[Self hearing recovery in infants with hearing loss after universal newborn hearing screening system].

OBJECTIVE: To analyze the reasons of hearing recovery of newborns and infants which were diagnosed hearing loss within 3 months old by universal newborn hearing screening (UNHS) system. METHOD: Newborns and infants born from Nov. 2001 to Jun. 2005 in Shanghai who failed UNHS were studied. Audiometric evaluations were performed in Shanghai Children's Hearing and Speech Center. Those with hearing loss were diagnosed within 3 months old and were followed-up at least 6 months. Analysis was carried out targeting for hearing recovery cases. RESULT: Six hundred and eighty-one cases failed the UNHS and were confirmed hearing loss within 3 months old. During the 6-24 months period of followed-up, 94 (13.8%) of 681 infants caught normal hearing. Among these 94 infants(115 ears), otitis media with effusion was found in 56 cases (59.6%), no reasonable cause was found in the other 38 cases (40.4%). Among the 115 ears which recovered to normal, there were 105 ears (91.3%) with primary diagnosis of mild hearing loss, 8 ears (7.0%) with moderate hearing loss and 2 ears (1.7%) with moderate-severe hearing loss. No recovery was found in the cases with primary diagnosis of severe and profound hearing loss. CONCLUSION: Newborns and infants with hearing loss diagnosed within 3 months old have possibility to recover as they are growing. The critical reasons might be self-recovery from otitis media with effusion and outcomes of central auditory system development.

[The thresholds comparison of auditory steady-state responses and auditory brain-stem response].

OBJECTIVE: To compare the thresholds of auditory brainstem response (ABR)and auditory steady-state responses (ASSR) in the same group of deaf children and give a clinical evaluation for ASSR after that. METHOD: Both kinds of thresholds have been recorded and analyzed of these deaf children. RESULT: The ABR thresholds are (85.82 +/- 12.39) and (82.70 +/- 14.93) dB nHL for left and right ears. At four testing frequencies of ASSR, the thresholds are (86.91 +/- 16.70), (90.32 +/- 16.11), (91.02 +/- 16.58), (89.80 +/- 17.08) and (85.15 +/- 18.16), (89.32 +/- 17.76), (90.41 +/- 18.87), (85.15 +/- 17.03) dB HL for left and right ears. The correlation coefficients between the ASSR and ABR of left and right ears are 0.622, 0.721, 0.757, 0.714 and 0.613, 0.732, 0.795, 0.739 at these frequencies respectively. These results showed significant correlation exiting. CONCLUSION: Reliable and frequency specific results could be obtained by ASSR so that it could be as a promising audiometry in clinic.

[Multi-frequency steady-state response for clinic audiometry].

OBJECTIVE: To obtain the residual hearing by using multi-frequency steady-state response test in a group of deaf children and to compare the results with that of ABR. METHOD: Both sides of ears of 278 deaf children had been tested while they were asleep. RESULT: Positive incidences of MFSSR at 0.5, 1, 2, 4 kHz are 33.09%, 70.50%, 44.96%, 19.42% (left ears) and 33.81%, 66.91%, 41.07%, 17.63% (right ears) respectively. Except 4 kHz, the ratios at the other three frequencies are significant higher than that of ABR. CONCLUSION: MFSSR is a promising objective audiometry, this kind of test could provide frequency-specific information for hearing diagnosis.

[Clinical analysis of primary squamous cell carcinoma of the breast].

OBJECTIVE: To study the clinicopathological features of primary squamous cell carcinoma (SCC) of breast and the diagnosis and treatment of the disease. METHODS: The clinical, operative and pathological data from 7 cases of SCC of breast were retrospectively analysed. RESULTS: Five patients complained of painless mass and two of them were accompanied with local pain. All patients were diagnosed as having SCC by fine-needle aspiration before operation. Two patients underwent modified radical mastectomy, and the others received radical mastectomy. Pathological examination showed primary squamous cell carcinoma of the breast with metastasis to axlliary lymph nodes in one patient. Follow-up of four patients revealed that two patients are living well and two patients died. Three patients lost to follow up. CONCLUSION: The diagnosis of primary squamous cell carcinoma of the breast is dependent on pathological results. Radical mastectomy including modified radical mastectomy is most effective in the treatment of mammary SCC. Postoperative chemotherapy and radiation are necessary auxiliary therapy.
