Foundation restorations in fixed prosthodontics: current knowledge and future needs.

Abstract

PURPOSE: The Ad Hoc Committee on Research in Fixed Prosthodontics established by the Academy of Fixed Prosthodontics publishes a yearly comprehensive literature review on a selected topic. The subject for this year is foundation restorations. METHODS: Literature of various in vitro and in vivo investigations that included technical and clinical articles was reviewed to provide clinical guidelines for the dentist when selecting methods and materials for restoration of structurally compromised teeth. Topics discussed and critically reviewed include: (1) desirable features of foundation restorations, (2) foundations for pulpless teeth, (3) historic perspectives, (4) cast posts and cores, (5) role of the ferrule effect, (6) prefabricated posts, (7) direct cores, (8) foundation restorations for severely compromised teeth, (9) problems and limitations, (10) future needs, and (11) directions for future research. CONCLUSION: This comprehensive review brings together literature from a variety of in vitro and in vivo studies, along with technique articles and clinical reports to provide meaningful guidelines for the dentist when selecting methods and materials for the restoration of structurally compromised teeth.

Fabrication of custom clips for bar-and-clip attachments for implant-supported overdentures.

Bar-and-clip attachments for dental implants are a common and versatile device for improving the retention and stability of a removable prosthesis. Manufactured clips may be unsubstantial and may not always fit or provide the desired amount of retention for the bar. The purpose of this article is to describe a technique for custom fabrication of clip attachments. The wax pattern of the clips is fabricated and then cast in dental gold alloy. The gold clips are then cut to an appropriate size that will allow for a variety of clinical applications.

[Effects of laser welding on bond of porcelain fused cast pure titanium].

OBJECTIVE: To investigate the influence of the laser welding on bond of porcelain fused to cast pure titanium. METHODS: Twenty cast titanium plates were divided into two groups: laser welded group and control group. The low-fusing porcelain was fused to the laser welded cast pure titanium plates at fusion zone. The bond strength of the porcelain to laser welded cast pure titanium was measured by the three-point bending test. The interface of titanium and porcelain was investigated by scanning electron microscopy (SEM) and energy depressive X-ray detector (EDX). The non-welded titanium plates were used as comparison. RESULTS: No significant difference of the bond strength was found between laser-welded samples [(46.85 +/- 0.76) MPa] and the controls [(41.71 +/- 0.55) MPa] (P > 0.05). The SEM displayed the interface presented similar irregularities with a predominance. The titanium diffused to low-fusing porcelain, while silicon and aluminum diffused to titanium basement. CONCLUSIONS: Laser welding does not affect low-fusing porcelain fused to pure titanium.
