[Individual 3-D image prototyping models for orbital reconstruction].

OBJECTIVE: To fabricate a rapid prototyping (RP) 3-D image models for individual reconstruction of orbital bony loss. METHODS: The skull was placed on a helical CT scanner table and the Frankfort plane was perpendicular to the table. The CT data was obtained by a Aquilion (TOSHIBA, Japan) with 1 mm thickness section in spiral mode. By adjusting of CT threshold value and pixels in order to stack the segmental defects, we obtained an approximate 3-dimension visual model of the scanned skull using MedGraphics software. An orbital RP model based on the dataset of the 3-dimension visual model was fabricated by RP machine. Both 3-dimension visual model and RP model were measured against the skull with several anatomic landmarks to examine the accuracy of the models, and the errors were analysed. RESULTS: Integrity precision rapid RP models of the orbital region were reconstructed. The anterior orbital rim, middle orbital section and posterior orbital section were all fabricated. Optic foramen, superior orbital fissure, infraorbital foramen, inferior orbital fissure, lacrimal sac socket and naso-lacrimal duct were shown clearly. But some fine hole and slot, such as the anterior ethmoidal foramen, posterior ethmoidal foramen and zygomaticofrontal suture were not obviously seen. The mean difference between the 3-dimension visual model and the skull was 0.10 +/- 1.02mm. For the RP and dry skull, the mean difference was 0.22 +/- 1.04mm. There were no statistical differences between them. CONCLUSIONS: Integrity precision orbital RP models were fabricated which fulfilled the requirements of the individual reconstruction with bony orbital pathologic changes. The keys to fabricate the precision orbital RP models included a closer cooperation between the surgeon and engineer, thin CT slice in 1mm thick and an appropriated threshold value. Better results for the orbital deformities should be achieved for the contour of orbital region or eye function.

[Effect of hospice care on the quality of life and psychological state of aged mortal inpatients].

OBJECTIVE: To investigate the influence of hospice care on the life quality and psychological state of aged mortal inpatients. METHODS: Seventy-six aged patients were randomly divided into 2 groups (n=38). The research group was treated with hospice care, while the control group was treated with conventional nursing. Before and after more than one month of the interference, the quality of life, social support, satisfied degree, anxiety, and depression mood were investigated. RESULTS: All the indexes, such as total scale of life quality, appetite, spirit, sleep, family comprehension and coordination, recognition of cancer, attitude to therapy, and facial expression of the mortal inpatients after the interference, were raised and higher than those of the controls (P<0.05 or P<0.01). Life satisfaction index and the total scale of social support and subjective support, utilization ratio of support were raised and higher than those of the controls (P<0.05 or P<0.01). The level of anxiety and depression was decreased and lower than the control (P<0.05 or P<0.01). CONCLUSION: Hospice care can raise the life quality, subjective support, utilization ratio of support and life satisfaction of aged mortal inpatients. It could decrease the negative mood, such as anxiety and depression and improve the psychological state.

[Avoiding the interference of ABCD1 pseudogenes in the molecular diagnosis of X-linked adrenoleukodystrophy by double amplification refractory mutation system].

OBJECTIVE: To avoid the interference of ABCD1 pseudogenes, the amplification refractory mutation system (ARMS) was used to analyze the mutation of ABCD1 gene in the molecular diagnosis of X-linked adrenoleukodystrophy (ALD). METHODS: The upstream primers (wild primer and mutation primer) were designed according to the principle of primer-design in ARMS. In addition, a common downstream primer was designed in the same way to discriminate ABCD1 gene from its prologous pseudogenes. The genomic DNA isolated from the peripheral blood leukocytes of the family members and normal controls was amplified by PCR. RESULTS: In double ARMS, a specific product of 107bp could be amplified from genomic DNA of the patient with R617C mutation in ABCD1 gene and his mother, while the same product was not found when the genomic DNA of the patient's father and normal controls was used. Thus, the interference of ABCD1 pseudogenes in molecular diagnosis of ALD was excluded successfully at genomic DNA level. CONCLUSION: Double ARMS is a quick and effective method to eliminate the interference of the pseudogenes in detecting ABCD1 gene mutations.

[The effect of a vascular endothelial growth factor receptor inhibitor on airway inflammation and airway remodeling in a murine model of asthma].

OBJECTIVE: To investigate the expression of vascular endothelial growth factor (VEGF) and the effect of a VEGF receptor inhibitor on airway inflammation and remodeling in a murine model of asthma. METHODS: BALB/c mice were randomly divided into three groups: group A (control group, n = 10), group B (asthmatic model group, n = 10) and group C (VEGF receptor inhibitor group, n = 10). The asthmatic model was established by OVA sensitization and challenge. Bronchoalveolar lavage fluid (BALF) cell count and differential were carried out, VEGF levels in BALF and serum were measured by enzyme-linked-immunosorbent assay (ELISA), the pathologic changes of bronchi and the lung tissue were evaluated, and the expression of VEGF was analyzed with immunohistochemistry (ISH). The airway wall thickness (WAt/Pi), the bronchial smooth muscle thickness (WAm/Pi), the bronchial smooth nucleus counts (N/Pi) and vascular counts were measured by using image analysis system. RESULTS: There was a significant increase in the numbers of total cells and eosinophils (EOS) in the BALF of group B [(142 +/- 63) x 10(7)/L, 98.0 +/- 46.9 compared to those of group A (30 +/- 14) x 10(7)/L, 0.7 +/- 1.1, all P < 0.01]. After administration of the VEGF receptor inhibitor, the numbers of total cells and EOS were significantly decreased [(41 +/- 17) x 10(7)/L, 4.9 +/- 3.5, all P < 0.01]. BALF and serum levels of VEGF in group B were [(55 +/- 26) pg/ml, and (72 +/- 26) pg/ml] respectively. They were significantly higher than those in group A [(37 +/- 9) pg/ml, (49 +/- 18) pg/ml, respectively] and in group C [(34 +/- 3) pg/ml, (43 +/- 19) pg/ml, respectively]. ISH showed that expression of VEGF in group B was increased compared with group A and C. WAt/Pi, WAm/Pi, and N/Pi in group B [(17 +/- 5) microm(2)/microm, (6.3 +/- 2.2) microm(2)/microm, (0.050 +/- 0.020)/microm, respectively] were all significantly higher than those in group A [(8 +/- 3) microm(2)/microm, (3.2 +/- 0.8) microm(2)/microm, (0.027 +/- 0.017)/microm respectively, P < 0.01 and P < 0.05]. The vascular count of the airways in group B (19 +/- 3) was significantly increased compared to that of group A (10 +/- 5, P < 0.01). Administration of the VEGF receptor inhibitor reduced WAm/Pi (4.5 +/- 1.3) microm(2)/microm, P < 0.05) and the vascular count (11 +/- 3, P < 0.01). CONCLUSIONS: VEGF was over-expressed in the murine model of asthma, and involved in angiogenesis and airway remodeling. VEGF receptor inhibitors may be effective in reducing allergic airway inflammation and airway remodeling.

[Study on the current status and trend of food consumption among Chinese population].

OBJECTIVE: To describe the current status and trend of food consumption among Chinese residents. METHODS: 23,470 households including 68,962 subjects were randomly sampled from 132 counties in 31 provinces, autonomous regions and municipalities. A 24-hour dietary recall method for 3 consecutive days was used to collect information on food intake, but the condiment intake was collected by weighting method. RESULTS: The average food consumption per reference man per day of Chinese people included 402 g (urban/rural: 366 g/ 416 g) cereals; 276 g (urban/rural: 252 g/ 286 g) vegetables; 45 g (urban/rural:69 g/36 g) fruits; 79 g (urban/rural: 105 g/69 g) meats; 30 g (urban/rural:45 g/24 g) fishery products; 24 g (urban/rural: 33 g/20 g) eggs; 27 g(urban/rural: 66 g/ 11 g) dairy; 16 g legume; 42 g (urban/rural:44 g/41 g) edible oil; 12 g (urban/rural: 11 g/12 g) salt; 9 g (urban/rural: 11 g/8 g) soy sauce. CONCLUSIONS: For Chinese residents, the quality of diet had been improved and consumptions of animal products, milk, and edible oil had been increased, but the consumptions of dairy and legume products intake was still low which called for improvement. The unbalance was characterized as the downward trend in cereal, vegetable and fruit consumption but significant increased in animal products and oil intake among urban people. Dietary pattern of some subjects adopted a "high energy density" diet. Double burdens from under-nutrition and over-nutrition should both be considered as problems which asked for attention by the government and researchers.

[The clinical characteristics and its relationship with metabolic syndrome in elderly subjects with impaired glucose regulation].

OBJECTIVE: To study the clinical characteristics of impaired glucose regulation (IGR) in elderly subjects and its relationship with metabolic syndrome (MS). METHODS: The exploration of IGR in 2 810 Chongqing citizens over 40 years old was done by OGTT in a cross-section study. Normal glucose tolerance (NGT), IGR and diabetes (DM) were grouped based on the1999 diagnosis standard of WHO. IGR was composed of impaired fasting glucose (IFG), impaired glucose tolerance (IGT) and both of which. RESULTS: The prevalence of IGR was 18.11%, among which IGT (85.27%). Compared with the NGT group, the IGR group had higher age, body mass index (BMI), blood pressure, triglyceride (TG), total cholesterol (TC), low density lipoprotein cholesterol (LDL-c) and HOMA-IR, lower high density lipoprotein cholesterol (HDL-c) and HOMA-B. The IGR group had lower blood pressure, TG and HOMA-IR, and higher HOMA-B than the DM group. When each subgroup of IGR was compared with each other, both IFG plus IGT subgroup and IFG subgroup had higher BMI and HOMA-IR, and lower HOMA-B than IGT subgroup. The prevalences of hypertension, lipid disorder, obesity/overweight, and microalbuminuria in each subgroup of IGR were statistically higher than that of the NGT group. The prevalence of MS in the IFG plus IGT subgroup was higher than that of the IGT subgroup. CONCLUSIONS: The incidence of IGR was high in elderly people over 40 years old in local district of Chongqing city. There were various metabolic disorders in the subgroups of IGR. The IFG plus IGT and IFG group had higher BMI, hypertension, microalbuminuria and HOMA-IR, but lower HOMA-B than the IGT group.

[Cell biological mechanism involved in the effect of sodium selenite on improving insulin sensitivity].

OBJECTIVE: To study the cell biological mechanism of sodium selenite improving insulin sensitivity in pubertal rats with insulin resistance. METHODS: The content of inositol 1,4,5-trisphosphate (IP3) was examined by anion resin chromatography, and mRNA levels of phosphatidylinositol 3-kinase regulatory subunits (PI3Kp85 alpha) and Se-P were detected by RT-PCR in hepatocyte isolated from pubertal rats with insulin resistance. RESULTS: The mRNA levels of Se-P and PI3Kp85 alpha and content of IP3 in isolated hepatocyte decreased in pubertal male rats with insulin resistance. The above indices increased and reached normal level in rats supplied with selenium. The response to insulin stimulation in isolated hepatocyte in rats with selenium supply was similar to that in the control group, and both groups had higher response than those with high-fat diet. Alone when inhibited by wortmannin, the concentration of IP3 increased slightly in rats with selenium supply, but still was lower than that in the control group. CONCLUSIONS: These results indicate that the effect of selenium improving insulin sensitivity may be related to phosphatidylinositol PI3K signalling pathway. The effect of regulation of IP3 by selenium is not as effective as that by insulin, which may explain the difference of effect between selenium and insulin.
