Transconjunctival upper blepharoplasty.

Abstract

Transconjunctival lower lid blepharoplasty now has an established role as an option in rejuvenation of the lower eyelid. Transconjunctival upper lid blepharoplasty, or transconjunctival removal of medial upper eyelid fat, also has a role in rejuvenation of the upper eyelid. However, this is a rather limited role. We have found this approach safe and efficacious as a primary as well as a secondary procedure for removal of excess medial upper eyelid fat. We report on 20 patients who have undergone this operation: 5 as a primary procedure and 15 as secondary. There were no complications, no revisions, and the patients have been uniformly happy with their results.

Laser blepharoplasty: diamond laser scalpel compared to the free beam CO2 laser.

BACKGROUND: Blepharoplasty has long been employed for functional and cosmetic purposes. Different modalities have been utilized to incise and remove excess eyelid tissue in an attempt to improve surgical technique. OBJECTIVE: This study compares a free beam CO2 laser to a diamond surgical scalpel combined with a CO2 laser for performance of blepharoplasty. METHODS: Ten patients underwent bilateral upper lid blepharoplasty. One side was randomly assigned to the free beam CO2 laser technique, while the other side was performed with the diamond laser scalpel. Intraoperative surgical times and hemostasis were measured. Masked observers assessed postoperative parameters at day 1, week 1, week 2, week 4, and month 4. These assessments were analyzed clinically and statistically. RESULTS: The two modalities produced similar results for the intraoperative and postoperative parameters studied. CONCLUSION: The diamond laser scalpel has the advantage of tactile feedback and the option of use without coagulation.

Lymphoedema of the eyelids.

Lymphoedema of the eyelids interfering with vision can be a very debilitating condition. The aetiology is frequently unknown. Successful treatment by debulking and skin grafting has been described in a few cases. We present a case of sudden onset of massive bilateral eyelid oedema, which was treated using staged debulking without skin grafting. However, the cosmetic result was poor in comparison with skin grafting, suggesting that skin grafting may be a better way of treating this difficult problem.
