[Latex allergy].

An avalanche-like increase of the cases with LA during the last decades is being noticed. The pathogenesis of LA is considered, the main latex allergens are characterized and their sources are presented in details. The risk factors and risk groups for the occurrence of LA are described, as well as the clinical presentation of the disease in its different expressions. The diagnostic methods for confirmation of LA are enumerated and evaluated. The significance of prevention is emphasized.

[The first impressions of the therapeutic effect of mequitazine (Primalan) in allergic patients].

The therapeutic efficacy of Mequitasin (Primalin) was studied in 31 allergic patients--26 patients with allergic rhinitis and 5 patients with skin rashes (4 patients with acute urticaria and 1 patients with atopic dermatitis). The patients were given 1 tablet Primalin twice daily in the course of 14 days. In the patients with allergic rhinitis the effect of the treatment was considered very good in 42.41% of the patients treated, good--in 50% and lack of effect--in 7.69% of the patients. In the patients with urticaria and atopic dermatitis the results were very good in 60%, good--in 20% and no effect in 20% of the patients treated. Side effects were found in 5 patients: in 3 patients--mild up to moderate drowsiness, in 1 patient--weakness and 1 patient--dryness in the nose.

[Androstenedione, DHEA sulfate, cortisol, aldosterone and testosterone in bronchial asthma patients].

The basic level and the 24 h rhythm of secretion of cortisol, aldosterone, androstanedion, DHEA-sulfate and testosterone were determined in 26 men with bronchial asthma, 19 up to 50 years of age, in a free from attacks period. 20 of the patients had never been treated with corticosteroids and 6 patients had used corticosteroid drugs. In 26.32% of the patients who had not been treated with corticosteroids the basic cortisole level was either decreased or on the lower normal limit. At the same time in 55% of these patients a different degree increase of the basic DHEA-sulfate level was found and in 30% of the patients the androstanedione basic level was increased. The changes in the 24 h rhythm of androstanedione and DHEA-sulfate secretion were similar to those of cortisol. Normal rhythm of secretion of cortisol, aldosterone and testosterone was established mainly in patients with preserved basic hormonal secretion. In contrast to these, normal rhythm of androstanedione and DHEA-sulfate secretion was found more frequently in patients with increased basic secretion of these hormones.

[The therapeutic effect of somofilin in bronchial asthma patients].

The sustained release theophylline drug Somophylline in capsules was applied in 21 asthmatic patients (12 women and 9 men), mean age 44.5 years in the course of 14 days in hospital conditions. The dose of the drug was 9-10 mg/kg body mass (24 h given in two equal doses in 12 h intervals). The effect was favourable in 17 patients (80.95%), weak--in 2 patients (9.52%) and no effect in 2 patients (9.52%). Somophylline is very well tolerated. The blood level remains in the therapeutic ranges during the treatment (about 10 micrograms/ml).

[The serum testosterone level of patients with bronchial asthma treated with corticosteroids and untreated].

The testosterone basic level was followed up in 427 asthmatic male patients treated and not treated with corticosteroids. In 172 of them (40.68%) basic low blood testosterone was found, in 1/3 of the patients (32.32%) the morning testosterone level was below 200 ng% and in 37 patients (8.67%) it was below 100 ng%. Low blood testosterone was more frequent in the corticosteroid-treated patients than in the patients who had never been treated with corticosteroids (31.63%). Among the untreated patients low blood testosterone was more frequent in the patients with nonatopic (40.82%) than in the patients with atopic (24.67%) and mixed type (27.16%) of bronchial asthma. Low blood testosterone was found mainly in the patients with severe (37.76%) and moderate (40.00%) form of the disease and very rarely in patients with mild form of bronchial asthma (8.51%). The basic testosterone level changes are probably due to the stress, hypoxia and corticosteroid treatment. The possibility of a direct suppressive action of exo- and endoallergens on the testes are discussed.

[Changes in hypophyseal-adrenal secretion in bronchial asthma patients].

The basic ACTH and cortisol (hydrocortisone) secretion was followed up in 678 patients suffering from bronchial asthma--494 men and 184 women, duration of the disease from 6 months up to 27 years. In 140 patients the 24 hour secretion of both hormones was studied. In 1/3 of the asthmatic patients the cortisol secretion was lower than the normal one (20.94%) or was near the lower normal range (14.01%). In about half of the patients the morning (at 8 o'clock) ACTH secretion was increased (53.10%). The disturbance of the basic hypophysial-suprarenal secretion was found more often in the patients with infectious bronchial asthma (24.9%) and with the mixed form than with the atopic form of the disease (12.0%). It probably takes part in the pathogenesis of infectious bronchial asthma. The corticosteroid treatment plays a leading role in reducing the basic cortisol secretion in asthmatic patients. In 53.85% of the asthmatic patients a disturbed 24 hour rhythm of cortisol secretion was found--inverse (30.71%) or monotone (27.14%). The rhythm disturbance was found more often in the patients with basic hypocortisolemia.

[Plasma theophylline level in patients with bronchial asthma treated with novphyllin].

The authors followed up the plasma theophylline level in 51 patients with bronchial asthma treated with novphyllin. Plasma theophylline level was determined (according to Jatlow--1975) before novphyllin intake and by 1, 2, 4 and 8 hour after it. The investigation was repeated in 18 patients on 4 th day after the systematic novphyllin treatment. The study revealed that plasma theophylline concentration gradually increased till the second hour after the intake of the preparation in all patients, whereafter it started to decrease. By 60 min since the beginning of the experiment the therapeutic plasma theophylline level (from 10 to 20 and over 20 mg/ml) was found in 31.25 per cent of the patients, by the second hour--in 50.99 per cent, by the fourth hour--in 28 per cent and by the eighth--only in 12.19 per cent. After a three-day novphyllin treatment, under the same investigation conditions, it was established that the therapeutic plasma theophylline level was observed twice more often by the fourth hour (44.44%). In patients, administered nophyllin and antibiotics in combination (tetraolean and rhondomycin), the plasma theophylline level was faster increased and was longer maintained within the therapeutic limits. The authors underlined that the therapeutic effect of the preparation begins after reaching a plasma level of 5 mg/ml, but has its maximum effect at a plasma concentration over 10 mg/ml.

[Clinical observation of the inthal treatment of bronchial asthma].

The results are reported from the long-term clinical observation on inthal effect in the treatment of 57 patients with various forms of bronchial asthma--atopic (18 patients), infectious--allergic (14 patients), mixed (25 patients). The most pronounced clinical improvement was observed in the patients with atopic bronchial asthma: 66.63 per cent with excellent effect, 11.11 per cent with good effect, 5.56 per cent with poor effect and 16.63 per cent--no effect. A good effect on the clinical symptomatics was observed also in the patients with mixed bronchial asthma: 40 per cent--with excellent effect, 28 per cent--good effect, 12--poor and 20 per cent--no change. The effect of the drug in patients with infectious-allergic bronchial asthma was rather unpretending: 28.58 per cent--excellent, 14.29 per cent--good, 12 per cent--poor and 25.71 per cent--no effect. Corticosteroid dependence was recorded in 46.42 per cent of the patients. The effect of the drug was discussed in connection with bronchial asthma type, graviy of the course and concomitant treatment.

[Hypersensitivity to acetysal in bronchial asthma].

The incidence and clinical characteristics of hypersensitivity to acetysal were followed up in 529 patients with bronchial asthma. Manifestation of acetysal hypersensitivity was found in 46 patients (8.69%). It was manifested with bronchospasm in 78.25 per cent of the cases, in 23.91 per cent it was accompanied by skin alterations. Individuaal skin alterations were found in 21.73 per cent of the asthmatic patients with acetysal hypersensitivity. No allergic shock manifestations were observed. In 97.76 per cent of the patients with bronchial asthma and hypersensitivity to acetysal, hypersensitivity to some other medicaments was also found: analgin, analgesics of phenylbutazolon group (butadion, rheopyrin), indomethazin, antibiotics.

[Changes in the activity of histamine-liberating enzymes in acute experimental anaphylaxis].

The authors examined the changes in the histamine level, DAO (diaminoxidase) activity and N--MT (N-methyltransferase) activity in blood (plasma) and tissues of lung and liver (N--MT was examined only in the indicated tissues) in 105 guinea pigs, 45 of whom were healthy and 60 sensibilised and examined during the anaphylactic shock. It was established that during the acute anaphylactic shock there was demonstrative hyperhistanemia and hyperhistaminasemia, which were not observed during experimental anaphylactic shock with subacute course. The activity of lung and liver N--MT in animals with acute and subacute anaphylactic shock did not differ from that of the healthy guinea pigs. The complete inhibition of DAO activity in healthy animals and those with anaphylactic shock did not induce change in the activity of N--MT as well. The latter did not alter after sharp or high level or moderate elevation of the tissue histamine for several days. This histamine was a specific substrate for this enzyme. N--MT is an inductive enzyme, which apparently does not participate in histamine metabolism during acute and subacute anaphylaxis in guinea pigs. For its change, probably, is necessary longer time.
