[Trophoblast expression of EGFR (epidermal growth factor receptor) in the preeclampsia placenta].

UNLABELLED: Placental pathology in preeclampsia and retarded fetuses is associated with changes of the pattern of expression of EGFR. The purpose of this study was to investigate the expression of epidermal growth factor receptor in trophoblast populations of preeclampsia and retarded fetuses in comparison with normal controls. Tissue microarray with paraffin sections containing a general of 90 placental specimens including 30 preeclamptic cases, 30 retarded fetuses and 30 normal pregnancies (controls) were studied immunohistochemically for epidermal growth factor receptor in the syncytiotrophoblast and villous cytotrophoblast. Epidermal growth factor receptor levels were estimated semiquantitatively. The percentage of strong/intermediate immunoreactivity of epidermal growth factor receptor in the syncytiotrophoblast was significantly higher in normal placental cases (98%) than in pathological conditions (P = .03), although strong/intermediate immunoreactivity in the syncytiotrophoblast was significantly lower in pathologic cases (35%) than controls (P < .001). No correlation (P > 0.01) was found between the trophoblast expression of EGFR and the weight of the newborn fetuses. Epidermal growth factor receptor expression in the villous and extravillous cytotrophoblastic cells were significantly different between controls and preeclamptic cases (P > .05). CONCLUSION: The development of pathologic pregnancies is possibly related to the differential expression of epidermal growth factor receptor in the syncytiotrophoblast.

[Proliferation of villous trophoblast and stroma in normal and pathologic pregnancies (preeclampsia)].

Pregnancies, complicated with preeclampsia, present with intrauterine growth retardation of the fetus and small (in size and weight) placentas. The present study aims at establishing the expression of proliferative nuclear antigen Ki-67 in the villous trophoblast and stroma, and to compare the results between study (preeclamptic cases n = 30) and control (normal n = 30) groups. The number of Ki-67 positive nuclei both in the preeclamptic trophoblast and stroma significantly increases those of the controls. Our results show that the defect in the development of the placenta in preeclampsia is not associated with low proliferative potential.

[Socio-economic characteristics of pregnant women with preeclampsia: a gypsies population survey].

Preeclampsia as a serious syndrome affecting both the mother and the baby is associated with a number of socio-economic, demographic and clinical characteristics. We have filed the distribution of major epidemiological parameters in two gypsy groups, comprising 58 preeclamptic and 118 control cases. Only the classical features of preeclampsia as well as single liveborns were included in our study. Data are taken from the medical files and from a direct interview with the patients. Nonparametric and multivariate analysis are applied. The two study groups show insignificant differences in the distribution of the major epidemiological factors. Significant p-values are obtained referring to maternal age, parity, obesity. Speculations as to gypsy lifestyle are discussed as possible explanation to the achieved results.

In vivo testing of Enterogenin for a mutagenic activity of bone marrow hemopoietic tissue of mice.

The purpose of the present study was to test Enterogenin for genotoxicity. The micronuclearic test of Schmid W. was used. Enterogenin was introduced intraperitoneally, once and/or twice in two doses on male and female mice of the BDF 1 hybrid line. The positive controls were injected with clastogenic agent. Bone marrow smear preparations were made, determining the ratio between the normochromic erythrocytes (NCE) and polychromic erythrocytes (PCE), and the number of micronuclearic erythrocytes (MNE). Enterogenin has no genotoxic effect on the hemopoietic tissue of bone marrow of mice the ratio NCE/PCE being within the reference values. The preparation showed no clastogenic effect (p > 0.05), compared to the controls, as the number of PCE with micronuclei did not change 24, 48 and 72 hours after treatment with the two studied doses.

[The assessment of the occupational risk to workers in electric steel production].

These studies, centering on occupational environmental factors, biologic monitoring, and toxicodynamic investigations, involved a total of 105 workers distributed into eight job groups, who were 45 years, of age on the average and had from 5 to 10 years of special occupational experience at "Electrosteel" Works. Evidence was obtained for presence of unfavorable microclimate conditions, elevated equivalent levels of noise, excess of general and local vibrations, exposure to manganese aerosols, carbon monoxide, and nitrogen oxides; some of the subjects and deviations in hepatic status and connective tissue. Job groups found to be at risk were those of steel founders, crane workers, and pourers.

[The assessment of arsenic exposure in copper smelting].

The arsenic exposure in the main departments and occupational groups of the copper smelter in Pirdop has been estimated in the present study. The contents of arsenic is measured in the air and in biological samples--urine and nails. At most of the workplaces the time-weighted average (TWA) concentrations do not exceed the threshold limit value (0.05 mg/m3). The maximum TWA level is about three times higher. The intake of arsenic is significantly increased in almost all observed occupational groups. However, the excretion of only 8% of the workers is higher than the maximum background level (100 micrograms/l). The estimated degree of exposure corresponds to a low health risk. Some of the most heavily exposed occupational groups may be expected to reach higher levels of intake and health risk.

[An evaluation of the biological action of carbon disulfide in clinico-laboratory studies of exposed workers].

A representative group of 122 workers from shop "Predilen" with 7 to 15 years length of service in specialty is taken. The professional groups are formed on the basis of measured average-shift concentrations of carbon bisulphide in the air of the working place, which vary from 4 to 50 mg.m-3. In order to establish deviations in some of the basic links of the biochemical mechanism of the toxic compounds effect the following indices are traced: copper metabolism [copper concentration and activity of ceruloplasmin in the serum]; lipidic metabolism [content of cholesterol, cholesterol of high and low density, triglycerides, beta-lipoproteins and nonesterified lipidic acids in the serum]; intercellular substance of the connective tissue [glycoproteins, glycosaminoglycans and hexauronic acids in serum and urine]; immune status [immunoglobulins A, G and M in serum]; peripheral blood [leucocytes, DKK, morphology and lipid content in granulocytes]. Decrease of the lipids in granulocytes is registered, as well as reduction of the ceruloplasmin activity, increase of immunoglobulin M and glycosaminoglycans in urine. The already established deviations follow the "dose-effect" and "dose-response" dependence.

[Skin reactivity and the biological assessment of chromium exposure].

Subject to the examination are 140 workers from the plant for hydraulic products "Iv. Tenev", town of Yambol. The dermatological examination embraces 57 workers from shop 21, 49 from shop 24 and control group of 24 persons. The examination establishes skin changes in 24.6% of the workers from shop 21 and in 38.7% in those from shop 24. In the same workers were proved occupational dermatoses respectively in 5.3% for these of the first shop and in 14.3% for those of the second shop. The results of the test of 10 workers from shop 24 gave positivation of the test with 0.5% chromic anhydride at 10.5%. The studies for toxicokinetics of chromium in the examined workers proved its high values in urine, erythrocytes and nails. Raised chromium levels are established in the urine of those working with chromium in comparison with the control group (p 0.0001). There is a good correlation dependence between the chromium in urine and in erythrocytes (r = 0.71; n = 45%; p < 0.0001). Persons, eliminating significant quantities of chromium in the urine have higher values of chromium in erythrocytes, which shows that the chromium entering the organism of workers in contact with chromium is 6-valent. The connection with the chromium in urine and in nails is more lightly expressed and is statistically insignificant (r = 0.35; n = 29; p < 0.05), which is due to the fact that the chromium in urine reflects the near exposure in time, while chromium in nails--the level of exposure before 6-8 months.(ABSTRACT TRUNCATED AT 250 WORDS)

Factors influencing prognosis of patients with CNS supratentorial astroglial tumors who underwent postoperative radiotherapy.

To find the association between survival and various prognostic factors such as age, extent of surgical intervention, duration of complaints, site of tumour, histologic grade of the tumours and the Karnofsky performance status, we studied retrospectively 179 patients with histologically proven cerebral gliomas treated postoperatively in the Radioisotopic Center from 1976 to 1989. The patient's histologic tumour grade, the Karnofsky performance status and age proved to be particularly very indicative for longer survival time. The obligatory treatment with postoperative radiation therapy has been discussed in our previous works in connection with irradiated volume and a total radiation tumor dose. The state of the patients prior to postoperative telegamma therapy (Karnofsky Performance Status) is of major importance both for the therapeutic effect of irradiation and for the quality and duration of patient's further life. Patients with low-grade tumours (I and II), the Karnofsky performance status of 70% to 90% and age under 40 years live longer.

[Toxicokinetic exposure tests for chromium].

In cases of occupational exposure to chromium, very often in the organism enters increased quantitatively or six-valent form of metal, possessing high toxicity, cancerogenic and allergenic effect. Concentrations of the element in urine (CrY), erythrocytes (CrE) and nails (CrH) are chosen as suitable tests for evaluation of the exposure. The studies are performed on 151 exposed and controlled persons. The results received are assessed in view of their accuracy and application for biological control. CrY is appropriate test for evaluation of the near in time exposure. CrH detects the cases of occupational CrH--of increased exposure and deficit. On the basis of the proved dependences dose (CrB)--effect (CrY, CrE, CrH) in exposed workers are recommended BMAC (MAC for Cr = 0.02 mg/m3): CrY-2.3 micrograms/l, CrE-3.6 micrograms/l CrH-2.2 micrograms/g.

[Epidemiological examinations of the workers at a thermoelectric power station].

Medical and clinico-laboratory examinations were performed of thermoelectric power station (TES) workers who were exposed to the hazardous factors related to the technological process and contamination substances transferred from other industrial sites. Following prepathologic and pathologic states were registered: respiratory functional disorders, ischemic heart disease, arterial hypertension, audiometric state deviations and acoustic nerve neuritis, irritative dermatitis. Basing on the data obtained, risk groups identification was performed.

[Primary tumors of the central nervous system from material of the Department of Pathological Anatomy--Plovdiv, 1975-1984].

8136 deceased at the clinics of the Higher Medical Institute "I. P. Pavlov" for the period of 1975-1984 are studied. 265 of the deceased (3.28%) have tumours of the central nervous system (cns), among which astroglial tumours predominate (73.21%), while nonastroglial tumours represent 26.79%. The largest is the number of astrocytomas II degree--99 cases, followed by astrocytomas III degree--47 cases, multiform glioblastomas--28 cases and astrocytomas I degree--20 cases in accordance with differentiation of astroglial tumours. A larger part of astroglial tumours are localized supratentorially (86.6%) as the mature age is manifested mainly (31-60 years)--61.34% (p less than 0.001). The male sex predominates (61.84%) over the female sex (38.16%) (p less than 0.001) among the decreased with astroglial tumours. The cerebral oedema--64.43% is the most frequent immediate cause for the death of these patients. Menigiomas (42.25%) predominate among 71 patients with nonastroglial tumours, followed by meduloblastomas and ependimomas--15.49% and respectively 12.68%, oligodendrogliomas 8.45%. The remaining histogenetic types of papillomas of choroid plexus, angioreticulomas (hemangioblastomas according to the classification the World Health Organization, 1979), craniopharingiomas are more rarely encountered tumours.

[Death from inflammatory central nervous system diseases during 1950-1984 at the Pavlov Medical Institute].

The inflammatory diseases of the central nervous system (cns) are characterized by dramatic clinical picture and the outcome depends on early diagnosis and early treatment as well. In the report the dynamics of the lethality after inflammatory diseases is studied retrospectively for 35 years (according to five-year-plans). Cases with inflammatory diseases of cns are within the ranges of 2.3% (1980-1984), 8.86 (1950-1954) or 3.58% on the average for the period out of 24,014 cases. The material is divided into two groups: 1. deceased with specific (tbc) meningitis and/or meningoencephalitis. 2. Deceased with nonspecific inflammatory diseases. Such grouping is justified by the difference, which is required in the therapy of these two groups of diseases. Deceased with specific inflammatory diseases--5.42% against 3.44% (with nonspecific) predominate during the first-year-plan (1950-1954). The deceased from the first group reduce sharply reaching 0.12% during the third and the following five-year-plans. The percentage of deceased from the second group show greater stability--2.82--2.14%. In this group there is a tendency to a reduction of purulent inflammatory diseases and an increase of serous (nonspecific) diseases. The relative number of secondary nonspecific inflammatory diseases, among which dominate otogenic, increase--47.1% due to a reduction in the number of deceased with primary nonspecific inflammatory diseases.(ABSTRACT TRUNCATED AT 250 WORDS)

[Epidemiological studies of the effect of organophosphate pesticides on health].

The subject of study are agricultural workers applying organophosphorus and other pesticides in an area with intensive agriculture and a control group from the same area having no professional contact with pesticides. The studies include: personal and professional anamnesis, a battery of neurobehavioral tests, electroneuromyography, evoked potentials, neurological status, internal status, hematological tests, liver enzymes, variability of the heart rhythm. The level of the exposure is assessed by: measuring the concentration of PhO compounds in the air of the work environment and deposit on the skin. The biological monitoring is carried out by measuring the level of the cholinesterase activity in serum and erythrocytes, neuropathy target esterase in lymphocytes and alkylphosphates in urine. The tests are performed before and after the active season of spraying. The results from the studies show that at continuous exposure to low concentrations of organophosphorus compounds, applicated in agriculture, no significant health disorders occur. Specific deviations are observed after the season of spraying in the neuro-behavioural tests as increasing the number of incorrectly put dots in the Aiming test, the time of reaction and evoked potentials. The interpretation of the results is impeded because of the large number of interfering factors and considerable quantity of alcohol.

[New toxicokinetic exposure tests for metals based on atomic absorption analysis of the nails].

The up-to-date methodology development for analysis of microelements permits the determination of many of them in small nail samples. The nails of the feet are chosen as biological bioptic material offering some advantages: stability in preserving; higher levels of most toxic elements (in comparison with the levels in serum, blood and urine); a lower rate of surface pollution (in comparison with the hair and nails of the hands); receiving information on the average level of exposure for a long period of time. For the purpose of using nails from the feet in biological control are developed direct AAC - procedures for defining As, A1, Cd, Co, Cr, Cu, Fe, Mn, Ni, Pb, Se, Zn, Mo, Sb and Sn on the basis setting the samples in solution with tetra-alkylammonium hydroxide. The levels of these elements are determined in unexposed persons. The analysis of nails from feet of workers exposed to Pb, Cr and Mn proves, statistically, that the level of these metals gives valuable information on the rate of exposure (the quantity entered into the organism) of groups and individuals. This analysis permits longitudinal control on Pb, Cr and Mn - status of the organism by painless sampling, accessible and reliable analytical procedure.
