[Far-advanced otosclerosis].

OBJECTIVES: The aim of this study was to report a series of patients with far-advanced otosclerosis who were unable to benefit from hearing aids. Among patients with profound hearing loss, it is particularly useful to diagnose far-advanced otosclerosis even if relatively rare, because stapes surgery can improve hearing to a level allowing conventional hearing aid use (sometime the only choice before cochlear implant). MATERIAL AND METHODS: We retrospectively reviewed the charts of 7 patients (9 operated ears) in order to highlight diagnostic criteria, surgical indications and results of stapes surgery (stapedectomy or stapedotomy). RESULTS: Diagnosis of far-advanced otosclerosis was based on clinical presentation, course of hearing loss, positive family history of otosclerosis, and results of CT scan which was helpful in all the cases. Obliterative otosclerosis was found in 55% of the cases. Success of stapes surgery was observed in 89%, with no significant difference between stapedectomy or stapedotomy. CONCLUSION: Although rare, far-advanced otosclerosis must be diagnosed because patients can benefit from stapes surgery (and subsequently fitting of appropriate hearing aids).

[Thyroid tuberculosis].

OBJECTIVES: The aims of this study were to emphasize the difficulties of diagnosing thyroid tuberculosis because of its non specific symptoms and to encourage physicians to seek it more frequently. MATERIAL AND METHODS: Six cases of thyroid tuberculosis diagnosed within a 3-year period were included in a retrospective study. RESULTS: There were two men and four women, mean age 45 years. Four patients underwent lobo-isthmectomy with frozen section analysis which enabled us to rectify the diagnosis. Medical treatment was based on a 6-month course of bactericidal chemotherapy. Outcome was favorable in all the cases with 17 months average follow up. CONCLUSION: Symptoms of thyroid tuberculosis are misleading, pathologic findings are of increasing importance for diagnosis, which with the new serology techniques may improve further.

[Benign cementoblastoma. Apropos of a case and review of the literature].

The cementoblastoma is a rare benign tumor of odontogen origin that interests the roots of the teeth especially the lower molars. The evolution is often silent, the radiological exam is not specific, the diagnosis of certainty is histological. The surgical treatment assures the recovery without aftermath. The authors report a case of cementoblastoma at a patient of 37 years for which he has benefited of a resection by vestibular way and debate diagnosis and therapeutic modes of this affection.

[Reposition flap techniques in fingertip amputations: 6 cases].

PURPOSE OF THE STUDY: The purpose of this study was to evaluate an alternative procedure for amputations distal to the distal interphalangeal joint: the reposition flap. MATERIALS AND METHODS: The reposition flap was used for 6 patients who underwent fingertip amputations in an emergency setting. Pulp was excised on the amputated segment and the remaining bone and nail bed were reattached to the proximal stump with a Kirschner wire. The pulp was reconstructed with a local advancement and sensitive flap. The patients were aged 18 to 44 years and had been victims of work accidents. All refused finger shortening. RESULTS: The fingers showed good scarring and good trophicity. Two-point discrimination was 6 mm. Bony fusion was constant but all distal interphalangeal joints remained stiff. Cosmetic results were correct except for two cases of claw nail formation. DISCUSSION: Fingertip amputations have been widely reported. Methods have varied from directed scarring to partial toe transfer. These situations present two types of challenge: insensitivity of the volar aspect or an overly sensitive pulp; cosmetic presentation and function of the dorsal aspect due to the complex role of the nail. Distal reimplantation remains the best technique, but the reposition flap offers an interesting alternative in case of failure or for patients who do not accept finger shortening. The advantage of the reposition flap is that it preserves finger length and the nail. Work stoppage and intolerance to cold can be an inconvenience due to the long time required for wound healing.

Plasma prostaglandin activities in some immuno-inflammatory diseases.

In the present study the relationship between plasma prostaglandins (PGs) level and the immuno-inflammatory reactions, known to accompany bronchial asthma and/or schistosomiasis infection, was investigated. It was found that plasma PGF2 alpha increased significantly above the control values in 30 asthmatics, 30 schistosomal and 30 asthmatic schistosomal patients. Plasma PGE increased also together with that of PGF2 alpha which is considered to be useful for the protection of the patient against the injurious effects of the infection, due to the known opposing actions of PGE to those of PGF2 alpha.

A new antitumour substance, 7-oxabicyclo (2.2.1)-5-heptene-2,3-dicarboxylic anhydride.

7-Oxabicyclo(2.2.1)-5-heptene-2,3-dicarboxylic anhydride has been found to possess antitumour activity against Ehrlich ascites carcinoma cells. The tumour cells incubated with the drug showed a decrease in the viable counts and cell proliferation. These effects were confirmed by in vivo studies in Swiss albino mice. The compound has a direct cytotoxic effect on the tumour cells. Vacuolization and disruption of the cytoplasm accompanied by unequal nuclear division and scattered chromosomes were recorded. In addition, 250 and 10 mg/kg were found to be the MTD and MED respectively. A dose of 25 mg/kg injected i.p. for 5 consecutive days in the tumour-transplanted animals caused a significant increase in their survival period. The compound has been shown to have a significant inhibitory effect on the DNA and RNA biosynthesis of EAC cells after 3 hr of administration; the protein biosynthesis was less affected. Meanwhile, the cellular contents of these metabolites were significantly reduced.

Kuwaitimycin, effect on synthesis of lipids in Bacillus subtillis cells.

The effects exerted by kuwaitimycin on synthesis of lipids as well as some metabolic activities of Bacillus subtilis were studied. The antibiotic not only arrested the inocrporation of 14C-acetate into the microbial lipids but also altered the fatty acids pattern, contents of i-C 15, a-C 15, i-C 17 and a-C 17 WERE MARKEDLY REDUCED, CONCOMITANT WITH AN INCREASE IN THE CONtents of i-C 14 AND N-C 14. Moreover, the rates of synthesis of phospholipids were decreased by the drug, especially that of phosphatidyl ethanolamine.

4,4'-isopropylidine-bis(2-isopropyl)phenol, a new inhibitor for cell wall formation of Bacillus subtilis.

4,4'-Isopropylidine-bis[2-isopropyl]phenol was found to possess antimicrobial activity against gram-positive bacteria and some fungi, whereas it had no effect on gram-negative organisms. The drug has a potent inhibitory action on the synthesis of cell wall mucopeptides of Bacillus subtilis by inhibiting the enzyme d-glutamate ligase, which is responsible for the incorporation of d-glutamic acid into uridine 5'-diphosphate-muramyl-l-alanine. The drug had a weak lytic effect on protoplasts and inhibited protein synthesis, whereas no significant effect on the synthesis of deoxyribonucleic acid and ribonucleic acid was found.
