Planning system for computer assisted total knee replacement.

Abstract

Total knee replacement (TKR) is a common orthopaedic surgical intervention and includes the removal of bone sections from the end of the femur and the top of the tibia for replacement by prosthetic components. Pain relief and functional improvement are predictable clinical results. But the accuracy of the alignment affects the surgical outcome and the longevity of the prosthesis. Hence, current total knee implantation systems attempt to align the knee joint in the mechanical axis for placement of the total knee components. These approaches use templates and plain radiographs for preoperative planning and alignment devices for bone cuts. To overcome the inherent inaccuracy of the presently used systems a computer-assisted planning system has been developed delivering the necessary control data for the intraoperative surgical robot system.

Chromosomal changes in a dedifferentiated chondrosarcoma: a case report and review of the literature.

The chromosome abnormalities observed in a dedifferentiated chondrosarcoma are reported. A new molecular cytogenetic technique, spectral karyotyping, was used to identify and confirm structural rearrangements in this case. A review of the literature revealed that nine cases have been reported, in eight of which a complete description of the cytogenetic abnormalities was described. Structural aberrations were most frequently reported in chromosomes 1 and 9, and chromosomes 7 and 19 were most frequently observed to be involved in numerical aberrations (trisomy and tetrasomy). In chondrosarcomas, structural aberrations in chromosomes 1 and 9 and trisomy or tetrasomy of chromosome 7 are among the more frequently observed aberrations.

The patella in total knee replacement: technical aspects on the femoral side.

The influence of femoral component positioning on patellar tracking is critical. The various possibilities of modifying the position of the femoral component relative to the distal femoral epiphysis are analyzed, and the ideal positioning in the medio-lateral, antero-posterior, and proximal-distal directions is described. Special emphasis is placed on the rotational positioning of the femoral component.

The patella in total knee arthroplasty: resurfacing or nonresurfacing of patella.

Data from the Swedish Knee Arthroplasty Registry were analyzed to compare bi- and tricompartmental knee arthroplasties carried out in patients operated on for arthrosis in 1990-1996. Of the 16,607 primary arthroplasties that were carried out there were 5,139 with patellar replacement in the primary procedure and 10,928 without. By April 1998, 280 revisions were performed, 250 of these cases were analyzed in this study. Patella-related complications were commonly the reason for early revision: in 99 of the 168 knees with a primary bicompartmental procedure and in 36 of the 82 knees with a primary tricompartmental procedure. This presentation merely analyzes the extent of patellar problems in knee arthroplasty, as a detailed analysis of the causes of this common problem is not possible using data from a national multicenter study.
