[Surgical therapy of arterial occlusions. Indication and results of revascularization in 5370 peripheral arteriosclerotic occlusions].

The results of 5370 revascularizations in peripheral arteriosclerotic occlusive diseases are reported. The operative mortality in 532 carotis interna desobliterations with ischaemia stage II was 2.3%. In stage I of 98 prophylactic operations we lost no patient. Instead of the transthoracic reconstruction we now prefer the extrathoracic bypass because of the lower risk involved. The abdominal aortic aneurysm always is an indication for resection because of the danger of rupture. In cases of 100 operated patients with aneurysm we had a mortality rate in the elective stage of 5.3%, in stage of covered rupture of 47.1% and in ruptured aneurysm 85.7%. The complications in vascular reconstruction of the lower extremity are low. The mortality rate after implantation of aortic bifemoral prosthesis is 3.7%, after an unilateral aortic-femoral prosthesis 2.8% and after all operations below the inguinal ligament 1.0%. Also the femoro-tibial occlusions could be revascularized in bypass-technique with a risk of 1.5% and good early results of 90.3%.

Aortic Aneurysm

[Diagnostic value of sonography (B-scan) in the upper abdominal region].

Minimal risk, easy applicability and high efficiency of ultrasonography, makes this method a valuable screening procedure in upper abdominal diagnostics. Case history, physical examination and laboratory diagnostics should be followed by ultrasound procedures as the first directed examination. This review intends to evaluate the efficacy of ultrasonography in different gastroenterological, oncological, hematological and angiological diseases of the upper abdomen.

Aortic Aneurysm

[The fibrin and plasma-protein content of human thrombi].

The protein composition of cell-free thrombus contained in abdominal aneurysms of the aorta was investigated using PAGE and immunoreactivity after solubilization. 1. The dry weight was 18--29%, with minor differences between various locations. 2. 35--40% of the dry weight was extractable with NaCl/citrate and was identified as albumin (80%) and IgG (15%). Trace amounts of fibrinogen immunoreactive material were present. 3. The NaCl/citrate insoluble part was 80% hydrolyzable with plasmin. The predominant fragment was D-D dimer (from cross-linked fibrin). In addition, fragments Dand E were observed. Reduction of the NaCl/citrate insoluble part with 2-mercaptoethanol resulted in almost complete solubilization. PAGE analysis demonstrated alpha-polmers, gamma-gamma dimers and little free alpha-chains, indicating almost complete cross-linking of fibrin. Thus, the major protein were cross-linked fibrin, albumin and IgG.
