[Managements of masked mastoiditis].

OBJECTIVE: To appreciate clinically of masked mastoiditis to explored how to reduce the incidence and associated morbidity of otogenic complications. METHODS: Between January 1998 and February 2005, 11 cases of masked mastoiditis were collected retrospectively. Their clinical presentation, results of investigations, and response to treatment were reviewed. RESULTS: Of 11 cases, there were 8 cases with hearing impairment, 5 cases with otalgia, 4 cases with facial nerve palsy, one patient with postauricular subperiosteal abscess, one case with meningitis, and one with thrombosis of the lateral sinus. Computed tomography (CT) scan revealed blurring (haziness) of the mastoid air-cells. After admission, intravenous antibiotics were prescribed and antro-mastoidectomy or mastoidectomy was performed for eradication of infection source. The predominant finding at mastoidectomy was granulation tissue filling the mastoid cavity and antrum. A varying amount of pus and osteitis was found in the 5 cases. The granulations into the antrum were severe, obstructing the drainage into the attic and the middle ear. The mastoid tip cells were filled with granulation tissue which spared the antrum. The patients recovered excellently postoperative, without facial palsy, vertigo or other complications. CONCLUSIONS: To be a contemporary otologist, such severe complications of otologic diseases should not be overlooked. Appropriate intravenous antibiotics and adequate surgeries, as soon as possible, were recommended. Advanced CT scans of the temporal bone were necessary. Failure to identify associated concomitant pathology might result in treatment failure or persistent neurological deficit.

[Coblation tonsillectomy versus blunt dissection tonsillectomy].

OBJECTIVE: Compare coblation tonsillectomy with blunt dissection tonsillectomy to see if there is any difference in intraoperative bleeding, operating time, postoperative pain, tonsillar fossae healing and return to normal diet and activities. METHOD: A randomised control trial was carried out to compare coblation tonsillectomy with blunt dissection tonsillectomy in 56 patients. The same surgeon operated on them all, using Arthro Care EVac 70 Wand for coblation tonsillectomy, and routine instruments for blunt dissection tonsillectomy. RESULT: Coblation tonsillectomy was fast and resulted in little intraoperative bleeding. Significant reduction in postoperative pain was found in the patients with coblation tonsillectomy. Patients who had coblation tonsillectomy returned to their normal diet sooner than those who underwent blunt dissection tonsillectomy. Slower healing of the tonsillar fossae was found in the coblation group. One patient in the coblation group suffered from secondary haemorrhage. CONCLUSION: Coblation tonsillectomy is easy to perform with little intraoperative bleeding. It offers significant advantages in the postoperative period, with rapid return to a normal diet and reduction in postoperative pain. But it need more time for healing of the tonsillar fossae.

[Percutaneous dilational tracheostomy].

OBJECTIVE: To describe and introduce a new technique for percutaneous dilational tracheostomy without the aid of a bronchoscope. METHODS: Ten patients who underwent rotating dilation tracheostomy with percutwist set were prospectively studied. The time needed for the procedure, the grading of the difficulty, the amount of bleeding, and the complications for the procedure were evaluated. All procedures were performed without the aid of a bronchoscope. RESULTS: During the operations, blood pressure, ECG parameters were stable. One patient had short period of intraoperative oxygen desaturation. The mean operating time was 6.2 minutes with a range of 3 to 10 minutes. Eight procedures were performed without any difficulty, 2 procedures were performed with some difficulties which could be managed by the surgeon. Six patients had been examined under fibrobronchoscopy within one week postoperatively, no posterior tracheal wall injuries were found. One patient had peristomal bleeding after the operation, one patient had mild infection of the tracheostoma. There were no life threatening complications attributable to this technique. CONCLUSIONS: Controlled rotating dilation is a simple, rapid and safe bedside procedure. In the absence of bronchoscopic guidance, the procedure can be safely performed with precautions.

[Restricted microcirculation disorder induced by photochemical reaction in the guinea pig cochlea].

OBJECTIVE: To investigate the effect of restricted cochlear microcirculation disorder on the cochlear blood flow, auditory function, and morphology. METHOD: Photochemical reaction was utilized to induce localized microcirculation damage to the second cochlear turn. After systemic administration of rose bengal, the second cochlear turn was illuminated with green light (wave length 540 nm +/- 40 nm) to cause thrombosis in the stria vascularis, the area was about 1.2 x 1.0 mm2. RESULT: At the site of the illumination, cochlear blood flow gradually decreased, and the threshold of action potential gradually increased. Surface preparing showed thrombus was in the stria vascularis. Three hours after the photochemical reaction, some hair cells in the illuminated area necrotized. The length of the lesion was 1152.50 +/- 363.26 microns (n = 4). But hair cells in the non-illuminated area were normal. CONCLUSION: The photochemical method is a reliable tool to produce an animal model with the restricted microvessel lesion in the lateral wall of the cochlea. It could be used as an animal model for the treatment of the cochlear thrombosis.

[Intra-arterial thrombolysis for the treatment of restricted cochlear microcirculation disorder in guinea pigs].

OBJECTIVE: To ascertain a new treatment method for restricted cochlear microcirculation disorder. METHODS: Photochemical reaction was utilized to induce localized microcirculation damage to the second cochlear turn of every guinea pig. The animals were divided into 5 groups. Group 1 was blank control. Group 2 and Group 3 were injected urokinase (UK) through left subclavian artery or left external jugular vein 30 minutes after photochemical reaction. Group 4 and Group 5 were two control groups. Instead of urokinase, saline injection was applied accordingly. RESULTS: Both intra-arterial thrombolysis and intravenous thrombolysis were effective to improve the hearing levels and the blood supply to the inner ear. 50 minutes after urokinase injection, animals with intra-arterial thrombolysis showed a lower action potential threshold than that with intravenous thrombolysis (P = 0.025). And this phenomenon lasted 30 minutes. Cochlear blood flow of the animals with intra-arterial thrombolysis began to decrease and action potential began to increase 80-90 minutes after UK injection. CONCLUSION: Both intra-arterial thrombolysis and intravenous thrombolysis were effective to the restricted thrombosis in the stria vascularis, and intra-arterial thrombolysis showed a better improvement in the early stage of thrombolysis.

[A preliminary study on the immune suppressive protein of stress in human tonsil].

Our previous work demonstrated that under the conditions of restraint stress and under the control of central nervous system (CNS), an immune suppressive protein of stress (ISPS) was generated in peripheral lymph tissue and released into the blood stream, acting as an immune suppressor. In the present work, a protein similar to ISPS was found in human tonsil (a peripheral lymph tissue). Human tonsil was homogenized and the extract was prepared. It was found that lymphocyte proliferation was significantly suppressed by the extract. The suppression induced by the extract was partially reversed by the monoclonal antibody against ISPS (2C4). In ELISA test, the extract was able to bind to the monoclonal antibody. By immunohistochemistry, many ISPS positive cells were found in human tonsil. The ISPS positive cells were also found in human lymph nodes. Taken together, all the results demonstrate that a protein similar to ISPS may exist in human peripheral lymphoid tissue.

[Effects of nimodipine on cochlear blood flow with normal and vertebrobasilar insufficiency status].

OBJECTIVE: To study effect of nimodipine (ND) on cochlear blood flow (CBF). METHOD: CBF was measured in twenty-two guinea pigs by laser Doppler flowmetry(LDF). RESULT: Nimodipine decreased CBF following the decrease of systemic BP and present a dose dependent mode, neither the dose of nimodipine is lower, normal and higher, nor the animal is under the status of normal supply of inner ear blood flow and ischemia of inner ear caused by vertebrobasilar insufficiency(VBI). CONCLUSION: It suggested that this compound may be of little clinical use to elevate CBF.
