The potential of electronic communication in medicine is assessed based on an analysis of a pilot project pertaining to internet based communication among referring and hospital physicians. Advantages of electronic data exchange in medicine pertain to speed and capacity for data transfer, availability of data and data integration, ultimately enabling consistent medical case management. Quality requirements of electronic communication of medical data are related to safety, availability, data integration, potential for case management and system qualities. Medical efficiency can be increased by use of electronic communication only if complex functions beyond the substitution of conventional mail by e-mail are implemented and an exhaustive use of the technology can be achieved.
As performance indicators and outcomes measures become essential parts of doing business, providers of mental health services are developing and using a number of access measures. One that is being used with increasing frequency is service penetration. However, the lack of standard methods for calculating and reporting service penetration has made the comparison of penetration rates cross studies difficult. This article discusses the conceptualization and operationalization of service penetration. In addition, it presents an exploratory study of service penetration using data from the same persons using very different data sources; these data were collected during an evaluation of a Medicaid managed care system in Florida. The article offers recommendations for the use and reporting of service penetration rates.
This paper presents the results from a longitudinal study in which the main purpose was to determine the health-care costs and financial consequences of changes in the health care demands related to tobacco consumption in Mexico. Eleven health interventions were selected to conduct this study and four probabilistic models were developed to forecast the expected changes in the epidemiologic profile of selected diseases. The costing method was based on the identification of case management costs using the instrumentation and consensus techniques, probabilistic models were designed using the Box-Jenkins technique and allowed us to identify the expected case trends for the 2001-2003 period. The generation of information on case management costs for the selected interventions is a central instrument in the planning of health programs, above all in that which refers to resource allocation by type of demand. On the other hand, the identification of expected cases and the financial consequences allowed us to know the growing trends of the sums required to satisfy health care demands for the period under study. The three types of information are a relevant resource for decision-makers in the production and financing of health services.
