Improving communication between care settings.

Abstract

A study investigating the exchange of information between primary care and hospital staff found no documented evidence that it occurred. Hospital staff acquired most of the necessary information about pre-admission care from the patients themselves. Managers in both settings need to implement measures to ensure the exchange of such information.
