[Comparison of restoring and maintaining the cervical curvature and height by using three different anterior cervical plate systems].

OBJECTIVE: To investigate and compare the effects of restoring and maintaining the cervical curvature and height of the fused segment by using three different anterior cervical plate systems. METHODS: From January 2002 to June 2004, 122 patients underwent anterior cervical decompression, autogenous iliac bone graft and plate fixation. Of the 122 patients (85 males, 37 females, aged 14-70), 37 underwent surgery involving the fixation with the Orion plate system, 39 with the Zephir plate system, and 46 with the Codman plate system. The cervical curvature and height of the fused segment were measured on the lateral X-ray films so as to compare the changes of the conditions preoperatively, 1 week after surgery, and during the follow-up, and also to compare the difference among the three groups. RESULTS: The follow-up of the patients for 6-35 months (average 17.3 months) showed that all the patients developed the bone fusion 6 months after operation. There was a significant improvement in the cervical curvature and height of the fused segment before operation versus 1 week after operation (P < 0.05); however, there were no significant changes 1 week after operation versus during the follow-up in each group(P > 0.05); there was no significant difference among the three groups (P > 0.05). CONCLUSION: The three plate systems can effectively reconstruct and maintain the cervical curvature and height of the fused segment, with a satisfactory effect in a short term.

[Operation on upper cervical instability without injury].

OBJECTIVE: To study operative methods of treating upper cervical spine instability without injury. METHODS: Twenty-three cases were treated by internal fixation with autologous bone grafts. Atlantoaxial arthrodesis were performed in 10 cases with interlaminar clamp (5 cases), Atlas cable system (3 cases) and Brooks (2 cases). Occipitocervical fusion were performed in the other 13 cases by using of CD-cervical (3 cases), Cervifix (8 cases) and U-stick fixation (2 cases). RESULTS: All the 23 cases were followed up for 2. 5 years in average (ranged from 6 months to 5 years). Solid arthrodesis was obtained in all 23 cases. Six months after operation, of the 20 cases with pre-operation nervous lesion, improvement was achieved in 16 cases. According to JOA standard and Hirabashi formula, the rate of improvement was 27.1%. CONCLUSION: Posterior fusion is recommended for upper cervical instability.

[Treatment of ischemic necrosis of the femoral head by transplantation of pedicled bone flap with deep iliac circumflex vessel].

OBJECTIVE: To investigate the treatment method of ischemic necrosis of the femoral head by the transplantation of pedicled bone flap with deep iliac circumflex vessel in adult and to assess its curative effect. METHODS: From February 1996 to September 2003, 46 adult patients with ischemic necrosis of the femoral head (58 coxas) were treated by transplantation of pedicled bone flap with deep iliac circumflex vessel. The locations were the left side in 34 coxas and the right side in 24 coxas(stage II in 16 coxas, stage III in 39 coxas and stage IV in 3 coxas). RESULTS: Thirty-seven patients (40 coxas) were followed up 6 months to 10 years after operation. According to the assess of FanQishen, the result was excellent in 19 cases (21 coxas), good in 11 cases (12 coxas), fair in 5 cases (5 coxas) and poor in 2 cases (2 coxas). The postoperative excellent and good rate was 81%. CONCLUSION: Operative treatment of ischemic necrosis of the femoral head in adult by transplantation of bone flap pedicled with deep iliac circumflex vessel has the characteristics of restoration of blood supply of the femoral head, decompression of marrow cavity, elimination of necrotic bone and support of the femoral head. It is an effective and advanced method for treatment of ischemic necrosis of the femoral head in adult.

[Simultaneous bilateral total hip arthroplasty in a single procedure].

OBJECTIVE: To study the effect of simultaneous bilateral total hip arthroplasty in a single procedure. METHODS: From October 1999 to March 2004, 15 patients (30 hips) underwent simultaneous sequential bilateral total hip arthroplasty (THAs) in a single procedure. Of the 15 patients, 11 were male (22 hips) and 4 were female (8 hips). Their ages ranged from 35 to 70 years. Their courses of disease ranged from 1 year to 50 years (4.8 years on average). The Harris scores of the joint function before the operation ranged from 12 to 45 points (27 points on average). Five were done with Smith-Peterson and 10 were done with Moore. RESULTS: The operative time was 3 hours and 25 minutes to 5 hours (4 hours and 10 minutes on average). The volume of blood transfusion during operation was 400 to 2 400 ml (1160 ml on average). All the 15 patients were followed up for 3 to 35 months (18 months on average). The Harris scores of the joint function after the operation ranged from 70 to 100 points (86 points on average). There was significant difference in the scores between before and after operations (P<0.05). There was only 1 death within 1 months of the operation and no serious between complications such as infection, pulmonary embolism, and deep vein plug. All the patients were still ambulant in the community and gained significant pain relief. CONCLUSION: Simultaneous bilateral total hip arthroplasty in a single procedure is a safe and effective method. However, the decision of performing single-stage bilateral total hip arthroplasty should be carefully made and preoperative preparation should be sufficiently made.
