Upregulation of serum and tissue vascular endothelial growth factor correlates with angiogenesis and prognosis of oral squamous cell carcinoma.

PURPOSE: To investigate both serum vascular endothelial growth factor (VEGF) concentrations and VEGF expression in patients with oral squamous cell carcinoma (OSCC), and to clarify whether upregulation of VEGF in OSCC patients is associated with increased microvessel density and various clinicopathologic features of patients with OSCC. PATIENTS AND METHODS: The study population included 31 patients with OSCC and 10 normal individuals. Concentration of serum VEGF was determined by using an ELISA kit. Immunohistochemistry was used to evaluate VEGF expression and microvessel density in OSCC. RESULTS: Our results showed that both serum VEGF levels and VEGF positivity were closely associated with both regional lymph node status and clinical stage of patients with OSCC. Increased microvessel density in oral cancer tissues was significantly higher in VEGF-positive tumors than VEGF-negative tumors. CONCLUSION: Our present study indicated that upregulation of VEGF in oral cancer was correlated with both tumor angiogenesis and disease severity.

A possible hypoxia-induced endothelial proliferation in the pathogenesis of epithelioid hemangioma.

Epithelioid hemangioma is an uncommon benign vascular lesion with distinct clinicopathological characteristics. Histologically, this entity mainly composed of well-formed but often immature vessels lined by epithelioid endothelial cells with prominent chronic inflammatory component. There is considerable controversy whether epithelioid hemangioma is a reactive lesion or a true neoplasm. We postulated that the local hypoxia may play a role in the pathogenesis of this vascular tumor. This local hypoxic condition may caused by many incidences such as congenital vascular malformation or trauma. The hypoxia will lead to the proliferation of endothelial cells and the formation of this vascular tumor, which may lead by putative stimulators VEGF and HIF-1. Hypoxia may also promote endothelial cell proliferation through the renin-angiotensin-aldosterone system. Additionally, the inflammatory cells including eosinophils and mast cells may contribute to the endothelial cell proliferation in EH. Further study investigating the associated factors of hypoxia may lead to new, potentially important insights into epithelioid hemangioma, and might also contribute to novel strategies for the management of this entity.

Coronal incision for treating zygomatic complex fractures.

OBJECTIVES: To study the value of coronal incisions for treating zygomatic complex fractures. PATIENTS AND METHODS: A retrospective study was conducted on 69 out of 83 patients regarding the indications and complications of scalp incisions for treating zygomatic complex fractures (the other 14 patients were treated by local incisions and approaches). RESULTS: In the early postoperative period, 5 patients suffered from haemorrhage, 2 had infections, 24 patients reported immediate postoperative anaesthesia and paraesthesia affecting the supraorbital region. Six had symptoms and signs of facial nerve injury: difficulty with wrinkling the forehead or to closing the eyes. After a follow-up of 3-5 years, 6 cases suffered from a scar wider than 0.5 cm, paraesthesia in 2 cases (parietal region and temporal region), depression of the temporal fossa in 2 and 1 patient had (persistent) palsy of the temporal branch of the facial nerve. CONCLUSION: On one hand, coronal incisions offer advantages such as: extensive exposure to ensure exact anatomical reduction. On the other hand, this incision has disadvantages such as obvious scars, long operating time, infections, haemorrhage, paraesthesia in the operative region, palsy of the facial nerve and depression of the temporal fossa. Therefore, the indications for coronal incisions should be strictly applied, and this incision should not be overused.

Eosinophilic granuloma of the jaws: an analysis of clinical and radiographic presentation.

Eosinophilic granuloma (EG) is the localized and mildest form of histiocytosis-X group of diseases. In this study, 22 patients of pathologically proven EG involving the jaws were retrospectively studied concerning the clinical and radiographic presentation. The clinical and radiographic presentation of the 22 cases was quite varied, and the accuracy of clinical and radiographic diagnosis was low. However, the clinical and radiographic presentation correlated with the lesion site, and accordingly all the jaw lesions could be divided into three types (Alveolar Type, Intraosseous Type, and Mixed Type). Our classification based on the lesion sites reveals the correlation between the clinical and radiographic presentation of EG and the lesion sites, and may be useful in the identification of lesions of EG in the jaws.

Abnormal union of mandibular fractures: a review of 84 cases.

PURPOSE: The purpose of this study was to investigate the characteristic and contributory factors of mandibular fractures with abnormal union in China, aiming to produce guidelines to facilitate the management of mandibular fractures so that this complication may be avoided. PATIENTS AND METHODS: In this retrospective review, the records of all patients who were identified as having abnormal union of mandibular fracture in a period of 10 years (October 1994 to October 2004) in our hospital were collected and analyzed for the features of the patients' age, gender, trauma type, fracture site, and type of abnormal union. At the same time, the data about treatment methods, antibiotics use, presence of infection, patients' compliance, patients' systemic disease, and personal habits such as alcohol or drug abuse were explored and analyzed to determine the main cause contributing to the development of abnormal union. RESULTS: Eighty-four patients met the inclusion criteria and were included in this study, and 31 to 40 years and 21 to 30 years were the common age groups. Traffic accidents were responsible for the majority of abnormal unions. A total of 135 abnormal unions were identified within the 84 patients, and 44 patients suffered more than one abnormal union. Inadequate unions were more frequent than malunions within the 135 abnormal unions, and most of the abnormal unions were in the condyle. The main causes contributing to the development of abnormal union included being untreated (n = 36), inexperience of the surgeon (n = 27) or patient's noncompliance (n = 6) during the process of treatment, and infection (n = 5). In our series, no patient with alcohol or drug abuse was observed. CONCLUSIONS: The characteristic of abnormal union is associated with patient's age and gender, severity and type of original trauma, and fracture site. In a sense, the development of abnormal unions is related to the social, economic, and cultural factors in China. At present the key problem of decreasing and avoiding mandibular abnormal unions in our country is to perform appropriate treatment methods in time. At the same time, it is important to obtain patients' compliance during the process of treatment and actively prevent infection.

Repair of mandible defect with tissue engineering bone in rabbits.

BACKGROUND: The aim of the present study was to investigate the effect of tissue engineering bone composed of bone marrow-derived osteoblasts and demineralized bone in repairing mandible defect. METHODS: Bone marrow-derived osteoblasts of 20 rabbits were cultured and seeded into scaffold of allogeneic demineralized bone to construct tissue engineering bone graft in vitro, which was used to repair the 10 x 5-mm bone defect made in the same rabbit mandible edge. Implant of demineralized bone alone was as the control. Rabbits were killed according to the schedule: five after 2 weeks, five after 4 weeks, five after 8 weeks, five after 12 weeks, and the implants were harvested for gross, radiographic, and histological observation. RESULTS: New bone formation at the margin region of defect and osteogenesis at the centre were observed in the implant of tissue engineering bone, and the bone formation pattern included osteogenesis, osteoconduction, and osteoinduction. In the implant of demineralized bone alone, the major bone formation pattern was 'creeping substitute'. CONCLUSIONS: The tissue engineering bone graft constructed by autogenous bone marrow-derived osteoblasts and allogeneic demineralized bone was better than demineralized bone alone in bone formation capability, which might be an ideal graft for bone defect repair.

[Function of disk reposition in the treatment of traumatogenic temporomandibular joint ankylosis].

OBJECTIVE: To investigate the method and results of disk reposition in the treatment of traumatogenic TMJ ankylosis. METHODS: In 19 cases of traumatogenic ankylosis of TMJ, the dislocated disks were repositioned during anthroplasty. In the operation, the dissection of dislocated disk was performed carefully around the TMJ. The disk was repositioned to its anatomic location over the top of condylar stump, then the lateral aspect of the disk was sutured to the soft tissue of zygomatic root. RESULTS: At the last follow-up examination, interincisal opening distance ranged from 24 to 43 mm in all cases (mean 32.63 mm), which approached or reached the normal level. No recurrence and TMJ symptom were found during the period of follow-up. CONCLUSIONS: The disk repositioning in treatment of traumatogenic TMJ ankylosis is a feasible and effective approach to reconstruct the structure and function and prevent recurrence.

[A clinical investigation on the surgical approach and fixation in operative treatment zygomatic fractures].

OBJECTIVE: To investigate the effect of soft tissue incision and rigid internal fixation in the treatment of zygomatic fracture. METHODS: 70 cases with zygomatic fracture were entered into the study. The clinical data of 65 patients who were performed operations in the treatment were analyzed. The study focused on the incision, number and locations of Titanium plate. The facial contour, reduction, fixation, healing of incision, facial bisymmetry and complications of the operation were followed up 3 - 24 months after surgery. RESULTS: All the incisions were primary healing. Clinical and radiologic assessment of reduction was symmetric and stable in 91.5% of all cases, asymmetric in 8.5% of all cases. Subcutaneous hematoma happened in 5 cases, Ti plate exposure 1 in case, scalp scar width was more than 0.5 cm in 6 cases, frontal part or scalp paresthesia 8 cases. CONCLUSIONS: Coronal scalp incision has advantages such as good exposure, precise removal of the bone block and good symmetry of both sides. RIF can improve the stability of fracture, promotes fracture healing and guarantees the rehabilitation of zygomatic complex. direct reduction and rigid internal fixation with Titanium microplate or miniplate are satisfactory in zygomatic fracture.

Effects of exogenous nitric oxide on oral squamous cell carcinoma: an in vitro study.

PURPOSE: Nitric oxide (NO) is a newly found unstable free radical gas, serving as an important mediator, messenger, and signal transduction molecule and involved in a variety of pathophysiologic processes. Recently, NO has been reported to have cytotoxic effects on several tumor cells as an effector molecule of activated macrophage. The objective of this study was to investigate the effects of exogenous NO on oral squamous cell carcinoma cell line and to try to clarify the possible mechanisms by which it kills tumor cells. METHODS: TSCCa cell line, established from a patient with oral squamous cell carcinoma of the tongue, was exposed to various concentrations of exogenous NO that were released from an NO donor, sodium nitroprusside (SNP), for 48 hours. Nitrite/nitrate levels in the culture supernatant were determined with a commercial available NO kit. Both morphologic and ultrastructural changes were evaluated by reverse phase contrast microscopy or transmission electron microscopy. The DNA was harvested from SNP-treated or untreated TSCCa cells and assessed by agarose gel electrophoresis. RESULTS: SNP released NO into medium in a dose-dependent manner. NO had a concentration-dependent cytotoxicity against TSCCa cells. NO induced tumor cell death through apoptosis, which was characterized by incompleteness of nuclear membrane, disappearance of nucleole and nuclear condensation, chromatin margination, or chromatin homogenization. Agarose gel electrophoresis showed a typical internucleosomal DNA cleavage pattern (DNA ladder), a reliable indicator of apoptosis. CONCLUSIONS: Our results suggested that NO had a tumoricidal potential against oral cancer cells. NO might exert its cytotoxicity as an effector molecule of activated microphage through at least apoptosis.
