Severe eye injury in an 11-year-old child during threshing--A case report.

A case is reported of severe eye injury in an 11-year-old boy which occurred during threshing in the parents' farmyard. A detailed description of surgical treatment, complications and 7-year follow-up is presented. Despite long lasting treatment and new surgical methods used, the eye became blind. On the basis of our experience we can conclude that adequate adult supervision of children is mandatory during work in farmyards, and this is probably the only way to avoid at least a part of severe injuries which cause great social and economic losses.

[Efficacy of intravitreal triamcinolone acetonide in the visualization of the vitreous during pars plana vitrectomy].

PURPOSE: To retrospectively evaluate the efficacy of triamcinolone acetonide (TA) injection during pars plana vitrectomy, to facilitate the visualization of the internal limiting membrane (ILM), residual vitreous cortex (RVC) and preretinal membranes. MATERIAL AND METHODS: Pars plana vitrectomy was performed in 164 patients due to rhematogenous retinal detachment, vitreous hemorrhage, macular hole, lens luxation, endophthalmitis, intraocular foreign body, malignant glaucoma, preretinal membrane and proliferative diabetic retinopathy. After surgical separation of the posterior vitreous and removal of any visible epiretinal membrane, TA suspension was injected over the posterior pole into the vitreous cavity. Then, visualized RVC was removed and ILM peeling was performed. RESULTS: Upon intravitreal injection of TA, vitreous and ILM could be visualized by numerous particles of TA dispersed as white specks. RVC and ILM were completely removed in all patients. No complications related to the use of TA were encountered, even after complex procedures such as, vitrectomy combined with scleral buckling or phakoemulsification surgery in a long term follow up. CONCLUSIONS: Intraoperative visualization of RVC and ILM with intravitreal TA was found to be a useful adjunct to pars plana vitrectomy. This technique may facilitate both removal of epiretinal membrane and separation of vitreous, especially in patients with undetached vitreous.

Work-related, penetrating eye injuries in rural environments.

PURPOSE: To establish the aetiology and visual outcome after penetrating eye injures in rural environments. MATERIALS AND METHODS: The records of 182 patients (184 eyes) with penetrating eye injuries treated in the 1 st Eye Hospital, Lublin, Poland, between 1994 and 2002, were reviewed. Twenty eight of the 184 eyes (15.6 %) were work-related agriculture penetrating eye injuries. Distribution by age, sex, season variation, cause of injury, place of entrance wound, visual acuity and late complications were estimated. RESULTS: Of the group of 28 patients, 24 (85.7 %) were male and 4 (14.3 %) female, with the age range between 11-76 (mean 48.2) years. Most injuries were a result of repair and maintenance work in 35.7 %, wood chopping in 25 %, machine use in 17.9 %, simple instruments use in 10.7 %, fall from one level to another in 7.1 %, and cow butting with a horn in 3.6 %. Eighteen eyes (64 %) were blind with visual acuity less than 0.05 at their most recent review. CONCLUSIONS: Our study has shown that perforating ocular injuries in rural environment are still a big therapeutic, social and economic problem.

[Corneal endothelial density in glaucoma patients].

PURPOSE: To asses mean endothelial cell density in glaucoma patients. MATERIAL AND METHODS: Examinations were conducted and registered using the Noncon Robo (Konan) non-contact specular microscope between years 2000 and 2004, in Tadeusz Krwawicz Chair of Ophthalmology and 1st Eye Hospital, Medical University of Lublin. The results of 159 patients (233 eyes) with glaucoma and 37 patients (44 eyes) with pseudoexfoliation syndrome were analyzed. Patients, 22-91 years old, were divided into 8 groups according to the clinical diagnosis and for each group an adequate control group was matched. Statistical analysis was carried out to compare the examined and control groups using the non-parametric U Mann-Whitney test. RESULTS: Corneal endothelial cell density were significantly lower in all groups of glaucoma patients. There was a significant reduction in cell count in patients with chronic angle-closure glaucoma: 2333 +/- 476/mm2, control group (C): 2809 +/- 378/mm2, p< 0.001), acute angle-closure glaucoma 2136 +/- 620/mm2, C. 2780 +/- 384/mm2, p< 0.001, pigmentary glaucoma 2537 +/- 355/mm2, C. 3003 +/- 335/mm2, p< 0.05, juvenile glaucoma 2337 +/- 277/mm2, C. 3001 +/- 168/mm2, p< 0.001, primary open angle glaucoma 2326 +/- 231/mm2, C. 2779 +/- 398/mm2, p< 0.001, normal tension glaucoma 2343 +/- 394/mm2, C. 2732 +/- 356/mm2, p< 0.01, capsular glaucoma 2128 +/- 483/mm2, C. 2753 +/- 354/mm2, p< 0.001 and in patients with pseudoexfoliation syndrome 2255 +/- 299/mm2, C. 2721 +/- 352/mm2, p< 0.001. CONCLUSIONS: Patients with glaucoma have lower mean corneal endothelial cell density than control groups. The density of corneal endothelial cells depends on the type of glaucoma.

[Fundus autofluorescence examination using a confocal scanning laser ophthalmoscope HRA (Heidelberg Retina Angiograph)].

PURPOSE: To present application of a confocal Scanning Laser Ophthalmoscope (cSLO) for fundus autofluorescence examination, as a new method of visualization of retinal pigment epithelium and its possible significance in the diagnosis of different retinal diseases. MATERIAL AND METHODS: Typical autofluorescence images in age-related macular degeneration (AMD), Stargardt disease, Best disease and pattern dystrophies are presented, based on the own experience and literature data. Autofluorescence images were obtained with a cSLO using an argon laser for generation of excitation light at 488 nm and a barrier filter >500 nm for the detection of the emitted signals. RESULTS: A variety of autofluorescence patterns, associated with the accumulation of lipofuscin in RPE cells, was found in the above entities. CONCLUSIONS: Presented method of fundus autofluorescence examination gives new possibilities in studying the pathogenetic mechanisms in various retinal diseases and may be useful in monitoring the follow-up and the effects of the treatment.

[Quantification of isopters using semiautomated kinetic perimetry (SKP) in glaucoma patients with advanced retinal nerve fiber layer (RNFL) loss].

PURPOSE: To quantify the area of isopters obtained using a new technique of kinetic visual field examination--semi-automated kinetic perimetry (SKP)--in case of advanced retinal nerve fiber layer (RNFL) loss, caused by glaucoma. MATERIAL AND METHODS: Forty-one subjects (19 men, 22 women; mean age 66.5 +/- 12.5 years) suffering from open-angle glaucoma (30 patients), normal tension glaucoma (5 patients), juvenile glaucoma (2 patients) and PEX glaucoma (4 patients) were examined, using SKP program implemented in Octopus 101 instrument (Haag-Streit, Koeniz, Switzerland). One eye of each patient was tested using three stimuli according to the Goldmann classification. The stimuli III4e and I4e were obligatory. Additionally one stimulus (I3e or I2e) was used depending on the character of the defect. The stimulus angular velocity was kept constant at the level of 3 degrees/s. Obtained visual field (VF) results were classified according to the Aulhorn classification into stage III (26 VF) and IV (15 VF). The areas of isopters were measured in deg2. RESULTS: The mean area of isopter III4e was 9860 deg2, I4e--5171 deg2, I3e--1093 deg2, I2e--1093 deg2 in the group of Aulhorn stage III. In the group of Aulhorn stage IV the mean area of isopter III4e was 7488 deg2, I4e--3736 deg2, I3e--1109 deg2 and I2e--818 deg2. The mean test time was 16 min. (range 9-25 min.), 15.7 min. in the group of Aulhorn stage III and 16.5 min. in the group of Aulhorn stage IV, respectively. CONCLUSION: SKP seems to be an effective method for quantitative assessment and evaluation of isopter and scotoma areas (in deg2). In this study it was demonstrated that in case of more advanced stages of glaucomatous visual field loss (Aulhorn stage IV), the areas of isopters appeared to be smaller.

[Comparison of SKP (semi-automated kinetic perimetry) and SASP (suprathreshold automated static perimetry) techniques in patients with advanced glaucoma].

PURPOSE: To compare the results of conventional suprathresold automated static perimetry (SASP) and a new method of kinetic visual field examination (semi-automated kinetic perimetry--SKP) in patients with advanced retinal nerve fibre layer (RNFL) defects, caused by glaucoma. MATERIAL AND METHODS: Nineteen patients (10 women and 9 men) suffering from advanced open-angle glaucoma (17 patients) and low tension glaucoma (2 patients) were enrolled in this study. The mean age was 65.3 years (range 49.0-82.9 years). One eye of each subject was examined with SKP and SASP, both implemented in Octopus 101 perimeter (Haag-Streit, Koeniz, Switzerland) on the same day. The examiner using SKP did not know the SASP results. The static visual field examination was performed first. In order to compare visual fields results obtained with both methods, only central 30 degree area of the visual fields was taken into consideration. The area obtained by creating a virtual isopter (option of SKP software) located between the position of defective and intact visual field regions of SASP and the area of I4e isopter obtained with SKP (stimulus angular velocity 3 degrees/s) were measured in deg2 and compared. RESULTS: The mean area (+/-SD) of I4e isopter obtained with SKP was 1882 deg2 (+/- 510 deg2) and the mean area of SASP 30 degree visual field was 1737 deg2 (+/- 546 deg2). This difference was not statistically significant. CONCLUSION: Quantitative comparison of static and kinetic perimetry is a difficult task to perform. The SKP software offers a possibility of direct quantitative assessment of the area of each isopters in deg2. This study shows that the results of visual field examinations obtained with both methods in this group do not differ significantly between SASP and SKP.

[Peroxidation of lipids in patients with senile cataract].

PURPOSE: The aim of the study was to elucidate a potential relationship between systemic and intraocular (aqueous humor and lens) levels of lipid peroxidation products (LPO) during development of senile cataract. MATERIALS AND METHODS: Population examined included 59 females and 44 males (average age 67.1 +/- 10.78), who underwent surgical cataract extraction. They were divided into 3 groups based on cataract stage. Conjugated diens (CD) and malonyl dialdehyde (MDA) levels in lens nuclei, aqueous humor and blood sera, were assessed. RESULTS: Our results showed significant increase of CD and MDA levels in lens nuclei and aqueous humor with clinical advancement of cataract accompanied by increases of serum CD and MDA levels. Significant, positive correlations between analyzed parameters were noticed. CONCLUSIONS: This indicates that systemic lipid peroxidation may affect local advancement of the disease within the lens through the influence on intraocular levels of LPO.

[Evaluation of antimicrobial properties of silicon oil--in vitro studies].

PURPOSE: The aim of the study was to evaluate antimicrobial properties of silicon oil in vitro against Staphylococcus aureus, Pseudomonas aeruginosa, Candida albicans, which are considered the major causative agents of endophthalmitis. MATERIAL AND METHODS: The clinical isolates of representative microorganisms (S. aureus, P. aeruginosa, C. albicans) were selected. The bacteria and the fungus were separately inoculated in PDMS 5000 (produced by AcriMed, Germany). Control inoculations in physiological saline and sugar bouillon were performed. The samples of 0.01 ml from each medium were diluted, according to serial dilution procedure and inoculate on Petri plate dishes 5% sheep blood agar for bacteria and Sabouraud medium for Candida albicans. After 24 h incubations for bacteria and 48 h incubations for fungus, CFUs were counted. RESULTS: All the microorganisms revealed an apparent decrease in CFUs in PDMS 5000. The total elimination was observed for S. aureus after 5 days. For P. aerugisosa solitary colonies (less than 25 CFUs) were observed up to 7 days. After 7 days of incubation no growth of P. aeruginosa was observed. High C. albicans CFU values were counted up to 3 day of the incubation. After 5 days single fungal colonies were observed. CFUs of the examined microorganisms declined slightly in physiologic saline. A growth pattern similar to the growth curve of microorganisms was observed in sugar boullion. CONCLUSIONS: Our study indicates that silicon oil could have an antimicrobial activity against Staphylococcus aureus, Pseudomonas aeruginosa, Candida albicans, which are considered the major causative agents of postoperative endophthalmitis.

[Penetrating eye trauma epidemiology in own material].

PURPOSE: Epidemiological estimation of penetrating eye injuries treated at the I-st Eye Hospital Medical Academy in Lublin in 1994 - 2002. MATERIAL AND METHODS: Distribution by age, sex, season, causes of the trauma, visual acuity were estimated. There were treated 182 patients (184 eyes). RESULTS: Out of the 182 patients the majority 145 were men and 37 women. The age distribution: 0 - 15 years (39 patients), 16 - 25 years (23 patients), 26 - 35 years (37 patients), 36 - 45 years (27 patients), 46 - 55 years (17 patients), 56 - 65 years (25 patients), over 65 years (14 patients). Mean age - 33.8 years (1 year to 80 years). The distribution of penetrating eye injuries was as follows: in 1994 - 19 cases, 1995 - 31 cases, 1996 - 20 cases, 1997 - 17 cases, 1998 - 17 cases, 1999 - 16 cases, 2000 - 18 cases, 2001 - 18 cases, 2002 - 25 cases of ocular trauma. In 75 cases we noted the connection with occupation, and in 107 cases there were no such correlation. Final visual acuity was less than 0.1 in 72 patients (39.1%), 0.1-0.3 in 34 patients (16.5%) and over 0.3 in 78 patients (42.4%). CONCLUSIONS: Penetrating eye injuries are still severe therapeutical problem and cause of 40% loss of useful visual acuity among treated patients. During 9 years of this study the decreasing of number of penetrating injuries was not observed, which showing again a problem of the prevention.

[Applicability of semi-automated kinetic perimetry (SKP) in the assessment of the visual field loss due to retinitis pigmentosa].

PURPOSE: To assess the applicability of a new technique of kinetic visual field examination--semi-automated kinetic perimetry (SKP)--in patients suffering from the visual field defects due to retinitis pigmentosa. METHODS: Thirty-five patients (19 women, 16 men: mean age 38.4 years) suffering from retinitis pigmentosa (10 with with Usher syndrome, one with Bardet-Biedl syndrome and one with Refsum syndrome) were examined using SKP software, implemented in Octopus 101 instrument (Haag-Streit, Koeniz, Switzerland). Three stimuli (1114e and 14e obligatory) were used to assess the hill of vision of each patient. The area of each isopter was measured in deg2. The test time was measured automatically in minutes. RESULTS: The visual field results were classified as complete or incomplete midperipheral "ring scotoma" (group I--13 patients) and concentric loss of the visual field (group II--22 patients). The area of 1114e isopter was 6147.5 deg2 in the group I and 1961.7 deg2 in the group II. The area of 14e isopter was 1468.4 deg2 and 781.7 deg2, respectively. The mean test time was 10 min. (range 4-20 min.): 8 min. in the group I and 13 min. in the group II. CONCLUSIONS: There is a large diversity of patterns of the visual field loss in retinitis pigmentosa. SKP, in contrary to widely used manual perimeter, gives a possibility of quantification of the visual field area. Future studies may be able to monitor the progression of the visual field loss caused by retinitis pigmentosa.

[Recent developments in kinetic perimetry].

The manual kinetic perimetry using Goldmann instrument is still widely used and remains a "gold standard" in examining patients with any kind of advanced visual field loss and impaired co-operability. Nevertheless, Goldmann instrument has several disadvantages, such as examiner dependence, response variability, lack of standardization of parameters, poor repeatability. The latest development in kinetic perimetry, which was realized at the University Eye Hospital in Tuebingen (Germany) is semi-automated kinetic perimetry (SKP), using Octopus 101 instrument (Interzeag/Haag-Streit, Koeniz-Bern, Switzerland). It is an examiner-independent, standardized and reproducible method. Additionally, obtained results can be corrected for the mean individual reaction time. This paper will briefly present characteristic, advantages and application of SKP in clinical practice.

Lipid peroxidation in diabetic retinopathy.

Oxidative stress has been implicated in pathogenesis of diabetic retinopathy. It has been hypothesized that hyperglycaemia may damage vascular endothelium and retina by inducing the synthesis of oxidant reactive species. The aim of the present study was to estimate lipid peroxidation by detecting the concentration of malondialdehyde and 4-hydroxynonenal in diabetic patients with retinopathy, subjects without retinopathy and the reference group. Sixty-one patients were studied, including 30 patients with severe non-proliferative retinopathy in NIDDM and 31 diabetic patients without retinopathy. The reference group were 11 systemic healthy patients. Concentration of malondialdehyde and 4-hydroxynonenal was measured in plasma sample using a Lipid peroxidation Assay Kit (Calbiochem-Novabiochem Corp.). The concentration of lipid peroxidation products in patients with retinopathy was statistically significantly elevated in comparison to diabetic patients without retinopathy (p < 0.001) and the reference group (p < 0.001). We do not notice any significant differences in levels of MDA and 4-HNE between patients without diabetic retinopathy and the reference group. In view of our results we can conclude that oxidative stress is an important risk factor in the development of diabetic retinopathy.

[Analysis of 24 viral keratitis cases treated by corneal graft from 1998-2000].

PURPOSE: To determine the clinical character of patients with keratoplasty after viral keratitis and comparison of pre- and postoperative visual acuity. MATERIAL AND METHODS: The study involved 24 patients treated using corneal graft after viral keratitis complications. Sex, age, permanent residence, duration of disease, previous pharmacological treatment, indications to corneal graft and postoperative functional results were evaluated. RESULTS: In 24 cases 16 (66.7%) were males in age 38-75 years old. Eight (33.3%) of the 24 cases were females in age 12-78 years old. Visual acuity improvement after corneal graft was observed in 23 patients. There was no visual acuity improvement in one case with considerable degree of optic nerve lesion. CONCLUSIONS: The results of these examinations showed, that keratoplasty after viral keratitis complications were more frequent in males, especially in 40-59 age range, professionally engaged. Statistical analysis of these cases showed good prognosis of visual acuity improvement after surgical treatment.

Acute allergy reaction after posterior sub-Tenon's triamcinolone injection in the treatment of intermediate uveitis in the asthmatic patient.

We report the case of a 21-year-old female patient afflicted with atopic asthma admitted to hospital in order to diagnose and treat bilateral uveitis. After diagnostic examination: serological tests for Candida and Aspergillus antigens, analysis of direct vitreous preparation and of culture searching for fungal and bacterial etiology, tests for antinuclear antibodies and for boreliosis, the diagnosis of idiopathic intermediate uveitis (pars planitis) were made. Routine treatment with Polcortolon in sub-Tenon's triamcinolone injection was applied. It resulted in acute allergic reaction characterized by blepharedema and chemosis. Hydrocortisone, Clemastin, Zyrtec, Calcium and locally Dexamethasone and Emadine in drops instilled to conjunctival sac were administered resulting in symptom disappearance.

[Vitrectomy in treatment of idiopathic and secondary pre-retinal fibrosis].

PURPOSE: To present the results of vitrectomy in the management of preretinal fibrosis. MATERIAL AND METHODS: 45 patients with preretinal fibrosis treated with vitrectomy were analysed. 33 of them were treated from 1997 to 1998 in Ophthalmology Dept. University of Heidelberg (Germany) and 12 were treated from 1998 to 1999 in the 1st Eye Hospital Lublin University School of Medicine. The reason for preretinal fibrosis: idiopathic-22 patients, retinal detachment--16, retinal vein occlusion--2, trauma--2, uveitis-1, perivasculitis--1, alopecia--1. RESULTS: Due to applied treatment 27 patients (60%) showed visual improvement, 13 patients (29%) had the same visual acuity before and after vitrectomy and 5 patients (11%) noticed a decrease of visual acuity after the treatment. CONCLUSIONS: Vitrectomy is an effective method of treatment of patients with preretinal fibrosis. In cases with no improvement of visual acuity after surgery, vitrectomy could reduce a metamorphopsia.

[Ophthalmic manifestations in Lyme borreliosis].

We reviewed ophthalmic manifestations in Lyme borreliosis, concentrating on clinical and laboratory diagnosis, differential diagnosis and treatment options. Ocular involvement may occur in every stage of the disease. Conjunctivitis and episcleritis are the most frequent manifestations of the early stage. Neuro-ophthalmic disorders and uveitis occur in the second stage whereas keratitis, chronic intraocular inflammation and orbital myositis have been reported in the third stage of borreliosis.
