Abstract

Transthoracic needle biopsy (TNB) has become the diagnostic procedure of choice in evaluation of focal chest lesions. Both advances in cross-sectional image guidance and cytopathologic techniques allow TNB to accurately diagnose malignancy and characterize a spectrum of benign conditions. Image-guided percutaneous drainage of intrathoracic collections has developed as an extension of similar procedures in the abdomen and pelvis. The ability of CT and ultrasound to accurately detect and characterize parenchymal and pleural collections, and advances in interventional techniques and catheter design, have made percutaneous catheter drainage the treatment of choice for a variety of intrathoracic collections. This article provides an updated review of the spectrum of image-guided diagnostic and therapeutic procedures in the thorax.

Confidence intervals for the mean of a variable taking the values 0,1 and 2.

The problem of setting a confidence interval for the mean of a variable on (0, 1,2) arises in three contexts: paired organ procedure rates; estimation of gene frequency, and three-point Likert scales. An effective approach to its estimation may be based on a good interval for a paired difference of proportions. Four good methods due to Newcombe and Tango are shown to have much better performance than more naive methods over regions of the parameter space corresponding to the above applications.

Detection of isolated tumor cells in bone marrow is an independent prognostic factor in breast cancer.

PURPOSE: This study was performed to disclose the clinical impact of isolated tumor cell (ITC) detection in bone marrow (BM) in breast cancer. PATIENTS AND METHODS: BM aspirates were collected from 817 patients at primary surgery. Tumor cells in BM were detected by immunocytochemistry using anticytokeratin antibodies (AE1/AE3). Analyses of the primary tumor included histologic grading, vascular invasion, and immunohistochemical detection of c-erbB-2, cathepsin D, p53, and estrogen receptor (ER)/progesterone receptor (PgR) expression. These analyses were compared with clinical outcome. The median follow-up was 49 months. RESULTS: ITC were detected in 13.2% of the patients. The detection rate rose with increasing tumor size (P =.011) and lymph node involvement (P <.001). Systemic relapse and death from breast cancer occurred in 31.7% and 26.9% of the BM-positive patients versus 13.7% and 10.9% of BM-negative patients, respectively (P <.001). Analyzing node-positive and node-negative patients separately, ITC positivity was associated with poor prognosis in the node-positive group and in node-negative patients not receiving adjuvant therapy (T1N0). In multivariate analysis, ITC in BM was an independent prognostic factor together with node, tumor, and ER/PgR status, histologic grade, and vascular invasion. In separate analysis of the T1N0 patients, histologic grade was independently associated with both distant disease-free survival (DDFS) and breast cancer-specific survival (BCSS), ITC detection was associated with BCSS, and vascular invasion was associated with DDFS. CONCLUSION: ITC in BM is an independent predictor of DDFS and BCSS. An unfavorable prognosis was observed for node-positive patients and for node-negative patients not receiving systemic therapy. A combination of several independent prognostic factors can classify subgroups of patients into excellent and high-risk prognosis groups.

Blind and ultrasound-guided percutaneous liver biopsy in children.

BACKGROUND: Percutaneous biopsy of the liver is the most commonly used procedure to obtain tissue for histopathological assessment of liver disease. Although, intuitively, image-guided liver biopsy might be expected to reduce the risk of bleeding, haematoma caused by a penetrating injury of a branch of the hepatic artery or portal vein, and puncture of the gallbladder, no trial has been large enough to show reduced mortality or morbidity with US guidance, and the mechanisms by which the use of US can reduce the risk of bleeding remain speculative. OBJECTIVE: To compare the mortality and morbidity of blind liver biopsy with that of US-guided liver biopsy. MATERIALS AND METHODS: A retrospective review of our experience of 140 procedures over a 16-month period. RESULTS: In the blind group, biopsy was unsuccessful in ten children (95% CL 7.3-25.4); no tissue was obtained in eight children and an inadequate sample was obtained in two. Three children (95% CL 9.2-14.7) suffered significant haemorrhage (indicated by a drop in haemoglobin of >20 g/l) with intrahepatic ( n=1) and subcapsular ( n=2) haematomas detectable by US after biopsy. An adequate sample was obtained in all children in the US-guided group. There were no complications requiring treatment in either group. CONCLUSIONS: Our results showed a significant difference in the complication rate between liver biopsy undertaken with US guidance and liver biopsy performed blind ( P=0.002).
