Gastric polyps.

Abstract

Endoscopy allows a better diagnosis and treatment of gastric polyps. Current methodology includes a detailed examination of the stomach after good insufflation, necessary to diagnose small fundic polyps, biopsies of the polyp as well as of the surrounding mucosa (antrum and fundus). Endoscopy allows a preliminary diagnosis based on the number, location and size of the polyps. Resection is not performed at initial endoscopy as it is not necessary for all polyps and may be risky if vascular lesions are confused with polyps. Glandulocystic fundic polyps are the most frequent (50%), are always associated with a normal gastric mucosa and sometimes with omeprazole. Resection and follow-up are not indicated. Hyperplastic polyps represent 25% of gastric polyps, are located everywhere, with a mean size of 1 cm, and sometimes erosions or superficial necrosis. Gastritis is present, Helicobacter pylori is frequent and eradication may cause regression or disappearance of polyps. The small risk of cancer and the risk of bleeding increase with size. Polypectomy and follow-up are thus indicated in most cases. Adenomatous polyps, although similar in appearance to hyperplastic polyps concerning macroscopy and gastritis have a higher risk of cancer in the polyp or in the stomach. Polypectomy and follow-up are thus mandatory.

Sarcoidosis presenting as colonic polyposis: report of a case.

A 47-year-old, West Indian male was referred for investigation of mild iron-deficiency anemia. He was asymptomatic. Two years earlier, he had an episode of transient facial weakness and a separate episode of diplopia. Gastroscopy and duodenal biopsies were normal. Barium enema demonstrated multiple small polyps throughout the colon. At colonoscopy, these polyps had the appearance of adenomatous polyps. Histology revealed noncaseous epithelioid granulomas. There were no acid-fast bacilli, no intervening colitis, and no features of Crohn's disease on small-bowel radiology. Chest x-ray demonstrated bilateral hilar lymphadenopathy. A diagnosis of sarcoidosis with colonic involvement has been made. Sarcoid has been described at various sites in the gastrointestinal tract, presenting with stricturing or ulceration. There have been no previous reports of sarcoidosis presenting as discreet colonic polyps.

TGFbeta1 polymorphism (L10P) and risk of colorectal adenomatous and hyperplastic polyps.

BACKGROUND: Transforming growth factor-beta1 (TGFbeta1) is a multifunctional signalling molecule with a wide array of roles. Animal experiments suggest that TGFbeta1 plays a biphasic role in carcinogenesis by protecting against the early formation of benign epithelial growths, but promoting malignant transformation of those growths that do develop. A polymorphism in the signal peptide sequence of the TGFbeta1 gene (L10P) has been associated with increased levels of plasma TGFbeta1 in individuals with the P allele. METHODS: We investigated whether this polymorphism was associated with the risk of colorectal adenomatous or hyperplastic polyps in a case-control study of individuals from Minnesota. Risk of colorectal polyps was evaluated separately for individuals with adenomatous polyps (n = 513) and hyperplastic polyps (n = 191) relative to polyp-free controls (n = 606) using logistic regression analysis. RESULTS: No overall association was seen between the L10P polymorphism and risk of colorectal adenomatous polyps. The age- and sex-adjusted odds ratios (OR) of developing colorectal hyperplastic polyps were 1.0 (95% CI: 0.7, 1.4) and 0.7 (95% CI: 0.4, 1.1) for individuals with the LP and PP genotypes, respectively, compared with individuals with the LL genotype. When stratified by smoking, evidence for a decreased risk of hyperplastic polyps associated with the P allele was seen only among ever smokers (P for trend = 0.05). CONCLUSIONS: Whereas adenoma risk did not vary by TGFbeta1 L10P genotype, these results suggest that the L10P variant allele may have a protective role in the development of colorectal hyperplastic polyps, possibly consistent with its role as an inhibitor of epithelial growths.

Colonic surveillance by CT colonography using axial images only.

Patients at increased risk of colon cancer require strict colon surveillance. Our objective was to establish the efficacy of 2D axial CT colonography as a surveillance test when performed in routine clinical practice. Eighty-two patients at increased risk of colon cancer underwent CT colonography followed by conventional colonoscopy on the same morning. CT colonography studies were performed on a four-ring multidetector CT scanner (100 mAs, 120 kVp, 4 x 2.5 collimation) and were interpreted by two radiologists using 2D axial images only. Results were correlated with findings at colonoscopy. Note was made of subsequent histology reports from polypectomy specimens. A total of 52 polyps were detected at colonoscopy. Using 2D axial images alone, with no recourse to 2D multiplanar or 3D views, the sensitivity of CT colonography was 100, 33 and 19% for polyps larger than 9, 6-9 and smaller than 6 mm, respectively. Per-patient specificities were 98.8, 96 and 81.5%, respectively. Twenty-nine percent of polyps smaller than 1 cm were adenomatous and there were no histological features of severe dysplasia. CT colonography is a useful colon surveillance tool for patients at increased risk of colon cancer. It has a high specificity for identifying patients who should proceed to colonoscopy and polypectomy, while allowing further colon examination to be deferred in patients with normal studies. Using 2D axial images only, CT colonography can be performed as part of the daily CT workload, with a very low rate of referral for unnecessary colonoscopy.
