[The clinical study of stereopsis after bilateral intraocular lens implantation].

PURPOSE: To observe the stereopsis of senile cataract patients after bilateral intraocular lens (IOL) implantation. METHOD: Subjects were divided into two groups: IOL group and normal group. Near stereoacuity with distance and near correction was checked with Randot steretests respectively. Near vision acuity was recorded with Logmar. RESULT: Near stereoacuity with distance correction was (101 +/- 59)" and (112 +/- 55)" respectively, the responding near vision acuity was 0.51 +/- 0.15 and 0.52 +/- 0.17. Near stereoacuity with near correction was (33 +/- 11)" and (34 +/- l0)", the responding near vision acuity was 0.09 +/- 0.10 and 0.11 +/- 0.11. There was no significant difference in near stereoauity and near vision acuity between two groups (P > 0.05). Stereoauity and near vision acuity with near correction was significantly improved in both groups (P < 0.01). CONCLUSION: Stereopsis after bilateral intraocular lens implantation was improved.

Ultrasound biomicroscopy of iris-claw phakic intraocular lens implantation.

PURPOSE: To study in situ the intraocular position of iris-claw phakic intraocular lens (ICPIOL) in myopic eyes using ultrasound biomicroscopy (UBM). METHODS: UBM echograms of the anterior segment were taken preoperatively and 62 to 115 days postoperatively in 6 eyes implanted with the Verisys ICPIOL (AMO). The echograms were assessed for the effect of the ICPIOL on iris tissue. RESULTS: The preoperative distance between the corneal endothelium and the lens ranged from 2.96 to 3.09 mm, and the postoperative distance between the ICPIOL and the corneal endothelium, from 1.86 to 2.03 mm. The distance between the lens and the posterior surface of the ICPIOL ranged from 0.61 to 0.76 mm. The distance between the superior, inferior optic edge and the iris ranged from 0.49 to 1.00 mm, 0.21 to 0.51 mm respectively. The shortest distance between the ICPIOL haptics and the angle of anterior chamber ranged from 1.25 to 1.65 mm. The indentation of iris tissue by the ICPIOL haptics without pigmentary dispersion and distortion of posterior curvature of iris was observed. CONCLUSION: Adequate space is maintained between the Verisyse myopic ICPIOL and the corneal endothelium, angle, and crystalline lens. Haptic indentation of the iris without pigment erosion and distortion of iris curvature is noted. The ICPIOL implanted in phakic eyes is a safe alternative for treatment of high myopia.

Comparison of the inflammatory reaction to laser photolysis and conventional phacoemulsification.

PURPOSE: To compare the postoperative inflammatory reaction induced by laser photolysis and conventional phacoemulification. METHODS: Anterior chamber flare and cells were measured preoperatively and 1 day, 1 week, 1 month and 3 months postoperatively in two groups. In group 1, thirty-two eyes underwent laser photolysis. In group 2, thirty-five eyes underwent conventional phacoemulification. RESULTS: Anterior chamber flare and cell measurements had no significant difference between two groups preoperatively and 1 day, 1 week, 1 month and 3 months postoperatively. Flare and cell values returned to preoperative values by the third month. CONCLUSIONS: Postoperative inflammatory response to laser photolysis was equal to conventional phacoemulsification, and recovery occurred in 3 months postoperatively.

Quantitative measurements of the decentration distance of four kinds of intraocular lens.

PURPOSE: To investigate a quantitative method to measure decentration distance(DD) and analyze the DD of four kinds of intraocular lenses (IOLs). METHODS: The pseudophakic eye photos were taken under the slit lamp after the IOLs were implanted a year or longer. Then the images were inputted into a computer and the Photoshop software was applied to analyze the DD. Four kinds of IOLs were analyzed, including AcrySof MA60BM (11 eyes), Allergan SA40N (11 eyes), AcrySof SA60AT/SA30AL (5 eyes), Corneal Quattro (4 eyes). RESULTS: The mean value of DD was (0.2643 +/- 0.1574) mm in the AcrySof MA60BM group, (0.3536 +/- 0.1719) mm in the Array SA40N group, (0.3099 +/- 0.1529) mm in the SA60AT/SA30AL group, and (0.3261 +/- 0.1877) mm in the Quattro group. The difference of the DD values among these 4 kinds of IOLs had no statistic significance (F = 0.506, P = 0.681) and the values were not equal to zero (t = 10.508, P < 0.001). CONCLUSION: The Photoshop method is a simple and efficient way for DD analysis. The modern foldable posterior-chamber-IOLs have good centration performances when implanted into the capsular bags.

Comparison of the accuracy of Canon KU-1 IOL measurer and VPLUS A/B scanner in axial length measurement.

PURPOSE: To evaluate the accuracy of Canon KU-1 IOL measurer (Japanese Canon Company) and VPLUS A/B scanner (French Quantel Company) in axial length (AL) measurement. METHODS: Canon KU-1 IOL measurer and VPLUS A/B scanner were used to measure axial length of human cataractous eyes before cataract surgery. Two hundred and twenty-two cases (433 eyes) were involved. The results were compared and the postoperative visual acuity, refractive results were recorded during the follow-ups to evaluate the accuracy of the two instruments. RESULTS: In the 222 cases (433 eyes), the absolute value of the measurement differences was 0.4 mm or above in 35 eyes, 0.8 mm or above in 17 eyes, 1.2 mm or above in 12 eyes, 2.0 mm or above in 5 eyes. The refractive error was less than 2.0 D in all patients. The mean values of ocular axial length by the two methods were 23.82 mm and 23.83 mm respectively and the difference had no statistic significance with compared t test (P = 0.902, two tail, alpha = 0.01). CONCLUSION: The accurate AL measurements can be obtained with the two instruments and the measurement results should be analyzed comprehensively to obtain accurate values in the complicated cases.

Efficacy of 2% lidocaine injection as a topical agent in cataract surgery.

PURPOSE: To determine whether 2% Lidocaine injection is an effective topical anesthetic agent for non-phaco small incision cataract surgery. SETTING: Charity eye clinic supported by Hellen Keller International. METHODS: One hundred and twenty-five consecutive cataract surgery patients received topical anesthesia with 2% Lidocaine injection solution just 1 and 0.5 minutes prior to non-phaco small incision cataract extraction and intraocular lens implantation. Each patient was asked about pain or pressure sensation during the operation. RESULTS: The surgeon felt ease in the operations. Many patients (93/125) were comfort during the whole surgery. Only 9 patients' score was above level 3, mostly complained during the nucleus extraction; Among those whose score was level 1-2, 82.6% (19/23) claimed discomfort at middle of the operation (nucleus extraction) or the beginning (creating the conjunctival flap). CONCLUSION: Lidocaine injection solution (2%) was an effective topical anesthesia agent in cataract surgery.

[Tear film changes after phacoemulsification].

OBJECTIVE: To evaluate tear film changes after phacoemulsification. METHODS: This prospective randomized study involved 68 consecutive patients with age-related cataract (79 eyes) who underwent phacoemulsification. Tear break-up time, Schirmer I test (S I t) value, corneal fluorescein staining, tear film pattern by DR-1 tear interfermetry and the height of tear meniscus were measured preoperatively and 1 day and 2, 7, 14, 30, 180 days postoperatively. RESULTS: At 1 day and 2 days postoperatively, the mean tear break-up time reduced greatly, and the number of patients who showed III to V tear film pattern, the mean S I t value, the height of tear meniscus and fluorescein staining scores increased significantly (P < 0.001 for day 1, P < 0.005 for day 2). S I t value at 7 days postoperatively (P = 0.831) and the height of tear meniscus at 14 days returned to their preoperative values (P = 1.000). The tear break-up time, corneal fluorescein staining scores and the grades of tear film pattern all recovered at 30 days postoperatively (P > 0.05). At 30 days postoperatively, 19.3% of patients showed shorter tear break-up time and less S I t value than that of preoperative day. In addition, 1/9 of the patients with normal preoperative tear film experienced dry eye at 30 days postoperatively. The cases with tear break-up time < 10 seconds compared to patients with tear break-up time >/= 10 seconds were more easily to show tear instability postoperatively (relative risk 13.5, 8.25, 20.0, 39.0, 8.6, 3.9 at 1 day and 2, 7, 14, 30, 180 postoperative days, respectively). CONCLUSION: Phacoemulsification significantly alters the tear break-up time, S I t value, corneal fluorescein staining, tear film pattern and the height of tear meniscus. Some patients with normal tear film may experience dry eye after surgery. Patients with preexisting tear break-up time < 10 seconds are easily at risk of experiencing the tear film instability postoperatively.

[A clinical observation of low and negative power intraocular lens implantation].

OBJECTIVE: To evaluate the clinical effects of the low power and negative power acrylic foldable intraocular lens (Acrysof IOL) implantation. METHODS: Forty patients (56 eyes) received phacoemulsification and above mentioned Acrysof IOL implantation. Intraoperative and postoperative complications, visual acuity and refractive error were observed. RESULTS: No intraoperative complications occurred. During 3 months of follow-up, the corrected visual acuity was less than 0.1 in 2 eyes, 0.1 - 0.4 in 12 eyes, 0.5 - 0.9 in 36 eyes, and 1.0 - 1.5 in 6 eyes. Posterior capsular opacification was found in 4 eyes (7.2%). Capsular distension was found in 1 eye (1.8%). No retinal detachment was seen. CONCLUSION: It is safe and effective for highly myopic patients with cataract to implant a low or negative power Acrysof IOL after phacoemulsification.
