Akin osteotomy with horizontal interosseous wire-loop fixation.

Abstract

The author has devised a new type of fixation for the Akin osteotomy. The fixation consists of double-stranded monofilament wire that is threaded through drill holes medially to laterally both proximal and distal to the osteotomy, encompassing four cortices. The fixation is placed prior to the cutting of the osteotomy, resulting in less chance of disruption to the lateral cortical hinge.

Isolated dorsal dislocation of the intermediate cunieform: a case report and review of the literature.

We present one very rare case of patient with an isolated dorsal dislocation of the middle cuneiform (closed injury) sustained after a fall. He was treated surgically through a dorsal approach and fixation with K-wires after manual reduction was unsuccessful. A cast was applied for a total of 6 weeks followed by active range-of-motion exercises. At the 18-month follow-up, he was asymptomatic.

Anteversion of the femoral neck in Indian dry femora.

The purpose of this study was to estimate the average angle of femoral neck anteversion in an Indian population. A total of 300 dry femora were classified by standard anatomical norms into male and female types and left-side and right-side types. They were evaluated by the Kingsley Olmsted and parallelograph methods, and the data were statistically analyzed. The average angles of anteversion obtained were 8.1 degrees and 8.3 degrees by the Kingsley Olmsted and parallelograph methods, respectively. The average female-type bone showed about 3 degrees more anteversion than the male-type bone. The average left-sided bone showed about 1.6 degrees more anteversion than the right-sided bone. A statistically significant difference existed between the sexes and the sides. The Kingsley Olmsted and parallelograph methods were found to be comparable with a correlation coefficient of 0.99. Altogether, 42.33%, 57.66%, and 79.00% of bones had readings of 5 degrees -10 degrees, 0 degrees -10 degrees, and 0 degrees -15 degrees, respectively. Thus, femoral neck anteversion has been found to be less in the Indian population than in Western populations.

Lower sternal reinforcement improves the stability of sternal closure.

BACKGROUND: This study uses a mechanical testing system to evaluate three methods of sternal closure. METHODS: Twelve sternal replicas composed of a polyurethane foam bone analogue were divided in the midline and reapproximated using three stainless steel wire techniques: six simple wires (6S), six figure-of-eight wires (6F8), or seven simple wires (7S), which included an extra wire at the lower sternum. The closures were subjected to increasing lateral distraction from 0 to 400 Newtons (N) (1 N = 0.224 lbs), and motion was measured using transducers stationed across the manubrium, midsternum, and lower sternum. RESULTS: With each method of closure, the manubrium was the most stable, the lower sternum the least stable, and the midsternum intermediate between the other two. There were also differences between sternal closure methods, but only at the lower sternum. Less sternal distraction was measured with the 7S than the 6S and 6F8 methods, starting at 100 N (0.20 +/- 0.06 mm vs 0.48 +/- 0.19 and 0.39 +/- 0.10, p = 0.003), and progressively increasing until the study was stopped at 400 N (1.64 +/- 0.39 mm vs 4.92 +/- 1.73 and 5.1 +/- 1.43 mm, p = 0.003). CONCLUSIONS: These data show that the lower sternum is the site of greatest instability and that reinforcement of this area with an additional wire effectively stabilizes the closure. Figure-of-eight wires are not superior to simple wires.
