance W was at the minimum acceptable level of 0.5. The results reveal considerable differences among examiners (competent judges) regarding internal criteria of acceptable level of performance, and they point out the need for further search for more objective methods of performance assessment.

A three-year training programme for primary health care physicians in Poland.

A three-year training programme was developed and implemented by the Medical Centre of Postgraduate Education in Poland with the aim of improving the knowledge and skills of primary health care physicians working in the State Health Services' community centres. The programme creates several learning and self-assessment opportunities with the general principle of not distracting a doctor from his daily duties in a community health centre for more than one day a month. Preliminary assessment of the programme revealed its positive impact on both the health services system and primary health care physicians themselves. It increased health authorities' activity in creating new learning opportunities and doctors' motivation to learn. Self-assessment made by the first group of trainees who completed the three-year programme (results obtained by means of a questionnaire) revealed substantial increase in professional competence resulting from the participation in the programme.
